EE WAS A PASSENGER ON A PERSONNEL CARRIER EXITING THE SECTION AT THE END OF THE SHIFT. THE CARRIER DERAILED ALONG THE 7A TRACK AT BREAK 33, COLLIDING INTO A COAL RIB. THE IMPACT OF THE COLLISI ON JARRED HIM FROM THE PASSENGER COMPARTMENT ANDON TO THE TRACK RAIL. HE SUFFERED DEEP CONTUSIONS TO HIS UPPER AND LOWER BACK.
EE WAS A PASSENGER OF A PERSONNEL CARRIER EXITING THE SECTION AT THE END OF THE SHIFT. THE CARRIER DERAILED ALONG THE 7A TRACK AT BREAK #33, COLLIDING INTO A COAL RIB. HE WAS BOUNCED AROUND TH E INSIDE OF THE COMPARTMENT, AND SUFFERED A SMALL LACERATION TO THE BACK OF HIS HEAD. SEVERAL STITCHES WERE REQUIRED TO CLOSE THE WOUND.
THE EE WAS OPERATING AN UNDERGROUND FORKLIFT HAULING SUPPLIES TO THE ROOF BOLTER WHEN THE ACCIDENT OCCURRED. WHILE TRAMMING TOWARD THE FACE THE FORKLIFT STRUCK A ROCK ON THE MINE FLOOR IN THE #2 ENTRY CAUSING THE EE TO BE THROWN INTO THE UNDERSIDE OF THE OPERATOR'S CANOPY.
EMPLOYEE WAS AT THE SECTION DUMPING POINT TO ASSIST IN THE REMOVAL OF A PIECE OF ROCK THAT WAS FOULED IN THE FEEDER. EH WAS ATTEMPTING TO ACTIVATE THE FEEDER WATER SPRAYS WHEN HE STEPPED OTNO THE MOVING CONVEYOR BELT AND SPRAINED HIS LEFT KNEE. THE EMPLOYEE RECENTLY BEEN PLACED ON RESTRICTED DUTY DUE TO REOCURRING PAIN IN HIS KNEE.
EE WAS OPERATING A BROOKVILLE 6 TON MANBUS WHEN THE MANBUS WHEEL CONTACTED A LOOSE ANGLE BAR ONNTHE TRACK CAUSING THE MANBUS TO STOP SUDDENLY. THE SUDDEN STOP CAUSED THE EE TO STIKE HIS FORHEA D ONT HE CANOPY POST. THIS RESULTED IN A LACERATION WHICH REQUIRED STITCHES. THIS IS A DEGREE: 6, CLASSIFICATION 17 ACCIDENT
EMPLOYEE THE OPERATING A 10SC SHUTTLE CAR, MOVING FORWARD. THE MINER WAS BEING BACKED UP BY THE MINER OPERATOR HLEPER, EE WHEN THE TWO HIT EACH OTHER.
INJURED WAS OPERATING A SHUTTLE CAR ON THE 001-0SECTION AT SOLID EMERGY WHEN HE RAN OVER A ROCK IN THE ROADSAY CAUSING HIM TO BE THRUST AGAINST THE OPERATIOR'S CANOPY RESULTING IN A STRAINED N ECK. THE ROCK MEASURED 2'L X 2'W X 4" THICK.
INJURED WAS IN PROCESS OF HANGING MINER CABLE WHEN THE ELECTRICIAN OPERATING A 4 WHEEL PERSONNEL CARRIER STARTED BACKING UP AND STRUCK HIM IN THE LEG. THIS RESULTED IN A SPRAINED LEFT LEG.
EE SAID HE WAS OPERATING A SHUTTLE CAR, WHEN HE HIT A BUMP IN THE ROADWAY, CAUSING HIM TO HIT AGAINST THE CANOPY. EE DID NOT REPORT UNTIL THREE DAYS AFTER ACCIDENT SUPPOSEDLY OCCURRED AND BEIN G REPRIMANDED BY SUPERINTENDENT.
INJURED WAS HOLDING UPA VENTILATION CURTIAN, WHENA SCOOP CAME THROUGHT HE CURTAIN AND CAUGHT HIM WITH THE BUCKE TCAUSING INJURIES TO HIS RIGHT LEG.
EE WAS OPERATING A SHUTTLE CAR HE WAS IN THE PROCESS OF TAKING A LOAD OF COAL TO THE FEEDER. WHILE TRAVELING HE HIT A HOLE IN THE ROADWAY, THROWING HIM AGAINST THE CANOPY.
SUBJECT WAS TIGHTENING SCRAPER. A SPLICE HIT THE SCRAPER AND CAUSEA THE PIPE WRENCH TO KICK, CATCHING SUBJECTS RIGHT THUMB.
EE DID NOT REPORT INCIDENT AT TIME OF OCCURRENCE NOR AT THE END OF HIS SHIFT. HE DID NOT REPORT UNTIL 5:30 AM, 10-27-94. HE STATES HE WAS OPERATING A SCOOP. HE WAS HAULING ROCK, WHEN HE RAN OV ER A ROCK, RSULTING IN INJURY.
EMPLOYEE WAS RIDING IN MANTRIP WITH HIS FOOT STUCK OUT AND IT GOT CAUGHT BETWEEN THE RIB AND THE MANTRIP.
EE WAS ATTEMPTING TO PUT A PIN IN THE SCOOP BUCKET JACK. EE PULLED HYD LEVER IN ACTIVATING JACK, IT CAUGHT EE'S HAND BETWEEN THE JACK AND SCOOP BUCKET.
HE WAS OPERATING A SHUTTLE CAR IN THE #4 ENTRY. HE GOT AGAINST THE RIB, COAL ROLLED OFF INTO OPEATOR'S DECK. AS HE RAISED UP TO GET OUT OF THE WAY, HIS HEAD HIT THE CANOPY STRAINING HIS NECK.
EE WAS CHANGING ROLLERS OUT ON #3 BELT WHILE INSTALLING A ROLLER HIS LITTLE FINGER ON HIS RIGHT HAND GOT CAUGHT BETWEEN THE ROLLER & RAIL & LACERATED FINGER NAIL. REQUIRED STITCHES. NOTE: TH IS WAS EE'S LAST SHIFT @ THIS MINE THE ENTIRE CREW WAS LAYED OFF 3/21/94. HE REPORTED FOR WORK ON 3/21/94 BUT THERE WAS NO WORK BECAUSE OF LAYOFF.
EMPLOYEE WAS OPERATING A UNIHAULER & STATED SHE HIT A BUMP IN THE ROADWAY CAUSING HER HEAD TO STRIKE THE CANOPY OF THE HAULER.

INJURED WAS OPERATING A SHUTTLE CAR 1 BRK OUTBY THE FACE IN #5 ENTRY ON 002 SECTION. HE RAN OVER A ROCK IN THE ROADWAY, STEERING WHEEL CAME OUTOF HAND AND HIT HIS LEFT WRIST.
EE WAS HELPING UNLOAD A SCOOP OFF TRUCK IN SUPPLY YARD ON SURFACE. THE SCOOP WAS BACKING OFF TRUCK ON RAMPS MADE OF CRIB BLOCKS, ONE KICKED OUT AND HIT HIM ON RT. KNEE RESULTIN IN A BRUISE.NOT REPORTABLE TIL 10-28-93 WHEN WATER WAS REMOVED FROM KNEE. HE TRANSFERRED JOBS AND MISSED 2 DAYSWORK.
EMPLOYEE WAS OPERATING A 3 WHEEL PERSONEL CARRIER. THE FRONT WHEEL HIT A ROCK AND TURNED SIDEWAYS CAUSING THE STEERING WHEEL TO SPIN RAPIDLY THEKNOB ON THE STEERING WHEEL STRUCK HIM ON THE RIG HT HAND AND FRACTURED A BONE BEHIND HIS RING FINGER. IT REQUIRED A SMALL CAST.
INJURED WAS RUNNING A SHUTTLE CAR WHEN IT HIT A HOLE IN THE BOTTOM. IT BOUNCED HIM UP AGAINST THE CANOPY, HIT HIS HEAD AND STRAINED HIS NECK.
INJURED WAS RUNNING A SHUTTLE CAR IN THE #3 ENTRY WHILE TURNING THE CORNER TO GO TOWARD DUMP. HE TURNED TO SHARP AND CAUGHT THE DECK OF THE SHUTTLE CAR AGAINST THE RIB. IT BOUNCED HIM AGAINST THE SEAT. BRUISED AND STRAINED HIS BACK.
EE WAS UNLOADING SUPPLIES FROM FLAT CAR ONTO SCOOP HAD THE SCOOP BUCKET RAISED UP SCOOP ROLLED FORWARD CATCHING HIS LEGS BETWEEN SCOOP AND FLAT CAR RESULTING IN BRUISED AND LACERATED LEGS.
EMPLOYEE WAS USING A SCOOP TO CLEAN SECTION HE WAS PUSHING LOOSE COAL UP IN THE #7 HEADING WHEN WHEEL OF SCOOP HIT A ROCK & TURNED IT UP & HIT HIM IN THE BACK RESULTING IN BRUISES & ABRASIONS.
INJURED WAS HAULING ROCK WITH A SCOOP FROM TEH NO. 1 HEADING WHEN A ROCK FELL OUT OF THE BUCKET AND HE RAN OVER IT, WHEN THIS HAPPENED IT THROWED HIM AGAINST THE SEAT AND CANOPY RESULTING IN A BRUISED AND STRAINED BACK.
OPERATING SHUTTLE CAR (21SC) AND HTI HOLE IN ROADWAY. INDIVIDUAL WAS BOUNCED AND HURT HIS BACK (STAIN).

HE WAS OPERATIN1 THE SHUTTLE CAR AND HIT A BAD PLACE CAUSING HIM TO BOUNCE IN THE SHUTLE CAR. HE HIT HIS HEAD AGAINST THE CANOPY.
