KNEE. EE LOST TIME ON 6-1-02 BECAUSE OF THIS ACCIDENT.
EE STATES ON 4-30-02 WHILE CLEANING THE COAL FEEDER WITH A SCOOP, A LOADED SHUTTLE CAR STRUCK HIS BUCKET OF THE SCOOP CAUSING THE EE TO STRIKE HIS HEAD ON THE SCOOP CANOPY. EE BEGAN MISSING TIME FROM WORK ON 5/24/02. WORK SCHEDULE IS ON A4ON 4OFF CYCLE AND THE MINE WAS IDLE FOR 8 DAYS BECAUSE OF FULL STOCK PILES AND NO SHIPMENT OF COAL.
EE STATED HE WAS OPERATING A 4-WHEEL RUBBER TIRE VEHICLE. THE STEERING WHEEL SPUN AROUND & STRUCK HIM ON BACK OF HAND & FINGER. DIDN'T GO TO DR. UNTIL 4-25-02.
THE WHEEL OF A BROKEN DOWN TRUCK WAS SCOTCHED WITH A ROCK, WHEN THE TRUCK WAS BEING TOWED, THE ROCK EJECTED AND HIT ANKLE.
THE VICTIM TRAMMED A SCOOP TO A PALLET OF ROCKDUST. HE POSITIONED HIMSELF BETWEEN SCOOP AND ROCKDUST SO AS TO LOAD DUST ONTO BATTERY END OF SCOOP. WHILE LANDING ROCKDUST THE SCOOP DRIFTED BACK WARDS MASHING HIS LEGS BETWEEN SCOOP AND PALLET.
OPERATOR STATED HE WAS IN THE PROCESS OF DUMPING COAL AT FEEDER WHEN HE TURNED TO CHANGE DIRECTION ON SHUTTLE CAR. HE FELT PAIN IN LOWER BACK. E.R. VISIT REFERRED TO MEDICAL SPECIALIST. HE WAS PLACED IN NON-WORKING STATUS ON 9/20/01.
EE WAS OPERATING 785 ROCK TRUCK & THE SEAT BROKE CAUSING LOW BACK PAIN. HE WORKED UNTIL 2/11/02 & WENT TO THE DOCTOR. ON 2/14/02 WHEN RETURNING ON HIS NEXT SCHEDULED SHIFT HE BEGAN WORKING RES TRICTED DUTY (EE WORKS A 4 ON, 2 OFF SCHEDULE). HE IS CURRENTLY WORKING BUT IS ON RESTRICTED DUTY WHILE GETTING THERAPY TREATMENTS.
EE WAS OPERATING SHUTTLE CAR WHILE TRAMMING TOWARD FACE HE HIT A OFFSET IN BOTTOM, BOUNCED UP IN SEAT WHEN HE FELT PAIN IN LOWER BACK. ER VISIT RETURNED TO WORK WITH NO RESTRICTIONS WORKED UNT IL 6-30-01 AT WHICH TIME THE MEDICAL SPECIALIST REMOVED HIM FROM WORK.
EE WAS TRANSPORTING A PIECE OF SHEET METLA IN THE OUTBY END OF A TRACK OPERATED PERSONNEL CARRIER. THE METAL VIBRATED LOOSE, STRUCK COAL RIB AT A CLOSE CLEARANCE AREA, CAME AROUND AND STRUCK V ICTIM.
EE WAS HOOKING A STRAP ONTO PIPE IN POND DURING CLEANING. OPERATOR OF EXCAVATOR LEFT HYDRAULICS ON DURING PROCESS OF GOING TO HELP EE.IN DOING SO THE BUCKET WENT INTO EE'S LEG ABOUT BOOT HIGH
INJURED WAS PUTTING A NEW SET OF BOLTS ON A VIBRATING SCREEN AND AS HE WAS TURNING THE SHEEVE WHEEL WITH A PIPE WRENCH, THE BELT WENT ON THE SHEEVE WHEEL. IT TURNED BACKWARDS AND CAUGHT HIS FI NGER ON THE MECHANISM CUTTING HIS FINGER.
EMPLOYEE WAS OPERATING A 992 G LOADER AND WHEN HE WENT INTO SHOT MATERIAL TO GET A BUCKET HE FELT PAIN IN HIS NECK.
EE STATES HE WAS RIDING W MANTRIP TO SECTION WHEN MANTRIP WENT THRU DEEP AND BETWEEN OUT CAUSING PAIN IN LOWER BACK ALLEGED ACCIDENT ON 10-10-00 WAS TREATED AS FIRST AID. EE HAS HAD BACK SURGR Y FOR PREVIOUS ACCIDENT ON JAN 1995. INCIDENT WAS TREATED AS RECORRAUCNE BY MEDICAL PERSONAL OF OLD INJURY ON 11-14-00. MEDICAL SPECIALIST MADE ASSEMBENT THAT THERE WAS A NEW INJURY AT WHICH
EE WAS OPERATING A RUPPER TIRE FOUR WHEELER, WHEN TRAMMING THRU A WATER HOLE FRONT TIRE CAUGHT ON ROCK CAUSING THE STEERING WHEEL TO JERK HITTING HIS MIDDLE FINGER ON RIGHT HAND. WENT TO ER. X-RAY REVEALED A POSSIBLE NON DISPLACED FRACTURE OF MIDDLE FINGER.
EMPLOYEE WAS RIDING A RAIL MOTOR TOWARDS THE OUTSIDE WHEN HE FELT A PIECE OF DUST IN HIS LEFT EYE. THE MANTRIP WAS STOPPED AT THE 15TH X-CUT &ANOTHER EMPLOYEE LOOKED IN HIS EYE BUT DID NOT SEE ANYTHING. THE EMPLOYEE WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS.
EE STATED HE WAS OPERATING A SHUTTLE CAR WHEN A PIECE OF DRAW ROCK STRUCK HIM ON THE HEAD. MINING HEIGHT WAS LOSS THAN 42" CAUSING LACERATION TO HIS HEAD, AND EAR.
COAL TRUCK WAS UNLEVEL WHEN TRYING TO DUMP.
EE WAS ATTEMPTING TO MOVE 30 RAILROAD CARS (LOADED) WITH A LOCOMOTIVE, WHEN HAND BRAKE WAS LOOSED THE CARS STARTED TO MOVE. INJURED GOT ON CAR AND FELL IN FRONT OF WHEEL RESULTING IN INJURY TO HIS LOWER LEFT FOOT AND LEG. LOWER LEG WAS AMPUTATED 4/21/00.
EE STATED HE WAS SCOOPING ENTRY--ROCK FELL OUT BETWEEN BOLTS STRIKING HIM ON NECK & SHOULDERS. (WENT TO DR. ON 5-23-00)(STARTED LOSING TIME ON 5-24-00)
EMLOYEE STATED HE WAS RUNNING EMPTY FROM DUMP. SAID HE HIT A PLACE IN ROAD SIGHTLY ROUGHER THAN NORMAL. THIS CAUSED A JERKING MOTION, TO HIS NECK. HE WAS WEARING A SEATBELT.
THE 5&6 BELTA SHUT DOWN AND VICTIM TELEPHONE OUTSIDE AND WAS TOLD PROBLEM WAS AT #5 DRIVE UPON ASSIVING AT THE #5 DRIVE HE FOUND THAT COAL AND ROCK HAD PILE UNDER THE DRIVE ACTIVATING THE GOH SWITCH THUS SHUTING DOWN THE BELTS. THE VICTIM TOOK A PIECE F DRILL STEEL AND BEGIN PUNCHING THE MATERIAL LOOSE DOWN ONTO #4 BELT WHEN SUDDENLY SOME OF THE ROCK SLID DOWN ONTO STEEL PUSHING I
EMPLOYEE STATED HE WAS TRAMMING THE #4 MBC INTO THE FACE OF #1 HEADING- THERE WAS A ROCK IN THE ENTRY THAT THE MBC TRAMMED UPON- THEREFORE CAUSING THE DOCK TO RAISE- BUMPED HIS HEAD AGAINST TH E TOP AND BACK OF HIS HEAD AGAINST THE DECK, CUTTING THE BACK OF HIS HEAD BELOW HIS HARD HAT.
