Employee was a passenger in a mantrip that picked a switch and derailed. Employee struck head and right shoulder on the inside of the passenger compartment of the mantrip. Injury was not the result of a training or compliance issue.

Employee was attempting to clean structure with a wedge. The wedge accidentally got pulled between the top belt and wing roller causing their arm to be caught between the top belt and wing roller causing injury to their arm.

Employee was operating a locomotive. The equipment came off of the track (D-railed) causing it to hit the ground. They braced to hit the ground by putting a hand on the frame of the windshield. The windshield bounced up and pinched their fingers causing a fractured middle finger.

Employee was helping offload relay bar extensions and some were on ground. The duck bill was tipped down and on top of one of the bars. The spotter told the scoop operator to tip up and there was a miscommunication. The operator backed up and the duckbill drug the bar and flipped it over accidentally striking employee in the hip and leg (fracture).

Employee was traveling inby on the BMX Supply track. EE stopped at the shop switch and began to exit EE's mantrip. The mantrip following EE inby, hit EE's parked mantrip causing EE to fall back and hit EE's lower back and right elbow on the re-rail control box. EE was diagnosed with a fractured rib and transverse process fracture.

A set of supply cars, that were chained down to the track, accidentally became uncoupled from a motor and shifted forward towards the end of the track. At the same time, the injured employee reached for toolbag that was on the inby end of the supply cars and got upper calf caught between the frame of the track stop and the supply car causing injury.

An employee was traveling inby on 3 East Mains track haulage. 109 wall switch was left for the turn and EE's mantrip traveled into the switch and struck a parked motor. EE hit their rib area in the operator's compartment. Diagnosed with three fractured ribs.

Employee suffered a compound fracture to the right forearm in the H4 section. They were cleaning up the coal pile when employee was injured by a piece of mining equipment. The exact location and cause of injury is still under investigation.

A Bailey Prep Plant warehouse employee was operating a forklift when the employee inadvertently hit a center strap that was located on a pallet. The center strap kicked up and struck employee's right-hand, ring finger. Employee finished shift.

While pulling the rails with the mantrip the clevis came loose and became a projectile. The employee was struck in lower left leg by the clevis causing fractures to the leg.

Employee was cleaning around the tail roller of an air conveyor and caused injury to hand.

Employee was transporting supplies through the mine on a motor, when the supplies caught an air hose hanging against the mine roof. The hose fell and hit the employee in the face. Surgery for a deviated septum on 6/7/22.

Employee was standing next to the can setter controls on the scoop with leg bent and foot against the rib causing a knee to stick out. Employee was injured when scoop bucket swung towards the rib causing injury to the knee.

Employee was attempting to throw a rock on a belt when a splice caught EE's glove causing EE's thumb to be caught between the roller and the belt causing injury.

Employee was riding in the mantrip and hit EE's head on the SCSR cache causing injury

EE was hauling material to dump area in the A40F Volvo Rock Truck. EE hit a small bump in the road and hurt EE's lower back and hip. EE notified another EE. An incident report was completed and EE was then taken to MedExpress.

While operating a locomotive, the windshield on the locomotive inadvertently flipped up from the down position. The injured employee's fingers were resting on the hinge of the window frame and were pinched by the window frame.

While switching out cars located in 1 South 4 wall track switch, the injured employee thought the switch was thrown for the strait. The cars unexpectedly went into the switch, made contact with the employee, resulting in four staples to the head and a fractured vertebra.

Employee was sumping the miner in when a 12" cc bolt sprung loose off of the top of the miner, striking the employee in the mouth chipping EE's front two teeth.

Employee was trying to get the winch rope out from behind the ram of the scoop. When the ram of scoop was pulled back, employee got fingers caught between the bucket and ram of scoop causing injury.

Employee was Pre-shifting the F29 Longwall belt at 6 1/2 wall walkway side, when left arm was injured by a moving roller.

Employee was loading cable into scoop bucket and foot got smashed under bucket causing injury.

Injured was driving a mantrip inby on the E-track when a derailment occurred causing the injured to strike EE's left side and face against the frame of the mantrip resulting to chipping of the 8th and 10th teeth and 7 sutures to EE's mouth.

A miner was walking in the parking lot when EE was bumped by the right front side of a pickup truck traveling at a low rate of speed causing EE to fall and strike the back of head. EE was taken by ambulance to a local hospital of EE's choosing and was treated for a laceration to the back of the head that required 10 staples and was released.

Employee was traveling inby on the E-track when the mantrip EE was operating derailed causing EE to strike EE's left rib area in operator's compartment of the mantrip. Was diagnosed with a non-displaced fracture of the 7th rib.

Employee was a passenger in a mantrip when it de-railed. Employee claims that EE struck head on the roof of the mantrip. The accident is currently under investigation. The employee was considered a lost time based on information received on 12/18/17.

Employee was helping offload cans off a car. When the scoop backed up, it hid a bedrail that swung around and employee in the foot causing an injury.
