Scoop operator was hauling supplies to the section and the roof bolter cable got hung on the canopy of the scoop, operator tried to move the cable and his finger got smashed between the canopy of the scoop and the cable.

HAULAGE SYSTEM WAS MOVING BACKWARDS FROM THE FACE WHEN THE BRIDGE OPERATOR WAS INJURED AS BRIDGE IN FRONT OF HIM FORCED FRAME OF BRIDGE INTO LEG.
THE INJURED WAS OPERATING A LOCOMOTIVE TO PUSH A ROOF BOLTER BACK TO CONNECT IT TO THE SLOPE HOIST. THE TRS PAD ON THE HELPERS SIDE STRUCK A STEEL LEG CAUSING THE BOARDS AND ROCK ON TOP OF THE M TO FALL FROM BETWEEN THE BEAMS STRIKING THE INDIVIDUAL.
EE WAS SEATED IN A MANTRIP OPERATORS COMPARTMENT WHEN THE MANTRIP INBY HIM, WHICH WAS BEING MOVED BY ANOTHER WORKMAN, INADVERTENTLY WAS PLACED INTO REVERSE TRAM AS THE OPERATOR SET THE BREAKER . BEFORE THE OPERATOR COULD STOP THE MANTRIP IT HAD STRUCK THE INJURED'S MANTRIP. BOTH MEN WERE SEATED.
EMPLOYEE WAS RIDING IN PASSENGER AREA OF 2 MAN- TRIP WHEN HE STRUCK HIS HEAD AGAINST A URNT PIPEWHICH CROSSED OVER THE TRACK. THE PIPE HAD DROP-PED DOWN AND THE OPERATOR TRIED UNSUCCESSFULLY, TO STOP. NOTICE: BEGAN MISS WORK AGAIN ON 11/6.
SHUTTLE CAR DROPPED INTO HOLE IN PAVEMENT CAUSING OPERATOR'S HEAD TO BE JAMMED UP AGAINST CANOPY.
