Kenworth blast truck was being used to load holes on a shot on the G haul road when driver lost control and rolled down a steep ramp tipping truck up onto its side at the bottom of the ramp. The driver was pinned under the steering column for approximately 2 hours. Employee was transferred by helicopter to Eastern Idaho medical center.

Mostrando los 14Employee was in the shuttle bus ridding back from pit area to the main mine parking area. The bus hit a rut in the road and as a result the employee hit left elbow on window frame, needing to see the Doctor. The Doctor could not verify the elbow was fractured but recommended light duty for a few days.
Employee was operating a Cat 988 loader feeding an ore hopper. When employee was digging into the stock pile a large rock was buried in the pile and he hit it with the loader jamming right arm and shoulder back causing moderate pain in shoulder. At the time no first aid but turned into a lost time on 1/18/2008.
Employee was trying to adjust the louvers in the heater vent of a Cat 988 loader, when his fingers, (pointer and middle) came in contact with the heater fan. Employee received 10 stitches in fingers.
Employee had hood up on company truck to check engine oil when shocks failed and hood fell onto employee's back. Employee worked his regular shift on 2-8-06 and 2-9-06. He worked 3 hours on 2-10-06 and then went to the doctor.
Employee was operating loader when his eye became irritated.
Ee was dumping a load of waste & on his way back to pick up a load of sand he hit a pot hole in road. Ee originally went to Dr on 9/15/05 for exam on this injury & since no observations were made, we considered this as diagnostic only - no lost time or medical treatment. This is a reoccurance of the same injury. EE is now losing time as of 3-13-06 for this injury.
While operating loader, employee complained of numbness in leg and arm because the seat in the loader was bouncing too much.
Employee said back was too sore to work.
WHILE SWINGING SNOUT ON LOADOUT THE GUARD AROUND SPROCKET CAUGHT ON THE CHAIN DRIVE, WHEN IT CAME LOOSE, IT SLAPPED HIM ON THE SIDE OF FACE
EE was moving a trommell screen assembly with an overhead crane when the weight of the screen shifted and hit EE in the left leg and right foot. EE was knocked to the ground by the screens.
BELLY DUMP GATES DIDN'T OPEN. TOLD DRIVER TO GO TO YARD. HE DID NOT GO TO YARD BUT TRIED TO OPEN GATES HIMSELF BY BUMPING THEM. DRIVER PUT TRUCK ON UNLEVEL GROUND, TIPPED TRUCK ON IT'S SIDE. POOR ACTION ON PART OF DRIVER.
EE was operating a 988 cat loader. The seat was loose causing him to rock back and forth. he injured his back. EE went to see the Dr on 3-9-04. he continued to work until 3-19-04. On 3-22-04 he was taken to the hospital and has not returned as of this report.
MOVING 4" SQUARE TUBING WITH A FORK LIFT. TOP PIECE SLID OFF AND PINCHED HAND AND WRIST BETWEEN THE FORK AND THE TUBING.
