Investigation is still going on at this time. Per preliminary report: Descending the coal haul road, victim began to lose brakes. He attempted to straddle an axle berm on his left. The truck ran up onto the berm for approximately 100' before it turned over. Victim was found partly out of the passenger side door beneath the right exaust stack. Unable to remove him until EMS arrived.

EE was walking from one crosscut outby the face in the #2 entry toward the face of the #2 entry when he was struck by the #1 shuttle car.

Was loading rock out of #7 face. The shuttle car was making the turn out of the entry. EE was caught between the shuttle car and the rib.

Truck out of control, driver not wearing seatbelt, ejected & crushed.

Employee was assisting in the movement of supplies at the Justice Mine of Independence Coal Company, Inc. During this process he was apparently pinned between a parts car loaded on a rail car and a rib near the first left switch.

EE was backing (rear lights on) on a 636 Long Airdox Shield Hauler 1 break out by the long wall face at cross cut #88 into the #3 left cross cut where he ran over an airlock door top beam which was hidden in the mud. The rear tire hit the beam resulting in the end rising, and entering the operator's compartment causing crushing injuries.

PORTAL VAN PULLED IN FRONT OF A 190 EUCLID ROCK TRUCK. ROCK TRUCK PULLED INTO VAN, CRUSHING THE VAN. IT WAS DARK AT THE TIME OF THE ACCIDENT.

EMPLOYEE WAS OPERATING A SUPPLY LOCOMOTIVE. THE LOCOMOTIVE WAS PULLING TWO SUPPLY CARS LOADED WITH CINDER BLOCKS AND WIRE MESH FROM ELLIS PORTALTO THE #1 SECTION. EMPLOYEE APPARENTLY LOST CONT ROL OF THE MOTOR WHILE ASCENDING A 0-4% GRADE (APPROX. 2,500' LONG) AND DESCENDED THE GRADE IN A RUNAWAY MODE. HE WAS FOUND AT THE BASE OF THE GRADE WITH THE WRECKAGE.

A 1989 AUTOCAR COAL HAULAGE TRUCK RAN OFF AND OVERTURNED ON THE LEFT SIDE OF THE ELEVATED ROADWAY RESULTING IN FATAL INJURIES TO THE DRIVER, A CONTRACTOR EMPLOYEE.

VICTIM IS BELIEVED TO HAVE CLIMBED CONVEYOR BELT SUPPORT STRUCTURE. IN AN UNKNOWN FASHION, WHILE ON BELT STRUCTURE, VICTIM CAME IN CONTACT WITH THE RETURN BELT IDLER AND SUFFERED FATAL INJURI ES.

EMPL DRIVING TRACTOR/TRAILER INTO MINE SITE-WENT DOWN STEEP GRADE AND WENT OFF ROADWAY AND OVER AN EMBANKMENT.
