the miner operating a Genie S-85 manlift. While he was moving it from one location to another the front wheel fell into a washout in the roadway. Which caused him to fall in the basket injuring his left shin.

While moving rock from stope to crusher staging area a rock fell out of the bucket. On returning he did not see the rock and ran over it causing him to loose control and hit the rib with his bucket. The jarring motion caused his leg to hit the wall of the cab.
During course of shift, EE alleges to have twisted his back while running a loader. He cannot recall a particular event that caused the injury.

ON MONDAY JULY 20, I,NAME CALLED AND SAID HIS BACK HURT AND HE GUESSED IT MUST HAVE CAME FROM WORKING T HE WEEK BEFORE 3O REPORT WAS FILED AT THE TIME BECAUSE IT WAS TURNED OVER TO PECKHAMS INSURANCE CARRIER TO INVESTIGATE.
WHILE DOING MAINTENENCE ON PAYLOADER. INJURED WAS CHECKING RADIETOR. RADIETOR WAS NOT COOLED DNOUGH HOT ANTIFREEZE BOILED OUT CAUSING BURNS TO LEFT FACE ARM & BACK.
