Birch River Mine Coal

Employee was bent over pre heating steel and when he went to standup he felt pain in his lower back (right side). He was diagnosed with strain/inflammation/prescription meds.

Employee states that as he was attempting to dump a load of coal from trailer, the coal stuck in trailer causing the front wheels of tractor to raise off the ground. When the coal dislodged, the trailer came down suddenly, jarring employee.

A contract employee, and a co-worker were in process of splicing a 6-inch water line with a metal band and banding tool. During the process, EE was pulling one end of the stuck band out of the banding tool with a pair of pliers. When the band sprang out, the sharp edge struck the side of his left hand, resulting in a laceration.

Injured stated he was standing outside the shop area talking to employees when he noticed something felt like it went in his eye. He immediately washed out his eye. However, two (2) days after the event, he sought medical attention where an abrasion was observed and was given a prescription for eye ointment. Note: This incident was not reported to managment until 11/5/2010.

Employee states he became nauseous from fumes upon inspection of battery box during the truck's daily service in shop. "REPORT BEING FILED UNDER PROTEST"

Employee stated while working in the shop he sustained 'welding flash' to both eyes due to a nearby co-worker's welding activities.

The injured, a contract employee, was in process of servicing a Caterpillar de-watering pump generator. While performing the task, his foot slipped on the trailer step causing his foot to slide in the space between the trailer and the house of the generator. As a result, he twisted and bumped his knee.

Employee stated that as he was checking tires, he tripped and fell. As he fell, he grabbed tires and twisted lower back.

Employee states he was hitting a bolt with a sledge hammer when a piece of metal him him in the eye.

Employee states that while attempting to tighten a wheel lug nut on a truck, the wrench slipped and he fell from ground level to ground level, resulting in a contusion to his finger and a strain to his back."REPORT FILED UNDER PROTEST"

Employee stated while working from a 2-foot step ladder and placing boxes of filters in the appropriate bins, she lost her balance and fell to the floor. In doing so, she twisted her left knee resulting in a sprain. Note: Employee started missing work due to injury on May 31, 2009.

The injured was helping install a seat in a bulldozer. During the process, while his hand was underneath, the handle that latches the seat was inadvertently bumped which caused his ring and middle finger to get caught between the seat and base. As a result, his middle finger sustained a fracture at the finger nail area.

After making electrical repairs at the dragline, the injured re-set the three (3) 480 volt breakers and while examining the circuit panel, one of the breakers suddenly 'blew,' causing the injured to sustain burns to his face and neck area. He was taken to the local hospital where he was treated and admitted over night for observation.

The injured employee stated he was in process of checking an air leak at the main water stand. While walking toward the air valve location, he stepped on a loose rock. When this happened, his foot rolled off the side causing him to twist his knee. The injured continued to work until his physician took him off work on 6/27/2008 due to the MRI showing a torn cartilage.

Employee was installing transmission pump on 785 Cat truck. While lifting pump, strain to upper back muscles.

EE was checking machine for proper working condition after returning to staging area to obtain pressure wand that was missing from truck. His foot became tangled in hose or hose reel section causing him to fall from back bumper/step of truck & accidently squeeze gun handle. He then said gun stuck in the open position. He let go of wand to catch himself. Water struck him in chest.

The EE was operating the dozer, dug a trench then drove into the trench. He was not wearing his seatbelt and struck his head on the dash board, He cut his head and had to have stiches. The EE was given three days without pay for falling to wear his seatbelt.

An employee of Gil's Contracting stated while using a razor knife (folding style) and cutting a section of rigid water line the knife slipped and struck his left hand, lacerating the top portion of his ring finger.

Employee alleges injury from latching truck tailgaate. Employee tested positive from subsequest drug test. Employment terminated."Report filed under Protest"

On July 25, 2007, the injured was walking on the crossover of a drill when his foot slipped, injuring his knee. As the months went by, his knee pain continued to get worse. Please note, on May 1, 2008, surgery was performed to repair a torn cartilage, thus making the injury a reportable event at that time.

EE was in process of removing the transmission from truck at the truck bay in the main shop. While removing bolts from the cross member it suddenly fell catching a portion of his left hand between the cross member and frame. As a result, he sustained a laceration on the palm of his hand, at the base of the little finger.

3/21/07, EE reported pain in left shoulder while tossing a truck headrest to coworker. EE was seen by a dr who ret'd EE to work on next shift. 5/21 EE off work due to pain. EE had rec'd cortis one treatment in Feb. EE has pre-exist condition & treated for pain prior to 5/21 report. Determined likely to accumlated trauma disorder.

The employee was operating a dozer. He was crossing a ditch and struck his head on the mirror of the dozer. He was not wearing his seatbelt. He cut his head which required stitches.

EE stated that while making an inspection for a suspected flat tire, he slipped on ice, falling and injuring his knee on a rock. Management questions this incident/injury.

EE reported on 2/9/07 that an alleged injury occurred on 2/8/07 while removing tire chain binders. He reported the alleged incident on 2/9/07 @ 6:10pm. He worked for 4 hours and sought medical attention @ 10:15pm. Managment questions this alleged incident/injury.

EE stated he was using a jack to push out a trasmission bolt, when it slipped and hit him in the mouth.

Employee states on evening of the injury, he lost a corner bit off his dulldozer. While he and a co-worker were scooting the corner bit back toward the bulldozer, he placed his fingers in the bolt hole resulting in his finger striking a burr resulting in a laceration. Note; management was not aware the ee went to a medical facility and received stitches until 3/13/07. see letter

EEa contract employee for JP Technical Services, was in process of changing an `idler? on the 345 backhoe when his little finger caught a jagged piece of metal (on the side of the idler) resulting in a laceration which required seven (7) stitches.

Employee stated that while stepping from truck he turned his foot and injured his left ankle.

Employee received a Workers' Compensation award for occupational pneumoconiosis.

Employee stated that he was climbing into his truck. He slipped and hit his knee on fuel tank step.

Employee stated that he was stung by a wasp.

As truck was staged in front of bin waiting to dump, the loader attempting to clear off bin pulled in front of truck to clean bin and backed into the waiting truck. No injury reported - Employee refused medical treatment.

Employee was walking carrying his chainsaw & fuel. He tripped and fell on a stump.

Employee stated he was tightening wheel nut with a lug wrench and pulled muscle in back.

Employee was in process of steaming his steam truck, when he rose up he suddenly felt dizzy & collapsed backwards, striking his head on the ground. This caused a puncture wound to the back portion of his head. He was taken to Charleston area medical ctr where he was kept over night for observation. He was released the next day.

Injured stated he was in process of cutting weeds using a weed eater. While doing so, he encountered some bees and was stung 6 or 7 times. Shortly thereafter, he had difficulty breathing and was taken to he hospital for an apparent allergic reaction.

EE stated after helping complete the preshift inspection on the 2800 electric shovel and after getting back on the ground, he proceeded to his dozer. While walking toward the machine, he stepped on a rock, which was under about 8 inches of wet mud, causing him to twist his right knee.

Employee stated he had gotten on the 2800 electric shovel to pick up the equipment preshift form. As he started to push down on the operators? cab door handle, his middle finger struck the corner of a bolt (head) that was sticking out, causing a laceration which required five (5) stitches.

TRAMMING 400 TO GET CLEAR OF SHOT. 2 LARGE ROCK TRUCKS COMING DOWN HILL WENT TOO HIGH TRAM SPEED TO GET CLEAR, RAN OVER LARGE IMBEDDED ROCK IN HAUL ROAD, CAUSING ERACTIC JAR .FELT PAIN IN LOWER BACK, WENT BACK TO SLOW SPEED WITHIN 100'.

EE received an electrical shock when his hand came into contact with one lead of a 3 phase high voltage power. EE received skin burns aqnd is expected to make a full recovery.

Employee stated he was performing early startup activities and as he was walking around a dozer blade to check oil levels, he apparently stepped on uneven ground, causing his knee to 'pop.' Shortly thereafter, he knee began to hurt and swell.

Employee apparently pulled a muscle in his lower back when he slipped on ice at the fuel farm. He had exited the cargo area of the fuel truck and when he stepped on the concrete pad, his foot slipped on the ice jerking his lower back.

The injured stated after performing his preshift inspection, he started to mount his rock truck. Upon reaching the third boarding step, the left side of the step broke, causing him to loose his balance and fall to the next rung. It is believed this incident aggravated his groin/hip area that was injured at the end of the previous shift.

Taking tilt hose loose, to allow nose cone to fall forward. Had to pry hose down out of the way. In doing so hose slipped hit Employee in the mouth, recieved 10 stitches to upper lip.

Employee stated that while pre-tripping his truck, he crawled under the front of his truck to make an inspection. When he began to exit he raised up too soon, striking his head on the bottom of the bumper.

While unloading the tanker at ICG, stretched right leg out to open the bottom hopper valve, cut right leg on a broken valve handle.

Injured and co-worker were working on a 785 rock truck engine. They had to work underneath the engine and were having problems trying to remove a piston. During the process, he overexerted or twisted his back while pushing up on a connecting rod. The pain grew worse as the shift proceeded. He worked the next shift but had to seek medical attention on Monday, 10/31/2005.

Employee was helping install a large pin in the bucket door for the 2800 P&H electric shovel. During the process, he was using a 20-pound sledgehammer & while striking the pin, a piece of metal flew off (pin), striking & penetrating his inside upper left leg. The piece of metal was successfully removed at the hospital the same evening; laceration required stitches.

The crew were removing `anfo mixture? from a disabled powder truck and placing it in the bin of an adjacent truck. The employee was standing on top of the powder truck and after placing the mixture in the bin, he lost his balance and fell (3 feet) down on the walkway. When he attempted to catch himself, he sustained a fracture to his left arm, right above the wrist.

Employee was in process of shoveling anfo mixture from a powder truck when his middle finger struck the open door of the bin. As a result, he sustained a laceration.

EE was beating on a hopper with a hammer to get the product to flow and smashed his right finger.

EE while lighting torch burn left hand, wasn't wearing gloves

EE and co-workers were in process of working on a 900 Komatsu loader bucket, installing a lip protector. AS he was striking the lip protector with a 16-pound sledge hammer, a piece of metal chipped off from the lip protector, striking and penetrating his left forearm. The piece of metal was successfully removed at the hospital the next morning.

EE stated that on the morning of the injury, a tire had to be removed from his grease truck. As he was pulling on tire, he felt pain in his lower back area. No report was filled out at the time of injury; safety department was informed on July 11, 2005. Note: Employee did not start missing work until August 25, 2005.

Employee received an award of compensation for Pneumoconiosis.

Employee was working on a gasoline engine for a mechanics truck. As he was picking up the intake manifold, his little finger on left hand apparently struck a burr or sharp edge on the gasket. As a result, he sustained a laceration that required six (6) stitches to repair. He was able to return to work on the same day.

Employee stated that on 05/28/2005, he was on the ground helping with the 2800 Shovel move. During this time, he thought he had gotten an eyelash in his eye. He continued to work with no problem until after shift and later in the night, his eye became irritated. The next day, he was taken to hospital where a rust particle was removed. He was given a prescription for eye drops.

Employee was stepping off the back of the truck and fell spraining/breaking ankle.

The injured, a lead blaster, was in process of helping load holes. After retrieving some surface delays and as he was walking around the corner of the power truck, he suddenly stepped in an undetected drill hole and fell to the ground. He subsquently sustained a fractured rib.

Employee was walking near where cutting edge bolts were being cut off. When last bolt was cut, the cutting edge flew off, striking his right thigh.

IT WAS APPARENT THAT EE AFTER LEAVING EE PERSONAL PICKUP TRUCK WAS STARTING ACROSS THE PARKING LOT WHEN EE LOST FOOTING AND FELL ON ICE

on 1-10-05, He was driving a rock truck and was a process of hauling material from the electric shovel to the backstack area for dumping. At the dumpsite, as the bed was being raised up about 8 feet, the frame suddenly broke and the bed abruptly fell back down on the truck. The impact of the bed striking the frame caused his neck to take a hard jolt. miss work on 8/3/05

While installing bottom structure cover (belly pan) to Cat. D11 bulldozer, chain slipped allowing cover to fall on left hand causing laceration & index finger to break.

EE was hooking light plant onto the trailer ball of the truck, light plant fell and smashed his finger and hand (right)

EE, injured, and co-workers had completed placing four (4) dragline bucket teeth on the shoe of the dragline. EE was in process of scooting them back when one tooth shifted, catching the top portion of his little finger on his right hand. He sustained a mashing type injury along with a broken bone.

EMPLOYEE STATED THAT WHILE ATTEMPTING TO TIGHTEN A LUG NUT ON A WHEEL, THE WRENCH SLIPPED. HIS FOOT SLIPPED, TWISTING HIS RT. ANKLE.

Employee received award of workers' compensation for hearing loss

Employee was working from a ladder, helping take bolts off the bearing cap from the dragline drum drive. As he was using a knocker wrench to help remove the bolts, it struck him on the little finger as it inadvertently fell to the floor.

Employee states that he was in process of unloading shovel bucket pins from the back end of his service truck. As he was rolling or scooting the pin from his truck, after it landed on the bumper, it either bounced or flipped and struck his right hand. Three of his fingers were mashed with the index finger being fractured.

Employee states that on July 8, 2004, while moving and handling de-watering pumps and waterline he strained his back. Employee continued to work and then on July 31, 2004, employee started missing work. Note: This injury was not reported to management until August 25, 2004.

Retired employee received award of compensation for occupational pneumoconiosis

Injured was in process of showing dozer operator work area. As they were walking, the injured stepped in an undetected drill hole, causing the injured to sustain a laceration on his lower leg.

Employee stated that he was in process of tightening roller cap bolts on a D-10 bulldozer. During the process, he inadvertently struck the trigger causing the impact wrench to thrust his right hand against the roller. He sustained a bruise and later on, was told that he may have a torn ligament in his knuckle area. Note: Employee did not start missing work until May 5, 2004.

Employee had been working on the radiator on 1068 D-9 dozer. As he started to stand up and straighen his leg, his knee suddenly popped. He was diagnosed on March 4, 2004 as having a meniscus cartilage tear. Note: Employee did not start missing work until March 5, 2004.

EE RECEIVED A WORKERS' COMPENSATION AWARD FOR HEARING LOSS.

EE AND CO-WORKER WERE WORKING ON HIGHWALL DRILL. DURING THIS TIME, THE WIND SUDDENLY INCREASED CAUSING DUST TO GET ON IN HIS FACE & EYES. LATER, HE NOTICED SOMETHING IN HIS EYE. THE NEXT DAY, HE WENT TO SPECIALIST WHERE EYE DROPS WHICH MADE THIS INJURY REPORTABLE.

EE WAS IN THE PROCESS OF CLIMBING DOWN FROM SCRAP METAL BIN WHEN HIS RING FINGER GOT CAUGHT ON THE CORNER CUTTING HIS FINGER. THE LACERATIONREQUIRED 6 STITCHES.

WHILE PERFORMING WORK ACTIVITIES AT THE SHOVEL WHEN MACHINE WAS DOWN FOR REPAIR, THE EE SUSTAINED A WELDING FLASH (REFLECTION) WHILE WELDING ACTIVITIES WERE BEING CONDUCTED.

ON OCTOBER 17, 2003, EE WAS DRIVING A ROCK TRUCK, HAULING MATERIAL FROM THE SHOVEL PIT TO THE BACKSTACK AREA. DURING ONE OF THE TRIPS, HIS NECK SUDDENLY "POPPED" AND LATER ON, HIS NECK BEGAN T O HURT WITH THE PAIN INTENSIFYING. HE WAS TAKEN TO THE HOSPITAL AND WAS TREATED BY HIS PHYSICIAN WHERE HE WAS DIAGNOSED AGAIN WITH THE SAME PROBLEM HE HAD IN MAY - CERVICALGIA.

EE STATES HE WAS IN PROCESS OF OPENING A DOOR ON THE WET BAG POWDER TRAILER AS HE TOOK HIS HAND OPEN THE DOOR THE HANDLE SUDDENLY CAME AROUND AND STRUCK HIM ON THE UPPER LIP, CAUSING A LACER ATION NOTE: PLEASE SEE ATTACHED MEMO

ON EVENING OF ACCIDENT, EE WAS ASSIGNED TO CLEAN HIGHWALL IN THE BEE RUN, UK BREAKDOWN AREA OF THE JOB. EE STATED THAT HE HAD CLEANEDN ABOUT 100 TON OF ROCK FROM THE FLOOR.EE WAS AWARE THAT THIS ROCK WAS LOOSE AND HIS INTENTIONS WAS A SCALE IT DOWN. HE PROCEEDED TO RAMP UP TOWARD THE ROCK AND AS HE WAS WORKING THE AREA AND WITH HIS BLADE UP, THE ROCK SUDDENLY SHIFTED AND BEGAN TO

EE WAS PERFORMING ROUTINE MAINTENANCE DUTIES. HE RAN OUT OF CLEAN CLOTHS AND ASKED A CO-WORKER TO HAND HIM SOME. CO-WORKER CAME DOWN STEPS & INJURED CAME UP STAIRWAY TO MEET HIM. INJURED START ED TO DESCEND STAIRWAY. ON HIS FIRST STEP BACKWARDS HE EITHER MISSED A STEP OR SLIPPED. HE STARTED TO FALL & CONTINUED UNTIL HE LANDED ON THE BOARDING PLATFORM BELOW(APPROX 4 FEET). STRAIN/BR

AT THE TIME OF THE INJURY, THE INJURED EE WAS OPERATING A CAT 793 ROCK TRUCK. HE WAS IN THE PROCESS OF BEING LOADED BY THE ELECTRIC SHOVEL. DURING ONE OF THE PASSES (SWINGS) THE SHOVEL STRUCK THE SIDE OF THE TRUCK BED. THE SUDDEN IMPACT CAUSED THE INJURED TO BE THROWN, RESULTING IN NECK & ARM PAIN.

EE (RETIRED) RECEIVED A WORKMANS COMPENSATION AWARD FOR OCCUPATIONAL PNEUMOCONIOSES

EMPLOYEE WAS IN PROCESS OF TIGHTENING A LUG NUT ON THE WHEEL OF A MACK COAL TRUCK WHEN THE "STUD" BROKE LOOSE CAUSING HIS WEIGHT TO SHIFT TO HIS RIGHT ANKLE. HE SUSTAINED SPRAINED ANKLE AS A R ESULT.

REMOVING A REAR WHEEL UNIT FROM A 785 ROCK TRUCK. SLIPPED AND FELL FROM LADDER, HITTING LEFT MIDDLE BACK ON FUEL TANK OF TRUCK. IT WAS SNOWING AT TIME OF ACCIDENT.

WHEN THE BINDER SHOT WAS DETONATED, FLY ROCK WENT MUCH FARTHER THAN ANTICIPATED AND CAME IN THE DIRECTION OF THE DRAGLINE, STRIKING THE MACHINE AND STRIKING EE'S DOZER. A ROCK THE SIZE OF A SO FTBALL CAME HURLING THROUGH THE SIDE GLASS AND STRUCK HIM ON TOP OF HIS FOOT. HE WAS TRANSFERRED TO HOSPITAL WHERE HE WAS DIAGNOSED WITH TWO BROKEN BONES ON THE TOP OF HIS FOOT.

ON SUNDAY, NOVEMBER 3,2002 APPROX. 8:45PM, EE OPERATING #06174 D-11 DOZER APPARENTLY SLIPPED WHILE DISMOUNTING THE MACHINE,STADDLED THE PUSH ARM AND THEN FELL TO THE GROUND. HE SUSTAINED A FRACTURED VERTEBRAE AND BECAME DISORIENTED DUE TO SEIZURES. HE WAS TAKEN TO WEBSTER COUNTY HOSP AND THEN TRANSPORTED TO CHARLESTON GENERAL. HE WAS RELEASED FROM THE HOSPITAL ON WEDNESDAY.

THE EE AND COWORKER WERE WORKING ON FAN ENDLOADER. THEY WERE IN PROCESS OF REMOVING A RACE (BEARING) BY HEATING IT WITH A TORCH. AS THE RACE WAS BEING HEATED, EE DECIDED TO POSITION HIMSELF IN TO THE CENTER AREA OF THE LOADER IN ORDER TO PRY ON THE RACE WITH A BAR. AS HE WAS STEPPING TOP THE CENTER AREA, SOME OIL WITHING THE SHAFT CAUGHT FIRE AND FLARED UP AND BURNT HIM ON THE NOSE

MECHANIC WAS IN THE PROCESS OF CHANGING A TIRE ON THE SERVICE TRUCK. AFTER LIFTING THE TIRE HE SAID HIS BACK AND STOMACH BECAME SOAR. HE WAS LATER DIAGNOSED WITH A HERNIA IN HIS STOMACH.

EE WAS DRIVING AN EMPTY TANDEM COAL TRUCK ALONG THE KNIFHT TRK HAULROAD AND DROVE THE TRUCK THROUGH A BERM (VEY HIGH) AND OVER A 200' EMBANKMENT. HE SUSTAINED A BROKEN GUMLINE BONE AND LACERAT IONS TO HIS CHIN, LIP AND HAND. EVIDENCE INDICATES AND INFORMATION GIVEN TO US AT THIS TIME IS THAT HE FELL ASLEEP AT THE WHEEL.

AWARD OF COMPENSATION FOR HEARING LOSS.

EE WAS STEPPING DOWN FROM A GENERATOR AND INADVERTENTLY PLACED HIS FINGER/HAND IN BETWEEN A LOOSE AND SECURED GUARD MASHING THE TIP OF HIS RING FINGER. THIS OCCURRED ON THE 1570 DRAGLINE ON MAINTENANCE DAY.

EE WAS DRIVING A 785 ROCK TRUCK, HAULING OVERBURDEN FROM A 5230 CAT SHOVEL. WHILE BEING LOADED BY THE SHOVEL A ROCK FELL OFF THE RIGHT SIDE OF THE BUCKET & LANDED ON THE TAIL BOARD OF THE ROCK TRUCK CAUSING THE OPERATOR TO GET "BOUNCED" IN HIS SEAT.

EE AND CO-WORKER WENT TO WORK ON A COAL LOADER, REPAIRING THE BACKUP ALARM. SHORTLY AFTERWARD, THEY CAME BACK TO THE WAREHOUSE AND STARTED GATHERING SUPPLIES TO PERFORM OTHER WORK. DURING THI S TIME AND WHILE EE WAS BENDING OVER TO PICK UP AN AIR HOSE, HE FELT A SHARP PAIN GO THROUGH HISLOWER BACK. HE WAS UNABLE TO SIT DOWN FOR A FEW MIN. EE RECEIVED X-RAYS AND MUSCLE RELAXERS.

EE RECEIVED A WORKMANS COMPENSATION AWARD FOR PNEUMOCONIOSIS.

EE & CO-WORKERS WERE REMOVING A BENT DRILL STEEL ON THE #4292 DRILL. AFTER PLACING DRILL MAST IN DOWN POSITION THE BOOM TRUCK WAS BEING USED TO REMOVE BENT SECTION. WHILE THE STEEL WAS BEING R EMOVED, IT GOT FOULED MOMENTARILY & WHEN IT SPRANG LOOSE IT SUDDENLY CAME IN HIS DIRECTION. WHEN MOVING TO GET OUT OF WAY HE WALKED INTO ANOTHER PIECE OF DRILL STEEL (EYE LEVEL) ON THE RACK.

AWARD OF WORKER'S COMPENSATION FOR HEARING LOSS.

EE WAS INJECTING AIR BRAKE DE-ICER INTO A FUEL TRUCK. EE STATED THAT SOMEHOW HIS HAND SLIPPED, ENGAGING THE AIR PRESSURE LEVER AND IN TURN CAUSED THE DE-ICER SOLUTION TO "BLOW" BACK INTO HIS FACE, WITH FORCE. THE SOLUTION WENT UP AND UNDERHIS SAFETY GLASSES AND GOT IN BOTH EYES. EE WAS TREATED (EYES FLUSHED) AND TAKEN TO HOSPITAL. HEWAS DIAGNOSED WITH A CORNEA ABRASION.

ON THE ABOVE DATE, EE, A ROCK TRUCK DRIVERS, WAS PERFORMING A PRESHIFT EXAM ON A ROCK TRUCK THAT HE WAS PREPARING TO OPERATE. SHORTLY THEREAFTER, HE WAS OBSERVED BY A COWORKER COLLAPSED OVER T HE STEERING WHEEL OF THE TRUCK. CPR WAS INITIATED & CONTINUED BY EMTS UNTIL HE ARRIVED AT THE HOSPITAL WHERE HE WAS PRONOUNCED DEAD. CAUSE OF DEATH WAS LISTED ACUTE CARDIOPULMONARY ARREST.

INJURED EMPLOYEE WAS WORKING ON HYDRAULIC SYSTEM ON 785 ROCK TRUCK. WHILE REMOVING A HOIST LINE (HOSE) THE '0' RING SUDDENLY BLEW WHICH CAUSED AN ENORMOUS AMOUNT OF PRESSURE TO BE RELEASED. TH E EMPLOYEE WAS STRUCK IN THE FACE BY THE HYDRAULIC FLUID AND KNOCKED HIS GLASSES OFF. APPARENTLY, SOME RUST WAS IN THE HYDRAULIC FLUID AND SOME GOT IN HIS EYE.

WHILE REMOVING AN ENGINE FROM A 777 ROCK TRUCK, EMPLOYEE WAS PULLING ON A WRENCH REMOVING A BOLT WHEN HE PULLED A MUSCLE IN THE LEFT SHOULDER AND RIGHT MIDDLE BACK.

EMPLOYEE RECEIVED A WORKER'S COMPENSATION AWARD FOR PNEUMOCONIOSIS.

DRIVER PULLED TRUCK IN FOR END LOADER TO LOAD & OPERATOR LEFT BOOM DOWN ON TRUCK, JERKING & TWISTING THE DRIVER INJURING HIS BACK & NECK.

EMPLOYEE WAS DISMOUNTING HIS COAL TRUCK AND WHEN HE STEPPED ON THE GROUND, HE APPARENTLY LANDED ON A ROCK AND TWISTED HIS KNEE.

MGMT. WAS NOTIFIED ON JUNE 19, 2001, BY EE, THAT HE WAS SCHEDULED FOR CARPAL TUNNEL SURGERY ON JUNE 27, 2001. (OCCUPATIONAL ILLNESS).

WHILE MONITORING AND PERFORMING WORK ACTIVITIES AT THE DRAGLINE ON MAINTENANCE SHUTDOWN DAY, THE EE APPARENTLY SUSTAINED A WELDING FLASH (REFLECTION) WHILE WELDING ACTIVITIES WERE BEING CONDUC TED.

EE HAD BEEN WORKING ON A WINDSHIELD WIPER ON THE #8 LOADER. AS HE STARTED TO DISMOUNT AND WAS DESCENDING DOWN THE BOARDING LADDER, HIS FOOT GOT CAUGHT INBETWEEN THE RUNG SPACING. WHEN THIS OCC URRED, HE SOMEWHAT SLIPPED CAUSING HIS WEIGHT TOSHIFT AND WHILE REGAINING HIS BALANCE HIS ONE ARM WAS MAINTAINING HIS WEIGHT AND THEREFORE STRAINED HIS NECK AND SHOULDER.

PUTTING BEAM INTO PLACE. BEAM WENT BACK IN PLACE AND CAUGHT FINGER BETWEEN OTHER BEAM.

LIFTING MOTOR OFF TRAILER, HE PULLED MUSCLES IN HIS BACK.

EEHIT ELBOW ON FEEDER WHILE TIGHTENING SPRING HANGER. DID NOT RECEIVE MEDICAL TREATMENT UNTIL 1/12/01.

INJURED STATED THAT HE WAS WORKING ON FUEL TRUCK TRANSMISSION. AS HE WAS PRYING A SMALL BAR, IT SLIPPED ACROSS HIS HAND, CAUSING A LACERATION.

ROAD WAS SLICK, AND EMPLOYEE'S TRUCK SLID THROUGH ROUGH PLACE, INJURING HIS NECK.

EMPLOYEE RECEIVED A WORKER'S COMPENSATION AWARD FOR HEARING LOSS.

ON THE DAY OF ACCIDENT "OFF DUTY" EE DROVE TO THE MINE OFFICE TO CHECK ON THE AVAILABILITY OF OVERTIME FOR THE FOLLOWING DAY. WHILE AT THE MINE OFFICE TALKING THE WAREHOUSEMAN HE WAS ASKED TO GO WITH HIM TO CHECK ON LEAKING AIRCONDITIONER. AS THE EE WAS LOOKING AT THE AIR CONDITIONER, HE INADVERTENTLY STEPPED FROM THE PLYWOOD FLOOR OVER ONTO THE CEILING TILE. AS A RESULT, HE FELL

REMOVING PARTS FROM OLD TRANSMISSION, TRIPPED OVER AIR HOSE IN FLOOR, LANDED ON RIGHT KNEE AND RIGHT WRIST.

EMPLOYEE STATED THAT HE WAS WORKING ON A 777 TRUCK. AS HE WAS CHANGING A GREASE HOSE AND REACHING ACROSS THE TRANSMISSION, HE FELT SOME- THING PULL IN HIS RIGHT SHOULDER/SIDE AREA.

EMPLOYEE EXITING A TRUCK AND TWISTED ANKLE

REACHING TO GET CHOKER AND GOT OFF BALANCE AND FELL. BROKEN WRIST.

EMPLOYEE WAS IN PROCESS OD STARTING EQUIPMENT AND SINCE THE ROCK TRUCK HE WAS WORKING ON WOULD NOT START HE CLIMBED THE BOARDING LADDER AND REMOVED THE OUTER AIR FILTER IN ORDER TO SPRAY STARTING FLUID. WHILE DOING THIS, HE LOST HIS BALANCE AND FELL APPROX 7 FT AND LANDED ON HIS FEET. HE SUBSEQUENTLY SPRAINED HIS LOWER LEFT LEG.

FEET SLIPPED ON WET EQUIPMENT CAUSING HIM TO FALL INJURING BACK AND KIDNEY.

EE WAS INSTALLING AN ENGINE IN A 785 TRUCK. EE SLIPED ON ICE AND FELL CATCHING RIGHT LEG BETWEEN STEERING JACK AND ROD FALLING BACKWARD, HANGINGBY LEFT UPSIDE DOWN.

EE WAS CUTTING OPEN EMULSION BAGS & PLACING THE MIXTURE IN BLAST HOLES. DURING THIS PROCESS, SOME OF THE EMULSION SPLASHED UP & WENT IN HIS OPEN MOUTH. HE SPIT MOST OF THE MIXTURE OUT EXCEPT F OR ABOUT 1/2 TEASPOON. SHORTLY THEREAFTER, HE GOT NAUSEOUS & COMPLAINED OF A HEADACHE. HE & THE MSDS SHEET WAS SENT TO CLINIC WHERE HE WAS GIVEN A SHOT FOR HIS HEADACHE.
