Right shoulder had been rubbing on seat in 2004 Cat 980G Loader. Employee reported on 11/28/07 that shoulder was swollen & sore and felt like a pinched nerve. Employee was sent to Occupational Health appointment on 11/29/07. Returned to work with restriction not to lift above shoulder. Was told to contact family Dr on high blood pressure.
AN ENGINE WARNING LOGHT CAME ON INSIDE THE DRIVERS TRUCK. DRIVER STOPPED THRUCK TO LOOK UNDER THE HOOD. DRIVER COULD NOT SEE VERY GOOD. WHILE REACHING INTO ENGINE AREA TO GET A BETTER LOOK AT THE COOLANT BOTTLE THE FAN BELT,FAN BLADE,OR SOMETHING GRABBED HIS MIDDLE FINGER ON HIS LEFT HAND CUTTING TIP APPROXIMATELY 1".
UNKNOWN AT THIS TIME. COULD BE A REOCCURANCE OFL AN OLD BACK PROBLEM.
EMPLOYEE WAS SHOVELING TAIL PULLEY THE SHOVEL GOT CAUGHT IN TAIL PULLEY AND EMPLOYEE TRIED TO PULL IT OUT HE THEN GOT HIS LEFT THUMB PINCHED BETWEEN SHOVEL AND TAIL PULLEY THIS TOOK ABOUT 1/ 4 OF MEAT OFF HIS LEFT THUMB
