RAN OVER ROCK IN HAULROAD WHILE OPERATING RAM CAR. HE BOUNCED OFF SEAT STRIKING HEAD ON CANOPY CAUSING CERVICE STRAIN. HE WAS TAKEN TO HOSPITAL AND PLACED ON RESTRICTED DUTY UNTIL 5-22-98.
THE EE WAS TRAMMING A SCOOP WHEN HE RAN OVER A CRIB BLOCK CAUSING THE OPERATOR TO JAM HIS HEAD AGAINST THE CANOPY. THIS HAPPENED ON #1 SECTION 3 BRREAKS INBY END OF TRUCK. THIS BECAME A LTA 05-18-989
THE EMPLOYEE WAS TRAMMING AN EMPTY SHUTTLE CAR TOWARD THE MINER WHEN HE HIT A HOLE IN THE ROADWAY, THROWING HIM INTO THE CANOPY. THE SEAT CAME UP AND HIT HIM AS HE WAS GOING DOWN. THIS HAPPENE D AT THE BELT HEADING, 1 BREAK INBY THE FEEDER.
THE EMPLOYEE WAS RIDING THE MANBUS OUTSIDE FROM 02 SECTION WHEN SOMETHING GOT IN HIS RIGHT EYE. HE WAS WEARING HIS SAFETY GLASSES. MEDICATION WAS GIVEN.
EE MOVING COAL WITH 980 F CATERPILLAR ENDLOADER. EE STATES HE POSSIBLY HAD 2 LOADER TIRES ON BANK & HE HAD BUCKET IN AIR WITH A LOAD IN IT. THE ENDLOADER TURNED OVER ON ITS SIDE.
HE WAS MOVING CABLE BACK WITH A SCOOP. WHEN HE TURNED TO WATCH THE MINER CABLE SLACK, THE SCOOP TRAMMED OVER IS RIGHT FOOT CAUSING A SEVERE BRUISE.
EE WAS IN A ROVER PULLING WIRE ROPE FROM #2 HEAD. THE ROPE CAUGHT ON SOMETHING CAUSING THE ROVER TO SUDDENLY STOP CAUSING HIS HEAD TO HIT INSIDE THE CANOPY OF THE ROVER CAUSING A LACERATION TO THE LEFT SIDE OF HIS HEAD, PAIN TO LEFT WRIST AND LEFT ELBOW.
EMPLOYEE WAS OPERATING A SHUTTLE CAR AND HIT A HOLE IN THE ROADWAY CAUSING HIM TO HIT HIS HEAD INTO THE CANOPY INJURING HIS NECK.
EMPLOYEE WAS REMOVING A SHUTTLE CAR CANOPY. AS HE WAS LOWERING THE CANOPY, HE GOT HIS LEFT LITTLE FINGER CAUGHT BETWEEN THE CANOPY AND FRAME MASHING AND CAUSING A LACERATION REQUIRING SUTURES.
EMPLOYEE WAS IN PROCESS OF BLEEDING AIR FROM THE FUEL INJUECTORS ON THE TOYOTA SKID LOADER(BOBCAT). AS HE WAS PERFORMING THE TASK THE SMALL CRESENT WRENCH HE WAS USING STRUCK THE ENGINE BLADE WHICH CAUSED THE WRENCH TO SUDDENLY GLANCE BACK STRIKING HIS HAND. APPARENTLY THE SHARP EDGES OF THE JAW END OF THE WRENCH CAUSED THE LACERATIONS.
DRIVING THE SCOOP TRHOUGH A CROSSCUT. THE BOTTOMS WERE UNEVEN. THE SCOOP STRUCK A HOLE AND IT BOUNCED CAUSING EE TO STRIKE HER HARD HAT AGAINST THE FRAME OF THE CANOPY. THIS CAUSED A STRAIN TO THE NECK.
EE HIT A HOLE IN THE ROAD WAY IN #3 ENTRY BOUNCING HIM INTO THE CANOPY OF THE SHUTTLE CAR CAUSING HIM TO HAVAE PIAN IN THE NECK & BACK AREQA.
EE WAS RETURNING FROM A MOBILE PATROL OF THE JOB SITE. EE WAS TRAVELING AT TOO HIGH A RATE OF SPEED. HE LOST CONTROL OF THE PICKUP ON THE GRAVELED AREA.
THE EE WAS TRAMMING DOWN #7 ENTRY & RAN OVER A BENT STRAP; IT FLIPPED UP & CUT HIS CHIN REQUIRING STITCHES. REPORTABLE ONLY.
INJURED STATED THAT HIS LOWER BACK & BUTTOCKS WAS HURTING HIM, WHILE HE WAS OPERATING A SHUTTLE CAR. [UNDER INVESTIGATION SINCE 7-9-97]
INJURED EMP WAS INSTRUCTING A CONT MINER OPERATOR IN THE NEXT MOVE. SHUTTLE CAR WAS HEADING INTOTHE CONT MINER. AS SHUTTLE CAR PULLED THROUGH PULL THROUGH CURTAIN INJURED EMP WAS STRUCK BY SHU TTLE CAR IN THE RIGHT LOWER BACK.
RAN OVER ROCK WHILE DRIVING RAM CAR JAMMING HIS BACK. THEN FELL AS HE GOT OUT OF CAR HURTING HISKNEES.
EE WA OPERATING HIS VEHICLE WHEN HIS VEHICLE CAME IN CONTACT WITH THE END OF A GATE, WHICH PENETRATED INTO THE CAB OF HIS VEHICLE.
EE WAS IN THE PROCESS OF WATERING THE BUGGY HAULROAD THE SHUTTLE WAS TRAMMED INBY TOWARD THE FACE. AS THE SHUTTLE CAR TRAVELED THROUGH THE CROSSCUT THE CABLE SHIPPED ACROSS THE ENTRY STRIKING THE EE CAUSING HIM TO FALL, LANDING ON HIS CAP LAMP BATTERY
THE EE WAS OPERATING A SHUTTLE CAR WHEN HE HIT A HOLE IN THE ROADWAY & HIT HIS HEAD INTO THE CANOPY. HE WAS WORKING ON #1 SECTION.
THE EE WAS TRAMMING A SHUTTLE CAR TOWARD THE MINER ON #2 SECTION/#5 ENTRY WHEN HE HIT A HOLE & STARTED HAVING PAIN IN HIS UPPER BACK. THIS WAS ORIGINALLY THOUGHT TO BE AN OCCUPATIONAL ILLNESS RESULTING FROM REPEATED TRAUMA FROM PREVIOUS BACK PROBLEMS, HOWEVER AFTER BEING EXAMINED THE RESULTS FROM THE DR. SHOWED THIS TO BE A SPRAIN, STRAIN TYPE OF ACCIDENT.
RAN OVERHOLE IN HAULAGE ROAD CAUSING SEAT IN BATTERY RAMCAR TO BOTTOM OUT JAMMING BACK. NO SCHEDULED TO WORK UNTIL 12-30-96. BACK WAS STILL HURTING 1-2-97 SO HE WENT TO DR AND FILED ACCIDENT R EPORT. DR PLACED HIM ON RESTRICTED DUTY BEGINING1-3-97.
ROCKDUST TRAILER HE WAS PULLING CAME UNCOUPLED. HYDRAULIC HOSE FROM SCOOP TO CAR STRETCHED THEN BROKE. THE COUPLER STRUCK HIM ON CHIN CAUSING LACERATION THAT REQUIRED STITCHES.
EE WAS BEING TRANSPORTED IN 107 TRACK SCOOTER TO BIG MUNCY MAINS WHEN THE TROLLY POLE DISENGAGED FROM THE TROLLY WIRE AND DISLODGED A LAMINATED LAYER OF SANDSTONE MEASUING APPROX. 1" THICK X 2 0" LONG X 20" WIDE. THE EDGE OF THE DRAW ROCK STRUCK THE EE A GLANCING BLOW ACROSS THE TOP OF THE LEFT WRIST CAUSING A LACERATION.
WHILE DRIVING BATTERY RAMCAR, EE RAN OVER A POT HOLE IN HAUL ROAD. THIS CAUSED EE TO BOUNCE OFF SEAT STRAINING HER BACK. TREATED & RELEASED ON THIS DATE BUT CONT TO HAVE PAIN AND REVISITED DR ON 11/11/96. BED REST FOR 2 DAYS THEN RESTSRICTED DUTY.
EMP WAS WORKING IN #5 ENTRY ON 04-TOMS BRANCH, HELPING TO RESET THE BREAKER ON A SHUTTLE CAR. WHEN CAR STARTED UP CABLE HIT HIM IN MOUTH AND NOSE CAUSING FRACTURES OF THE FACE AREA.
EMPLOYEE WAS OPERATING A SHUTTLE CAR WHEN HE STRUCK ROAD IRREGULARITIES WHICH CAUSED THE EMPLOYEE TO STRIKE AGAINST THE UNDER SIDE OF THE CANOPY. EMPLOYEE SUFFERED A NECK STRAIN.
EE WAS IN #4 ENTRY/15BRK ON #10 BELT WHEN THE SHUTTLE CAR WAS STARTED. THE CABLE CAUGHT THE EE'S LEGS AND FLIPPED HIM CAUSING PAIN IN THE BACK OF HIS HEAD.
EMPLOYEE WAS WALKING BY A SHTTLE CAR ANCHOR WHEN THE CAR WAS STARTED CAUSING HIM TO FALL OVER THE CABLE HURTING HIS LOWER BACK. HE WAS AT THE POWER CENTER ON 002 SECTION.
EE WAS STEPPING ONTO THE SLOPE BELT TO EXIT THE MINE AT THE END OF THE SHIFT. AS HE STEPPED ONTO THE BELT HE LOST HIS BALANCE, SET DOWN ON THE BELT , AND BOUNCED OFF THE BELT FALLING TO THE MI NE FLOOR LANDING ON HIS HEAD. HE STRUCK HIS HEADAGAINST THE MINE FLOOR AND THE 4" WATERLINE. THE INJURED EE RECEIVED DAMAGE TO THE #4 VERTEBRA. (CERVICAL).
EMP WAS DOING HIS NORMAL JOB OF RUNNING THE SHUTTLE CAR ON 001 SECTION WHEN HE BEGAN FEELING PAIN IN HIS MID AND LOWER BACK. THERE WAS NO INCIDENT TO CAUSE THIS TO HAPPEN. HAS HAD BACK PAIN BE FORE. REPORT WAS NOT FILED UNTIL THE DRS REPORT AND DIAGNOSIS FROM RADIOGRAPHS WERE RECEIVED. THIS SHOWED A PROBABLE ANTERIOR COMPRESSION FRACTURE OF L1 AND WAS MADE 8-8-96.
EMP WAS PULLING THE PANLINE BACK AND A RAM BOX ON THE SHIELD FEET STRUCK HIS RIGHT ANKLE CAUSINGA CHIP FRACTURE TO THE ANKLE.
EMP WAS HELPING BLOCK UP TRACK TO GET THE SCOOP ACROSS. SCOOP MOVED PUSHING THE BLOCKING AGAINSTMETAL RAIL CUTTING THE TIP OF EMPS RIGHT MIDDLE FINGER. DID NOT BECOME A LTA UNTIL 7-22-96.
HE WAS OPERATING SCRAPER HAULING REFUSE, HAULROADS AHD BEEN WATERED CAUSING SCRAPER TO SLIDE. WHEN SCRAPER SLID, EE STRUCK DOOR HANDLE. THE IMPACT CAUSED FRACTURE TO RIBS.
EE WAS HELPING MOVE THE CONTINUOUS MINER CABLE. AS HE BENT OVER TO TIE THE PULL ROPE A SHUTTLE CAR CAME DOWN THE HAUL ROAD AND THE SHUTTLE CAR CABLE STRUCK HIM ON BOTH KNEES, KNOCKING HIM DOWN .
EE WAS INJURED WHILE REPAIRING A SHUTTLE CAR. THE WORK AREA WAS BEHIND A CHECK CURTAIN, THE INJURED WAS STRUCK BYA SCOOP WHEN IT WAS TRAMMED THROUGHT THE CHECK CURTAIN PINNING THE EE BETWEEN T HE SCOOP AND SHUTTLE CAR. THE EE WAS ADM. HAD TOA LOCAL HOSPITAL WHERE IT WAS DETERMINED NO SUFFORED NO MAJOR INJURIES.
USING CHAIN TIED TO SCOOP AND SLATE BAR TO DISLODGE A LARGE PIECE OF ROCK FROM MINER CONVEYOR. CHAIN SLIPPED AS HE WAS PRYING WITH SLATE BAR, CAUSING BAR TO FLIP UP STRIKING HIM ON RIGHT SIDE OF FACE, FRACTURING HIS JAW.
EMPLOYEE WAS WORKING ON THE WINDSHIELD WIPER SYSTEM ON A WHEEL LOADER. WHEN HE ACTIVATED THE WIPER TO ADJUST/APPLY A RETAINER CLIP, HIS FINGER WAS PINCHED BY THE WIPER LINKAGE ARM. HIS LEFT MI DDLE FINGER WAS LACERATED AND BONE CRACKED.
EE WAS STANDING 2' INGBY PULL THRU CURTAIN IN #4 ENTRY OF 5L2MMU STATION 4+35 TALKING TO PRODUCTION COORDINATOR. A RAM CAR PULLED THRU CURTAIN AND BUMPED BOTH MEN. IN AN ATTEMPT TO GET OUT OF WAS, EE STRAINED HIS RIGHT KNEE AND WAS PLACED ON RESTRICTED DUTY.
WHILE LEAVING THE FEEDER ON A SHUTTLE CAR THE CAR HIT A HOLE IN THE ROAD CAUSING HIM TO HIT HIS HEAD AGAINST THE CANOPY AND PAIN IN THE NECK.
EMPLOYEE WAS PULLING ON A CABLE WITH A ROPE ON #2 SECTION #9 ENTRY. THE ROPE WAS LOOPED ON ONE END AROUND A BAR IN THE SHUTTLE CAR WITH A HAMMER HANDLE IN THE LOOP AROUND A BAR IN THE SHUTTLE CAR TO SECURE THE ROPE. AS THE SHUTTLE CAR PULLED THE CABLE THE BAR BROKE ALLOWING SOMETHING TO STRIKE HIM IN THE LEFT SIDE OF HIS HEAD RUPTURING HIS EAR DRUM.
EMPLOYEE WAS STRUCK BY A BUGGY CABLE WHICH CAUSED HIM TO FALL TO THE WALKWAY AND LANDED ON HIS SELF-RESCUER.
EMPLOYEE IS CLASSIFIED AS A SURFACE UTILITY MAN, HE WAS GREASING A TOP ROLLER AT THE TAIL OF C-BELT, HE WAS USING A HAND HELD GREASE GUN WITH A 18" HOSE, THHE HOSE CAUGHT BETWEEN THE BELT AND A ROLLER, IT MAY OF PULLED THE HAND HOLDING THEU HOSE INTO THE BELT, A KILL ROPE AND A SWITCH WERE IN THE IMMEDIATE AREA BUT WERE NOT UTILIZED, THEH BELT RUNNING OVER HIS HAND INFLICTED BURNS
EMPLOYEE IS CLASSIFIED AS A CONTIUOUS MINEER OPERATOR, EH WAS RIDING OUT OF THE MINE AT THE END OF THE SHIFT ON A DIESEL MANTRIP, WHEN GOING THROUGH THE INBY WOODEN AIRLOCK DOOR ON THE MINE BO TTOM, HIS LEFT FOOT CAUGHT BETWEEN THE EDGE OF THE DOOR AND THE MANTRIP, HIS FOOT WAS NOT INSIDE THE MANTRIP, IMPACT RESULTED IN A FRACTURED DISTAL PHALANX ON THE 4TH DIGIT ON HIS LEFT FOOT.
EE WAS OPERATING A SHUTTLE CAR WHEN HE STRUCK A HOLE IN THE ROADWAY THIS CASUSED HIM TO BOUNCE IN THE SHUTTLE CAR CAB CAUSING HIM TO STRIKE THE SHUTTLE CAR CANOPY WITH HIS HEAD.
EE WAS WAITING FOR ANOTHE EE TO OPEN A TRAP DOOR. THE VELOCITY OF AIR CAUSED BOTH DOORS TO SHUT ON THE EE'S HAND. SEEING WHAT HAPPENED, EE JUMPED OUT OF THE VEHICLE HE WAS IN AND RAN TO THE DO OR TO HELP GE THE OTHER EE OUT AND THE VEHICLE ROLLED DOWN AND CAUGHT EE BETWEEN THE DOORS AND THE D.U.V.
SMOKE INHALATION--CABLE REEL GOT HOT ON SHUTTLE CAR. TRANSPORTED TO HOSPITAL/ OXYGEN WAS ADMINISTERED.
EMPLOYEE ALLEGED THAT SHE WAS BACKING AWAY FROM THE MINER WHILE OPERATING A SHUTTLE CAR WHEN SHEHIT THE COAL RIB.
