The miner was using a walk behind skid steer to clean out a tunnel. While moving under a conveyor tail pulley the skid steer ran over a rock throwing the control end up into a guard. The miner broke their left arm near the wrist.

HIS HELMET MAY HAVE OBSTRUCTED HIS VISION, CAUSING A JUDGEMENT ERROR ON LOCATION OFMANLIFT'S BASKET. WHEN ADJUSTING THE BASKET, HE BECAME PINNED BETWEEN BASKET & I-BEAM, CRUSHING.
