THE EMPLOYEE'S LEFT HAND WAS ON THE PIN WHILE LOADER MAN BUMPED THE CAR CAUSING HIS RIGHT THUMB TO BE MASHED BETWEEN PIN HANDLE AND THE CAR FRAME.

WHILE ATTEMPTING TO REVERSE THE DIRECTION OF A CONVEYOR FEED CHAIN BY USE OF THE MANUAL LEVERS THE PALM OF THE OPERATOR'S LEFT HAND WAS PINCHED BETWEEN THE BASE OF THE LEVER & A HYDRAULIC HOSE LEAVING A LACERATION THAT REQUIRED SUTURES TO CLOSE. EE WAS NOT WEARING GLOVES AT THE TIME & ONE HANDLE ON THE FEEDER WAS MISSING. RELEASED TO FULL DUTY.
EMPLOYEE WAS OPERATING AN OPEN DIESEL PERSONNEL CARRIER. HE WAS TRAVELING ALONG THE UNIT 1 TRAVEL WAY. THE BELT CREW WAS PULLING THE UNIT 1 BELT TOGETHER. THE BELT WAS BEING PULLED W/A DIESEL SCOOP ATTACHED BY A WIRE ROPE. THE DIESEL SCOOP PULLED ACROSS THE TRAVEL WAY AND EMPLOYEE RAN INTO THE CABLE. THE INCIDENT CAUSED A LACERATION TO HIS FOREHEAD REQUIRING STITCHES.
COMING OFF OF A HILL AT QUITTING TIME, TURNED TRUCK BED OVER ON TRUCK.
WHILE OPERATING AN OPEN TOPPED PERSONNEL CARRIER THE OPERATOR LOOKED LEFT AND STRUCK HIS HAT ON A LOW HANGING OBJECT KNOCKING OFF HIS HAT AND CAUSING PAIN TO HIS NECK. DIAGNOSED WITH A SPOT OF BLEEDING ON THE BRAIN FROM IMPACT. NO DAMAGE TO NECK OR BACK. LOST TIME ACCIDENT.
EMPLOYEE STATED THAT HE WAS OPERATING A COAL HAULER TRUCK WHEN HE HIT A HOLE IN THE ROAD CAUSING HIM TO JAR HIS BACK. EE WORKED UNTIL 5/4/04, WHEN HE REPORTED THAT HIS BACK WAS SORE. HE WENT TO HIS DR. AT THAT TIME & REPORTED OFF WORK.
EMPLOYEE WAS USING A WALK BEHIND ELECTRIC FOKLIFT TO MOVE AN ELECTRIC MOTOR FROM THE ELECTRICAL PART STORAGE BUILDING. AS HE WAS BACKING UP THE FORKLIFT, HE WAS WATCHING A COMPRESSOR ON THE FL OOR SO THAT HE WOULDN'T HIT IT. WITH HIS ATTENTION ON THE COMPRESSOR, EMPLOYEE PROCEEDED TO SLOWLY BACK UP THE FORKLIFT AND CAUGHT HIS FOOT BETWEEN THE FORKLIFT AND A PALLET.
BED DROPPED ON 789 CAT TRUCK, KNOCKING EE TO THE FLOOR OF CAB. EE HIT KNEES ON DOOR EDGE IN FLOOR.
WHILE RIDING AS A PASSENGER IN A ROCK TRUCK EE CLAIMED HE WAS HURT WHEN TRUCK OVERTURNED. HE REFUSED ANY MEDICAL ATTENTION AT THE TIME. WENT TO DR. ON 1-23-04 ON HIS OWN. HE WAS PLACED ON LIGH T DUTY WORK. THERE WAS NO DAMAGE TO THE ROCK TRUCK.
OPERATOR OF ROCK TRUCK WAS BACKING UP TO DUMP LOAD AND OVERTURNED. OPERATOR RECEIVED A HEAD WOUND REQUIRING 10 STITCHES. THE TRUCK HAD DAMAGE TO HEAD BOARD.
EMPLOYEE WAS LOADING SHOT ROCK WITH A LOADER INTO A ROCK TRUCK. THE LOADER SLID OFF A ROCK, JARRED HIS BACK.
TRESPASSER WAS RIDING A DIRT BIKE ON A COUNTY ROAD UNDER CONSTRUCTION IN A RECLAIMATION AREA. HE HIT HOLE AND LOST CONTROL OF HIS BIKE. HE FLAGGED EMPLOYEES DRIVING BY WHO CALLED FOR AN AMBULA NCE WHO CALLED FOR LIFEFLIGHT BECAUSE OF A POSSIBLE HEAD INJURY. HE HAD BEEN PREVIOUSLY WARNED NOT TO RIDE ACROSS MINE PROPERTY. HE WOULD RIDE AROUND GATES AND OVER BERMS ON CLOSED ROADS.
EE PARKED A FORD F250 PICKUP IN ORDER TO GET TOOLS OUT OF ANOTHER VEHICLE. HE LEFT THE TRUCK RUNNING. A SHORT TIME LATER HE NOTICED THE TRUCK ROLLING BACKWARDS & RAN AFTER IT TO STOP IT. WHEN HE JUMPED IN, HE HIT HIS BACK ON THE BACK OF CAB AT THE DOOR OPENING. HE DROVE THE TRUCK TO THE SHOP & WENT TO HIS FAMILY DOCTOR AFTER THE SHIFT. MAINTENANCE FOUND A BROKEN CLIP THAT HOLDS THE
EE STATED THAT AS HE WAS LOADING MATERIAL ONTO A SKID STEER LOADER A DIESEL SCOOP ATTEMPTED TO PASS AND STRUCK HIM WITH THE BUCKET.
WHILE HANDLING TORCH HOSES A LOOSE SECTION OF HOSE WAS PULLED INTO A RUNNING FEED BELT ROLLER, PULLING THE EE'S HAND INTO THE BELT BEFORE HE WAS ABLE TO RELEASE HIS GRIP ON THE HOSES. EE S HAN D WAS PULLED THRU A NARROW OPENING 1/2 INCH AND THROWN CLEAR. TREATMENT INCLUDED CLEANING AND SUTURING.
RIDING IN HELPERS COMPARTMENT OF 14-TON TRACK MOTOR - HAD BOTH FEET RESTING ON RAISED LIP OF COMPARTMENT. MOTOR JUMPED TRACK - JERKING AND WHIPPING EE. THIS ACTION CAUSED BOTH FEET TO SLIP OUT OF DECK OF COMPARTMENT AND BE ROLLED BETWEEN MOTOR AND RIB. BROKE 5 BONES IN TOP OF FOOT.
AT THE TIME OF THE INJURY, THE INJURED EE WAS OPERATING A CAT 793 ROCK TRUCK. HE WAS IN THE PROCESS OF BEING LOADED BY THE ELECTRIC SHOVEL. DURING ONE OF THE PASSES (SWINGS) THE SHOVEL STRUCK THE SIDE OF THE TRUCK BED. THE SUDDEN IMPACT CAUSED THE INJURED TO BE THROWN, RESULTING IN NECK & ARM PAIN.
EE WAS USING A SKID MOUNTED PORTAJOHN LOCATED ONTHE DRILL BENCH. ANOTHER EE UNAWARE THAT SOMEONEWAS USING THE PORTAJOHN HOOKED A SMALL TRACTOR TO THE PULL CABLE TO MOVE IT OUT OF THE WAY OF A SHOT. WHEN THE PORTAJOHN WAS MOVED, IT CAUSED THE EE TO BE THROWN AGAINST THE SIDE OF THE PORTAJOHN & SHE LANDED ON THE FLOOR. EE DID NOT REPORT
EE WAS OPERATING THE DIESEL MATERIAL SCOOP. AS HE TRAVELED ALONG THE UNIT 2 TRAVELWAY THE SCOOP STRUCK A BUMP CAUSING THE SCOOP TO BOUNCE AND EE STRUCK HIS HEAD AGAINST THE SCOOP CANOPY. THE INCIDENT HAS BEEN UNDER INVESTIGATION WAITING FOR MEDICAL REPORTS FROM THE DR.
EE STATED THAT WHILE TRAMMING THE SCOOP UP THE ENTRY, HE RAN OVER AN UNSEEN PIECE OF BELT STRUCTURE IN THE MUD. THE STRUCTURE FLIPPED UP AND HIT HIM ON THE RIGHT CHEEK.
EMPLOYEE WAS HOLDING SHUTTLE CAR CABLE UP, TO LET SCOOP CLEAN FEEDER. SHUTTLE CAR OPERATOR STARTED SHUTTLE CAR PULLING CABLE DOWN INTO EMPLOYEE HURTING BACK AND RT SHOULDER. EMPLOYEE WORKED 2- 05, 2-06, 2-07 WENT TO DR BECAME LTA STARTING 2-10.
WHILE HAULING ROCK FROM THE PIT TO THE FILL AREA, EE HIT AN UNEVEN SPOT IN THE ROAD, CAUSING THE SEAT OF THE TRUCK TO BOTTOM OUT. EE FELT PAIN IN LOWER BACK INSTANTLY.
EE WAS OPERATING END LOADER, AND WHILE BACKING UP HE RAN UPON A ROCK. WHEN LOADER CAME OFF, IT BOUNCED HIM AROUND IN CAB, CAUSING HIM PAIN IN NECK & BACK. EE STARTED MISSING 2-5-03.
EE REPROTED THAT WHILE BACKING DOWN INTO A PIT, HE RAN INTO A HOLE IN THE ROADWAY AND FELT A PAIN IN HIS LOWER BACK.
THE EE WAS DIRECTING A SCOOP OPERATOR TO BACK UPTHE SCOOP TIRE DROPPED INTO A HOLE. THE SCOOP BUCKET DROPPED ONTO THE EE FOOT. `
EE WAS OPERATING A 988B CAT LOADER IN THE PROCESS OF BUSTING COAL. HE STATED HE RECEIVED A SUDDEN JAR WHICH RESULTED IN NECK PAIN. HE CONTINUED TO WORK AND DID NOT TAKE OFF WORK UNTIL 12-2-02.
WHILE OPERATING THE FRONT ENDLOADER A ROCK ROLLED AND STRUCK THE ENDLOADER BUCKET.
EE WAS OPERATING A SHUTTLE CAR WHEN HE RAN OVER A BUMP IN THE ROAD THAT CAUSED HIM TO HIT HIS HEAD AGAINST THE MINE ROOF. AFTERWARDS HE SAID HIS NECK GOT STIFF AND COULD NOT TURN HIS HEAD.
EE WAS OPERATING A CAT ROCK TRUCK WHEN HE RECEIVED A SUDDEN JAR WHILE GETTING LOADED COMPALINED OF PAIN IN LEFT SHOULDER AREA. EE FAILED TO REPORT ACCIDENT UNTIL NEXT SHIFT 8-8-02 AT WHICH TIM E FOREMAN SENT HIM TO HOSPITAL FOR X RAYS FIRST DAY OFF WORK WAS 8-12-02.
RIDING RAILRIDE OUT OF MINES AND WAS STRUCK BY TROLLEY POLE.
EE WAS OPERATING ENDLOADER, AND WHILE TRAMMING IT TO PIT, HE RAN OVER A ROCK, CAUSING HIM TO BE JERKED AROUND, AND HURT HIS RIGHT SHOULDER WHEN HE BRACED IT AGAINST STEERING WHEEL.
EE WAS DRIVING AN EMPTY TANDEM COAL TRUCK ALONG THE KNIFHT TRK HAULROAD AND DROVE THE TRUCK THROUGH A BERM (VEY HIGH) AND OVER A 200' EMBANKMENT. HE SUSTAINED A BROKEN GUMLINE BONE AND LACERAT IONS TO HIS CHIN, LIP AND HAND. EVIDENCE INDICATES AND INFORMATION GIVEN TO US AT THIS TIME IS THAT HE FELL ASLEEP AT THE WHEEL.
THE SECURITY GUARD WAS RELIEVED FROM HIS POST AT 6 AM. HE WAS LEAVING THE PROPERTY AT 6:15 AM WHEN HE COLLIDED WITH AN EMPTY COAL TRUCK.
AS SHUTTLE CAR WAS TURNING INTO #6 ENTRY IT HIT A ROCK CAUSING HIM TO VEER INTO CURTAIN, HITTING EMPLOYEE CAUSING BRUISES AND CUTS TO BOTH LEGS.
EE LOST BALANCE WHILE RIDING MANLIFT AT PLANT & FELL FIVE FEET. THIS RESULTED IN A FRACTURE TO LEFT HEEL.
EE WAS OPERATING A 992G CAT LOADER WHEN FRONT TIRE DROPPED IN HOLE CAUSING MACHINE TO SHIFT. EE COMPLAINED OF NECK PAIN TOOK HIM TO HOSPITAL. X-RAYS NEGATIVE. EE WENT ON VACATION AND R.T.W. HIS NEXT SCHEDULED SHIFT. HE CONTINUED TO WORK UNTIL 6-28-02, WHEN HE INFORMED HIS SUPERVISOR HIS DR. HAD TAKEN HIM OFF WORK.
EE WAS DRIVING A 785 ROCK TRUCK, HAULING OVERBURDEN FROM A 5230 CAT SHOVEL. WHILE BEING LOADED BY THE SHOVEL A ROCK FELL OFF THE RIGHT SIDE OF THE BUCKET & LANDED ON THE TAIL BOARD OF THE ROCK TRUCK CAUSING THE OPERATOR TO GET "BOUNCED" IN HIS SEAT.
EE WAS TAKING A LOAD OF COAL TO KCT KY COALTERMINAL. WAS DESENDING THE HAUL ROAD. HIS TRANSMISSION CAME OUT OF GEAR. HE STAYED WITH THE TRUCK ABOUT 75' TRYING TO STOP THE TRUCK. HE JUMPED OUT AS THE TRUCK WENT OVER THE HILL. THIS WAS NOT A SERIOUS INJURY.
EMPLOYEE STATED THAT WHILE BEING LOADED THE TRUCK HE WAS DRIVING WAS SHAKEN, HE FELT PAIN IN HIS RIBS.
EE WAS OPERATING A ROCK TRUCK. HE LOST CONTROL OF THE TRUCK, STRUCK A BERM AND SPOIL PILE COMING TO A STOP. HE SUFFERED INJURY TO LOWER BACK AND POSSIBLE HEAD INJURY.
THE EMPLOYEE WAS DIRECTING THE MOVEMENT OF A SKID STEER LOADER WHEN THE FORKS OF THE LOADER SWUNG INTO HIS LEGS.
EE & CO-WORKERS WERE REMOVING A BENT DRILL STEEL ON THE #4292 DRILL. AFTER PLACING DRILL MAST IN DOWN POSITION THE BOOM TRUCK WAS BEING USED TO REMOVE BENT SECTION. WHILE THE STEEL WAS BEING R EMOVED, IT GOT FOULED MOMENTARILY & WHEN IT SPRANG LOOSE IT SUDDENLY CAME IN HIS DIRECTION. WHEN MOVING TO GET OUT OF WAY HE WALKED INTO ANOTHER PIECE OF DRILL STEEL (EYE LEVEL) ON THE RACK.
EE WAS FOLLOWING A COAL TRUCK IN ON THE HAUL ROAD & ANOTHER WORKER WAS COMING OUT (FROM JOB). EE SAID HE HEARD NOTHING ON THE CB SO HE THOUGHT NO ONE WAS COMING. THE 2 PERSONAL TRUCKS HIT HEAD -ON, DRIVER'S SIDE.
OPERATOR WAS OPERATING A 980C CAT LOADER CLEANING UP DUMP PAD AND BACK INTO COAL TRUCK. LOADER OPERATOR WENT TO HOSPITAL WITH COMPLAINTS OF BACK PAIN. XRAYS WERE NEGATIVE. HE RETURNED TO WORK AND CONTINUED TO WORK UNTIL 3-25-02. HE CALLED AND SAID DR. PUT HIM OFF FROM WORK.
THUMB GOT CAUGHT BETWEEN LINKS OF CHAIN A HE WAS GUIDING ROLL OF CONVEYER BELT. LACERATION REQUIRED THREE SUTURES. HE WAS RELEASED TO RETURN TO WORK.
EE WAS TRAMMING THE END LOADER INTO THE SHOT MATERIAL. HE HIT HARD MATERIAL IN THE BOTTOM. THE LOADER CAME TO A SUDDEN STOP, JARRING HIS BACK.
OPERATING LOADER AND BUCKET ON LOADER CAME DOWN FAST AND WHEN HE PUT BUCKET LEVEL CONTROL IN LOCKED POSITION, BUCKET STOPPED SUDDNELY, CAUSING JARRING EFFECT TO LOADER, INJURING HIS BACK.
EE HAD CAME OUT OF THE MINE AND WAS WALKING ACROSS THE MINE YARD. EE HAD STARTED TO MOVE A PERSONELL CARRIER WHEN HE RAN IN TO EE HITTING HIS LEG WITH THE CARRIER EE WAS KNOCKED TO THE GROUND INJURING HIS LEFT KNEE.
OPERATOR WAS LOADING ROCK TRUCKS WITH A 992G CATERPILLAR LOADER WHEN HE RECEIVED A JAR CAUSING HIM BACK PAIN.
