Employee was operating a gob truck, backed up to the berm and began to dump and felt the truck move backwards. EE attempted to pull up but felt the truck still sliding. EE exited the truck cab and jumped to the ground injuring ankle and both heels.
Employee was exiting the water truck when hand got stuck in the door and ripped off finger nail.
Truck 376 had a front bumper marker light pole that was broken and had a sharp edge on it. Driver got out of truck and came into contact with the broken sharp edge and cut EE's right arm.
Employee had stopped to take a break and when EE exited EE's truck EE tripped over a frozen dirt clod twisting EE's lower back.
Miner was getting off of dozer when EE notices the step was full of dirt. EE stepped off the push arm (approximately 3 feet) to the ground and landed wrong on EE's foot and twisted EE's ankle.
Employee was re-filling the grease truck and slipped while getting off of the truck. The employees foot missed the step.
Miner stated that EE was returning to EE's equipment when EE stepped in a rut and twisted EE's knee. The walking surface was dry.
Employee was installing a bearing on the left front strut on end-dump. Employee was hammering a bearing in place with a 10 pound hammer while holding it with his left hand. Employee's hammer become entangled with some hoses and missed the bearing and struck his hand with the hammer.
Miner was performing maintenance when stepped on pipes and foot slipped. EE attempted to catch himself with hand. EE then felt pain in right wrist.
Laborer was removing the wheel chalks from fuel truck when he felt a sharp pain in his back at the end of day-shift. He returned to work his next scheduled shift and had stiffness in his back. Employee returned home.

Miner was hooking a hose to a pump and using his hand to hit the dog ears on the connector. The dog ear had a sharp piece on it causing a puncture wound.
Employee had just pulled his powder truck under the powder bin to reload. The powder truck was parked in a rut requiring him to step a little higher to the first step. The employee then felt pain in his groin area. The employee continued to work the rest of the shift and returned the next day but the pain increased so he went to get it checked.
Miner was walking in the parking to his dozer when he slipped and tried to reach for the track to prevent himself from falling. He felt a strain in his shoulder.
The miner was helping another miner tighten a bolt on a Dryer Feed Tube with an impact wrench. The impact socket had an adapter connected to it by a 1/4 inch bolt. The miner was holding the socket on the bolt head when his hand came into contact with the spinning socket. The miner received a cut between his two middle fingers requiring 4 sutures
Employee was operating end dump # 1158 and after 2 or 3 loads he said his right shoulder was hurting. Right shoulder strain.
Employee was working on clean coal dryer. he grabbed the vibrator belts to position himself. the belt rolled and his pinky finger was caught between belt and pulley. fractured right pinky finger
Employee stated he was driving down ramp that had just been watered for dust control. when he got to the bottom when he got to bottom of ramp he need to steer quickly using his left arm. pulled muscle or strain muscle in left shoulder.
Employee was climbing out of (it14) loader unit # 1095. He stepped on a frozen clod of mud twisting his left ankle and foot.
Employee was working on feeder belt under plant hopper. They were replacing belt and pulling it through. He hurt his right shoulder. Pulled muscle or tear.
Employee was taking drain plug out of unit 1115 diff. Volvo 40F articulating truck. Hammer glanced off of breaker bar- pinching right hand index finger between hammer handle and breaker bar. Five stitches in index finger.
EMPLOYEE WAS COMING INTO MINE ON HAUL ROAD AND HIT A THE REAR END OF A SEMI TRUCK WAITING IN LINE ON THE HAUL ROAD TO BE LOADED. EMPLOYEE MAY HAVE FALLEN A SLEEP. THE EMPLOYEE HAD INTERNAL INJURIES. INJURIES TO THE LIVER AND INTERNAL BLEEDING.
Employee was opening a drain valve on a water pump and it was hot. When he opened the valve hot steam hit his right hand causing second degree burn on hand.
Employee was refilling his grease truck and when he slid the side door shut it caught his finger between the door and the door jam. (left hand- middle finger)
Employee was using a grinder with a cut of wheel to cut the ends off of a cable that was looped and woven back together. Employee said that he didn't know what happened but he cut his left forearm with the grinder requiring 9 stitches.

Employee was taking bearing races off a pump shaft when it he hit the race with a hammer the shaft slipped and fell and caught his little finger on his left hand between the end of the shaft and the edge of the bench he was working on. Cutting his little finger and requiring 11 stitches.
Employee was going to operate an end dump unit #1194 he had just started to climb the ladder when his left shoulder popped and he was in a lot of pain, he fell to the ground but said his pain was all in his shoulder. left shoulder sprain with possible derangement.
Employee was operating unit number 1154 end dump and said his right shoulder popped. He could not use his right arm. went to IMED Medical center and doctor said he had a pulled a tendon out of place.
Employee was working on a D10 Dozer he had a 6 foot ladder leaned against the push arm. He started to climb the ladder and was about five feet off the ground he said the ladder tipped over with him. He fell hitting the steps and the push arm and falling to the ground. He said he was all right. later it was found that he had broken ribs punctured lung and punctured spleen.
three employees were moving a 4" hose out of the way of equipment. employee complained of back pain afterward. went to IMED medical lower back strain.
Employee was getting off of Coal stockpile truck unit number 1146 (HD 325 Komatsu) when he slipped on rough ground. He said it felt like something pulled in his right hip. He went to doctor on 04/09/2014. He worked on 04/10/2014. Went back to the doctor on 04/11/2014 and was off till 04/16/2014. Lumbar strain.
Two employees were trying to get to a sheet of metal in a metal rack, when two pieces of metal shifted in the rack and caught one employee hand in between the sheets of metal pinching his left hand also caught his right foot. Contusion and abrasions to left arm.
Employee was cleaning out the batter box on scraper. the wind was blowing very hard that day and dust blew dirt up under his safety glasses. Employee went to Imed 2 days later and had his eye washed out and eye drops.
employee was loading the weed burner in the back of unit 2303 pickup truck. He slipped on the thawing ground and fell on his back and said he must of fell on a rock or piece of coal hurting his middle back. worked until 01/14/2014 then went on light duty 01/15/2014.
