Employee was checking offside of CM to check bottom, rock fell from between bolts striking CM and then landing on employee. The rock struck employee on shoulder and leg area. Employee was taken outside and transported to Greenbrier Med. Center and later transferred to CAMC. Treated for contusions and released next morning

As Employee was carrying a sand jack he tripped and fell. The sand jack fell on Employee and struck him in his lower back. Employee was treated at Raleigh General Hospital.

A non injury roof fall occurred in the No.7 entry, this is a continuation of a non injury roof fall reported for 5/20/2011. MSHA roof control specialist investigated the non injury roof fall.

EE was riding mantrip with the crew to #2 section. He was riding with the operator instead of the passenger compartment. The mantrip was not over crowded. EE said he was just riding there. He reached for his tobacco and his head hit the mine roof causing neck pain. He was released from CAMC hospital and told to remain off work a couple of days to rest his neck.

EE was holding fork by the end to lock bottom into position on the forklift. Fork slipped off and fell onto his foot causing a fracture to the big toe.

An unplanned roof fall occurred along the #3 belt at break #8

AN UNINTENTIONAL ROOF FALL OCCURRED AT THE FIRST RIGHT OFF OLD #8 BOLT SPAD NO. 3144. THE APPROXIMATE AREA OF THE SAID FALL WAS A LENGTH OF 50 FEET, 18 TO 20 FEET WIDE, AND 6.5 TO 8.5 FEET THICK. THIS FALL DID NOT EFFECT THE OPERATION OF THE MINE. THIS AREA HAS BEEN CRIBBED AND DANGERED OFF PREVIOUSLY BEFORE THIS FALL. THE DISTANCE OF THE FALL TO THE FACE WAS 3,440 FEET.

Employee was on his knees while bolting the #2 entry. He was positioned at his previous drilling location. As the bolter trammed forward, the side of the bolter hit his rescuer that was located on his left hip causing his left knee to twist into the mine floor.

EE was holding a wash hose washing off a scoop. The pressure pump kicked on and the nozzle was pulled from his hand causing a laceration.

While bolting top the roof bolt operator turned to make a bolt up and a piece of roof fell striking the employee on the right ear causing a laceration, Employee received first aid at scene and was transported to HRMC at Prestonsburg Ky,where he received stitches to his right ear.

An unplanned roof f all has occurred at brk 33 of 4 belt in the #7 entry. The fall approx. 70' L. X 17' H. X 19' W. There were no injuries involved.

While mine fire boss examiner was traveling the weekly intake travelway with MSHA inspector a roof fall was discovered in #6 entry at 70 breakacross 5 north belt. Fall measured approximately 4.5'high, 20'wide and 40'long. Area is under 103 K order and was dangered off. Ventilation and travel is not impeded.

Injured member was operating the roof bolter when he placed his left hand on the canopy and smashed his middle finger between the canopy and roof resulting in a laceration requiring 5 sutures and a distal fracture of the left middle finger. He was released to return to work.

While mine fire boss examiner was traveling intake with MSHA inspector for the weekly exam a continuation of an existing roof fall was discovered. The fall fell thru two thirds of adjacent intersection. Ventilation and travel was not impeded. Area was dangered off. Spad #19284.

While mine fireboss examiner was traveling the weekly intake travel way with MSHA inspector a roof fall was discovered in #1 enty at 66 break. Fall measured approx. 4' high, 20' wide and 35' long. Area is under 103 K order and was dangered off. Ventilation and travel is not impeded.

Roof fall occurred in the #3 enrty at spad #1576. Fall occurred 5600 feet from the active face on the 001-0 MMU. This area was developed March 2008. Area was originally supported with 5 torque tension bolts. Roof fall did affect ventilation and passage of persons. Fall will be rehabilitated. Fall 20w X 20l X 8H. Federal and state agency were notified immediately.

Employee stepped on a miner bit which caused him to twist his left knee, went to MedExpress at Lewisburg and was diagnosed with an Internal Derangement of Knee, will be sent to a specialist and have an MRI.

Employee was bolting in 5 right crosscut. As he was putting his steel in the tray the rib popped and a piece of it went into his right eye. He was wearing safety glasses. he went to the doctor and had it checked. He was released to come back to work with no restrictions.

Roof fall occurred in the number 4 entry, while pillaring the left block on the 002-0 MMU. The fall was approx 22' high at its highest point & 60' long & 20' wide. Two mobile roof supports were caught in the fall area. A large slip was laying at a slight angle along the #4 entry when the continuous miner was pillaring the left block the entry begin to give indications of failure.

Employee was opening a bucket of plaster and some of the plaster splashed into his left eye. Employee says he was wearing safety goggles, management does not agree. Employee was transported to Summersville Regional Hospital and given time off from work.

On May 24, 2011 EE was pulling on a chain hoist to lift a crusher roll out of the crusher. He injured his shoulder when doing so and took himself to Summersville Memorial ER where he says they diagnosed him with an infection. On May 30, 2011 he went to his family doctor who referred him to a specialist.

Roof fall was found during a pre-shift exam on MMU. 003-0 section. Roof fall is located in the #3 entry at crosscut #10 at survey station 6979 of 1-east submains. Roof fall is approx 18' wide x 7-10' thick and 60' in length. Roof fall is approx 30' feet inby and 30'feet outby crosscut #10. No injuries associated with this roof fall. The roof fall is located on a in-active section.

An unplanned roof fall has occurred at brk. 39 on 1st N.W. Panel. The nearest spad to this location is #1322. Fall was approx. 17' high x 18' wide x 50' long, there was no injuries involved with the fall.

Employee was in process of removing a damaged roller on the beltline when a section of the rib rolled out striking him on his left side knocking him to the ground.

Member was pulling 5'roof bolts out of a bundle to load onto his tray and his back started hurting .He was seen by a specialist and told to remain away from work because of a sprain or a herniated disc in lower back.

Employee was hanging line curtain and as he turned around to leave the area, he tripped over a rock and fell landing on his knee.

Employee was running the scoop when his canopy caught the bolter cable. He used his hand to push the cable clear of the canopy when the scoop rolled, pinching his finger between the canopy and the mine roof. He drove to Appalachian Regional Hospital and received ten stitches. He returned to work on his next scheduled shift.

Employee was walking in walkway along the track when he tripped and fell landing on his knee against track rail. The result was a fracture to his knee cap.

The brake system on the hoist car at the Roadfork #51 mine malfunctioned for a period of time that exceeded 30 minutes. When examined it was found that the tips "burned up" on a BC contact. The contact was replaced which restored the proper function of the braking system. No injuries were reported.

A non injury Roof fall occurred in the #7 heading, The roof fall was reported to MSHA by Supt. of White cabin #9, EE was assigned by MSHA to investigate the roof fall. The roof fall measured 18'x18'x7'

Employee was taking down MRS cables. The cables had been hung with nylon hangers with two hooks. He removed one hook. The hanger had been wrapped around the cable several times. The weight of the cable caused the hanger to spin free. The cable hook hit his nose. He had six stitches to the left nostril. He missed no work.

During a power move, Employee was cutting nylon hangers from highline. Using his left hand he cut hanger while his right hand was supporting the cable. The knife cut his right forearm. He drove to Raleigh General and received four stitches. He returned to work on his next shift.

An Unplanned roof fall has occurred in the #6 entry between crosscuts 13 and 14. (#5belt) 4' to 6' thick X 17' wide X 30' long. timbered and dangered off 2 days prior to the roof fall, no injuries involved with fall and the fall did not affect ventilation of travel ways.

Employee was loading an old drill pot onto flat car, by hand, when he felt a pain in his lower back and down his left leg. He started missing work on 5-31-11.

An unplanned roof fall has occurred in the #3 entry between crosscuts 16 and 17. (#5 belt). Fall was approx 10'thick x 15' wide x 55' long. Fell inby dumping point resulting in K-order and change of RCP. No injuries involoved and the fall did not affect ventilation or travel ways.

An unplanned roof fall has occurred at brk. 7 of 4A belt in the #3 entry. The nearest spad to this location is #1592 approx. 100' outby the fall. Fall was approx. 14' high x 18' wide x 25' long. There was no injuries involved with fall and it did affect ventilation or travelways.

An unplanned roof fall has occurred at brk. 3 of 4A belt in the #1 entry. The nearest spad to this location is #1829. Fall was approx. 16' high x 18' wide x 25' long. There was no injuries involved and the return was re-routed around the fall area.

Employee was taking fly boards from the mine floor and loading them onto the scoop. As he was pulling one of the boards from the mine floor, the board struck him in the mouth, chipping his front tooth.

An unplanned roof fall has occurred at brk. 16 of 1st S.W. Panel. The nearest spad is #1141. Fall was approx. 25' long x 18' wide x 12 to 15' high. There were no injuries involved and the fall did not affect ventilation or travelways.

Member was bolting in 2 left putting up the 2nd bolt he was putting glue and bolt into hole when a small rock hit the canopy and then causing a small cut to his face. He worked his next shift and then missed 2 days because he sprained his neck when he seen small rock falling

Employee was shoveling loose coal from mine floor onto 4-A belt conveyor line at number 6 break when a piece of coal rolled from the inby corner, on the walkway side of the belt. It struck him on the left leg just above the ankle.

Member was unloading 4 wide by 8 long pallets made out of 2 by 6s with 4 runners on the bottom off of a flat car into a scoop bucket. While doing so he experienced sharp pain in his lower back causing him to go to his knees with the pain. He took himself to Summersville Medical Center Emergency Room for treatment.

An unplanned roof fall has occurred at 34 brk. in the main return air course 40' outby the return overcast in the #2 entry at spad #1271. Air course was for both #1 and #2 sections the fall measured 17' high x 19' wide x 20' long there were no injuries involved.

An unplanned roof fall has occurred at brk. 18 of 1st S.W. Panel. The nearest spad is 50' inby fall spad #1158. Fall was approx. 25' long x 18' wide x 12 to 15' high. There were no injuries involved and the fall did not affect ventilation or travelways.

Injured member was lifting supplemental roof support timbers when he twisted his left knee. He did not cease work and continued to work his regularly scheduled shifts until May 25, 2011. Due to continued discomfort, injured was evaluated and excused from work by his doctor and awaiting an MRI on his knee.

As employee was moving the rub rail on a 14CM15 Continuous Miner he felt a pulling sensation in the lower left abdominal area. Employee was diagnosed at Med Express with a hernia on 5-17-11. Employee returned to work on restricted duty from 5-18 to 5-24. The hernia was repaired on 7-27-11 which the Safety Department was not notified of his surgery until 8-3-11.

Injured member was stepping off a haulage car when he slipped in the mud causing his legs to split and injuring his left hip. He was diagnosed with a muscle sprain/strain and excused from work.

The EE and 4 other EE's were positioning a piece of metal to be welded onto the conveyor belt tailpiece dump box. The metal slipped and caught the EE's finger between the metal and the tail piece frame. This resulted in a laceration which required sutures.

Employee was in the process of getting the bendictor cleaned at eh #9 head. He shut the #8 belt off by the switch. He the got into the rock box, at which time the belt started again with him on it. Employee grabbed a belt chain which threw him off the belt. He was transported by ambulance to Summersville Hospital, received stitches and treatment for fractured ribs.

EE was tramming feeder on belt move. Had to place crib blocks under it due to busted up bottom. Feeder slid off from crib blocks causing it to hit solid. EE's left ring finger was caught between lid and frame and pinched the end of his finger.
