Miner reached behind the seat and cut right hand on a piece of glass.
Employee was operating a gob truck, backed up to the berm and began to dump and felt the truck move backwards. EE attempted to pull up but felt the truck still sliding. EE exited the truck cab and jumped to the ground injuring ankle and both heels.
Operator was traveling down an incline that had been spot watered by another water truck and started sliding. When he corrected the water truck he traveled on dry ground and the truck turned over on its side.
Miner was trying to adjust the seat on Cat Scraper. When miner pushed his weight forward to move the seat the seat did not move. Miner felt pain in his right shoulder. Continued to work the rest of the shift thinking that it would get better. Miner returned to work the following morning and worked the entire shift but still felt pain in right shoulder.
Employee was operating end dump # 1158 and after 2 or 3 loads he said his right shoulder was hurting. Right shoulder strain.
Employee stated he was driving down ramp that had just been watered for dust control. when he got to the bottom when he got to bottom of ramp he need to steer quickly using his left arm. pulled muscle or strain muscle in left shoulder.
EMPLOYEE WAS COMING INTO MINE ON HAUL ROAD AND HIT A THE REAR END OF A SEMI TRUCK WAITING IN LINE ON THE HAUL ROAD TO BE LOADED. EMPLOYEE MAY HAVE FALLEN A SLEEP. THE EMPLOYEE HAD INTERNAL INJURIES. INJURIES TO THE LIVER AND INTERNAL BLEEDING.
Employee was operating unit number 1154 end dump and said his right shoulder popped. He could not use his right arm. went to IMED Medical center and doctor said he had a pulled a tendon out of place.
EE was in unit 1113 a Volvo gob truck hauling slurry from wash plant to slurry pit. While in transit employee started sliding on wet roadway over correcting and applying retarder the bed slid around the cab and he hit dry ground and turned over the bed, which laid the cab on its right side. Water truck had just watered road. Operator had seat belt on, ankle sore and back sore.
Employee was operating loader #1044 in coal yard pushing up coal on the coal piles. When he backed down off a pile a large piece of coal got under his right front tire and he stopped. The chunk of coal burst jarring the loader and injuring his back. This is an aggravation of a problem he has had before. We were not notified of accident until 6/27/2013.
He was running a tractor pulling a scraper at the Shamrock Mine. He was wearing a seatbelt and not driving at any excessive speed. He crested the ramp and hit a rut. The bounce that it cause made him sprain his back.
EE was walking on refuse walkway up to the refuse bin. He was pinging the rollers with a hammer when the hammer got caught in the belt and pulled his hand and arm between the belt and roller. Trauma/compression injury left arm.
Unloading ash at mine - truck and trailer not level
Employee was operating end dump #1192 and seat broke. He complained of back pain and was taken to hospital Emergency room. 190 ton truck.
Employee told his foreman at the end of his shift that he might not be in the next day because his back was hurting. Employee called the office the next day and said he had been to the doctor because he had hurt his back driving his end dump over rough roads.
EE was dumping a load of gob from a 35 ton end dump. The truck back to the berm and stopped, then the berm gave away and the truck went into the gob pit. EE had contusions and lacerations.
Employee was driving a 190 ton end dump. He was on the fill dumping his load. he said that the truck bed slamed back and jerking his neck. He was dizzy, vision was blured, and light headed.
It was dark (before daylight). Driver drove off edge of road and the truck laid over on it's side.
Employee was changing a hydro senser in the hydraulic system of a end dump. Employee had bed raised and secured. He had released the pressure from the tank. When he removed the sensor, hot oil sprayed on his hands resulting in a 2nd degree burn to his thumbs and 1st degree burns to his hands. Unit #1155.
EE was driving unit #1199, a 190 ton end dump, across the shovel bench to get a load of rock. He drove through a dip in the road. He hit his knee on the dash. He went to the hospital ER, contusion to knee.
A Mack dump truck driven by EE of Triple C Trucking was traveling north on CR 900 E when a 2000 Mercury passenger car was traveling west on CR 100 S failed to yield right of way to the dump truck. The dump truck impacted with the car in the area of the driver's door & pushed the car off the north west side of the road into a ditch. Driver of car pronounced dead at scene.
Employee was driving a water truck and going down into the pit. When he got to the bottom of the road there was a dip in the road. He hit his head on the air conditioner resulting in a cut to head and receiving 11 staples. He did not hav his seat belt on. (I did not receive this until 5/20/2008).
Employee said he was running unit #1176-R190. He was dumping mud. When the bed goes up some time the mud slides out of the bed all at once. Then bed goes up and comes down fast until the cylinders catch up.
ON 05/07/04, EE WAS OPERATING A CONTINUOUS MINER WHEN A PIECE OF ROCK FLEW OUT FROM THE TAIL CONVEYOR, STRIKING HIM ON THE BACK OF THE HEAD CAUSING A SMALL LACERATION REQUIRING FOUR SUTURES TO CLOSE. HE WAS TREATED AND RETURNED BACK TO WORK.
EE was operating an End dump, his back was hurting and he went home early. He said the seat was broke, but others said the seat was not broke.
EE was running a Euclid R-120 end dump. His right leg was hurting him, he could not continue working. He had trouble walking, could not remember injuring himself.
EMPLOYEE AND A CO-WORKER WERE USING A CLA FORKLIFT TO UNLOAD A HYDRAULIC CYLINDER FROM THE BACK OF A MAINTENANCE VEHICLE AND AS THE EMPLOYEE GUIDED THE CYLINDER AWAY FROM THE VEHICLE HIS RIGHT HAND WAS CAUGHT BETWEEN A SIDE BOARD ON THE CARGO AREA OF THE VEHICLE AND THE HYDRAULIC CYLINDER. THIS RESULTED IN A SIMPLE FRACTURE TO THE EMPLOYEE'S RIGHT RING FINGER. HE WAS TREATED & RELE
EE WAS OPERATING COMPANY PICKUP ON COUNTY ROAD GOING FROM MIDWAY PIT TO CANNELBURG WASH PLANT. HE CAME TO AN INTERSECTION WHEN ANOTHER VEHICLE RAN A STOP SIGN AND EE RAN INTO THE SIDE OF THEIR VEHICLE. EE WORKED SOME DAYS AFTER THE ACCIDENT. THEN WENT TO DOCTOR AND FOUND HE HAD A BROKEN RIB.
ON 4-8-03 EE WAS LOCATED BETWEEN A PERSONAL CARRIER AND BELT SUPPLY SKID AND HE WAS UNLOADING BELT SUPPLIES FROM THE PERSONAL CARRIER A BATTERY SCOOP TRACTOR THAT WAS TRAMMING BY THE CROSSCUT WHERE THE PERSONAL CARRIER WAS PARKED INADVERTENLY CAUGHT THE VEHICLE WITH THE SCOOP BUCKET. THEVEHICLE WAS PUSHED INTO THE SUPPLY SKID CAUSING THE EE LEGS TO BECOME CAUGHT BETWEEN EDGE OF SUP
EE WAS CHANGING A TIRE ON A BATTERY POWERED COAL HAULER AND AS HE WAS IN THE PROCESS OF BLOCKING-UP THE HAULER, THE HAULER MOVED APPROX. 6"KICKING OUT A CHOCK BLOCK, PINCHING HIS LT.RING FINGE R. HE SUSTAINED A LACERATION TO HIS FINGER REQUIRING 6 SUTURES TO CLOSE THE WOUND.
EE WAS OPERATING A CONT MINER, HE WAS STANDING BETWEEN A LINE CURTAIN AND THE RIGHT RIB. A COAL HAULER HAD JUST BEEN LOADED AND WAS TRAMMING AWAY FROM THE FACE AND AS THE COAL HAULER BEGAN TO MAKE A TURN THE VICTIM WAS STRUCK ON THE LEFTKNEE BY THE COAL HAULER TRAILER. THE VICTIM RECEIVED A CONTUSION TO HIS LEFT KNEE AND SPRAINED HIS RIGHT ANKLE.
ON 1-9-02, EE WAS OPERATING A LOADER, WHICH COLLIDED WITH A TRUCK, DUE TO MUDDY CONDITIONS. AT THE TIME OF THE INCIDENT, EE DID NOT SEEK MEDICAL CAR, BUT ON 1-28-02, SAW DR. WHO DIAGNOSED A ME DIAL MENISCUS TEAR TOE THE RIGHT KNEE. HE WAS T AKEN OFF WORK ON 2-19-02 AND WILL HAVE SURGERY.
EE WAS DRIVING WATER TRUCK DOWN THE MIDDLE RAMP. HE RAN OVER A BUMP IN THE RAMP CAUSING HIM TO HIT HIS HEAD ON THE TOP OF THE CAB. HE HIT THE AIR CONDITIONER THERE IS A CUT ON HIS HEAD. IT TOO K 8 STITCHES TO CLOSE.
WHILE RUNNING 988 LOADER EE PINCHED A NERVE IN HIS LEFT SHOULDER.
EMPLOYEE WAS DISMOUNTING A COAL HAULER WITH THE MACHINE ENERGIZED. AS HE WAS DISMOUNTING HE WAS USING BOTH HIS HANDS TO SUPPORT HIMSELF ON THE EDGE OF THE MACHINES CANOPY. WHEN HE WAS PARTIALL Y OUT OF THE OPERATORS COMPARTMENT HE HIT THE STEERING LEVER WITH HIS FOOT, CAUGHT HIS FINGER BETWEEN CANOPY AND RIB. HE RECEIVED A LACERATIONTO HIS RIGHT INDEX FINGER 2 SUTURES REQUIRED.
EE WAS TRAMMING A BATTERY COAL HAULER INTO THE MINE. HE WAS TRAVELING DOWN THE 8 DEGREE MAN & MATERIAL SLOPE & WHEN HE WAS APPROX 100 FT FROM REACHING THE BOTTOM OF THE SLOPE, HE REPORTED LSOI NG POWER ON THE MACHINE, CAUSING HIM TO LOSE BRAKE & STEERING FUNCTION.AFTER LOSING POWER ON THEMACHINE,, & WITH NO STEERING FUNCTIONS HE LOST CONTROL & STRUCK THE ARCH SUPPORTS NEAR THE SLOP
EE WAS RUNNING UNIT #1186 END DUMP, HE WAS LEAVING THE DUMP AREA. THERE WERE RUTS THERE, THE TRUCK LEANED ONE WAY AND EE THE OTHER, PULLED A MUSCLE IN LOWER BACK.
