CAUSED OF THIS INJURY WAS USING UNSAFE MATERIAL CHAIN COMPLAINCE WITH RULES REG WAS NOT A FACTOR, PROTECTIVE EQUIP OK, MINER KNOWLEDGE A FACTOR, SYSTEM FACT.
ALLEDGED ACCIDENT WAS TO OCCUR WHILE DRIVING THE REFUUSH TRUCK DURING HIS SHIFT. HE STATED THAT HIS BACK WAS STIFFENING UP THROUGH OUT THE SHIFT AND WHEN HE GOT HOME HIS DISCOMFORT INCREASED. HE CLISM THAT THE BOUNCING OF THE TRUCK CAUSED THIS STRAIN. CAUSE OF THIS INJURY IS UNKOWN, COMPLIANCE OK, MINER PROFICIENCY FACTOR, PROTECTIVE EQUIP OK, EQUIP OK.
THE EE WAS OPERATING A 3 WHEELER. THE MACHINE HIT A ROCK CAUSIG THE FRONT WHEEL TO TURN RADICALLY WHICH IN TURN CAUSED THE STEERING WHEEL TO SPIN STRIKING THE EE ON THE RIGHT HAND. HE BEGAN LOSING TIME ON 6/9/97.
EE WAS TRAMMING A SHUTTLE CAR HIT A BUMP IN THE ROADWYA, WHEN THIS HAPPENED THE SEAT CAME OFF THE HOLDER AND HE JARRED HIS BACK. CAUSED OF THIS INJURY WAS LOCKING MECHANISM ON SEAT INOPERATIVE . COMPLIANCE WITH RULES/REGS WAS NOT A FACTOR, PROTECTIVE EQUIPMENT WAS INOPERATIVE, MINER KNOWLEDGE A FACTOR, MINING EQUIP/SYSTEMS A FACTOR.

EMPLOYEE CLAIMS THAT WHILE DRIVING THE REFUSE TRUCK HE HIT A HOLE AND JARRED HIS NECK. CAUSE OF THIS ACCIDENT WAS DRIVING CONDITIONS AND SPEED.
EE WAS ON TOP OF THE BELT WELDING ON A BELT CHUTE. THE BELT MOVED AND HE JUMPED OFF THE BELT AND TWISTED HIS KNEE. CAUSE OF THE INJURY WAS ANOTHER EE TRYING TO START THE BELT.
EMPLOYEE WAS IN THE PROCESS OF SCOOPING A X-CUT OUT PRIOR TO A MOVE ON WEEKEND. THERE WAS GOB AND LOOSS BLOCK TO SCOOP OUT OF X-CUT. WHLE PUTTING DOWN PRESSURE ON SCOOP BUCKET, CAUSED KITCHE N COMPARTMENT TO RAISE CATCHING EMPLOYEES HEAD BETWEEN COMPARTMENT AND ROOF.
EE WAS RE-RAILING A 10 TON BATTERY MOTOR BACK ONTO THE TRACK. HE USED A 10 TON SIMPLEX JACK TO PUSH THE MOTOR OVER. WHEN THE MOTOR STARTED TO GO, THE SIMPLEX JACK FELL AND STRUCK HIS FOOT.
EE TWISTED HIS BODY AND FELT A SHARP PAIN IN LOWER BACK.
ALLEGED INCIDENT WAS TO OCCURR WHILE OPERATING HIS SHUTTLE CAR GOING THRU ROUGH ROADS. HE SAID THE SHUTTLE CAR FELL INTO A RUT AND WHEN THIS HAPPENED HE STRUCK HIS HEAD AGAINST THE CANOPY. CAU SE OF THIS INJURY WAS AWARENESS AND FILLING IN HOLES ON TRAMROAD. COMPLIANCE WITH RULES/REGS WAS NOT A FACTOR. PROTECTIVE EQUIPMENT NOT WORN (BACK SUPPORT). MINER PROFICIENCY.
