Employee was walking in the warehouse and a forklift came up beside employee and made a hard turn to the right and hit employee on the right side of body. It knocked employee down on the floor. Employee sprained left ankle. Employee is on restricted work duty.
Employee entered the wrapper to remove an empty pallet. He failed to lock-out tag- out and entered the conveyor from the top walking on the middle of the belt when he removed pallet the conveyor started and pinned his right leg between the exit conveyor and the gravity rollers. He was taken to ER and fallowed up with company Dr. 08/29/2014. He is on restricted work duty.
Employee injured back when struck by pallet of plywood when forklift tail-swing bumped into the stack on opposite side. Soreness only symptom. Internal First Aid report was submitted. Returned to work next scheduled day. Nearly two months later, he saw a doctor with soreness in back. Occupational doctor released employee to work with lifting and bending restrictions on 4/10/14.
While re-positioning the forks on JLD hydraulic forklift, one of the forks got stuck in the out of place position. When EE reached in with left hand to free it the fork came loose and dropped in position smashing his little finger on his left hand. Sutures were required.
Employee was removing a stuck bag of salt at the eject conveyor on the Bernis palletizer. When the conveyor came up, his left arm was pinched between it and the next conveyor in line. Employee had lacerations to forearm and small hairline fracture on top side of hand. Dr at ER released him to work restricted duty 9/3/2008.
Employee was opening railcar door when the loader operator hit the coupler with loader, closing the door on his right little finger (open fracture at the first joint of the little finger).
Ee stated that the upper waste screw for cyclone choked down. He thought the motor kicked heaters, so he lifted the cover up from screw and screw caught his glove on right hand. Cutting the first joint off of third finger and cut ring finger.
EMPLOYEE STATED HE WAS CLOSING DOOR TO THE BAGGING MACHINE MAGAZINE FROM THE SEAT OF THE FORKLIFT, WHEN HIS FOOT SLIPPED OFF OF BRAKE. FORKLIFT WENT INTO FORWARD MOTION PNCHING HIS RIGHT HAND BETWEEN FORKLIFT FALL PROTECTION CAGE AND THE EGDE OF DOOR. EMPLOYEE WILL BE ON MODIFIED DUTY.
EE STATED HE SAW A PIECE OF PAPER GOING AROUND THE HEAD PULLEY ON JOHNSON CONVEYOR BELT. HE REACHED IN TO GRAB IT WHEN THE SIDE OF PULLEY CAUGHT HIS RIGHT GLOVE AND HAND PULLING IT INTO SIDE O F BELT. EE REQUIRED SUTURES BELOW THE THUMB ON HIS RIGHT HAND. PANIC BUTTON TO SHUT DOWN BELT WAS ACTIVATE BY EE.
EE WAS CLEANING STACKING TABLE ON INCLINE BELT TO THE BEMIS BAGGER, WHEN HIS GLOVE GOT CAUGHT AGAINST BELT AND PULLED HIS FOREARM UNDER BELT ROLLER.
EMPLOYEE WAS MOVING A RAILCAR WITH A TRACK MOBILE, WHEN HE STRUCK HIS HEAD OUT THE BACK OPENING OF THE CAB, WHEN HE MADE CONTACT WITH OTHER CARS AHEAD. HIS HEAD WAS THROWN INTO THE CAB FRAME.
OPERATOR WAS TRYING TO LIFT A LARGE SALT ROCK WITH A CAT 966C FRONT END LOADER. THE BACK-END CAME UP AND THREW THE OPERATOR OUT OF THE CAB. HE DID NOT WEAR A SEAT BELT. THE OPERATOR SUSTAINED A BROKEN COLLAR BONE, SMALL LACERATION TO THE HEAD AND BRUISES.
EE WAS DRIVING FRONT END LOADER WHEN HIS BACK BEGAN HURTING.
EE WAS STACKING BAGS FROM A BELT AND TABLE WHEN A SPLICE CAUGHT HIS GLOVE AND PULLED HIS FINGERS INTO A PINCH POINT BETWEEN BELT AND TABLE. STITCHES REQUIRED ON LITTLE FINGER.
EE DROVE TRUCK OFF ROAD TO TAKE A SHORT CUT TRUCK FELL THRU SOFT SALT SEAT BELT HURT HIS STOMACH. HE STOPPED TO CHECK AND FELL OUT OF THE TRUCK ON THE HARD SALT.
INJ OPERATING LOADER ..BUMPY ROAD CONDITIONS CAUSED HIM TO BOUNCE AROUND IN SEAT AND CAUSED HIS BACK TO HURT.REPORTED CONDITION 9-15 SENT TO DR COMPLAINT OF SORENESS IN HIS BACK.

EMPLOYEE WAS DRIVING A FORKLIFT TO THE GAS PUMP, HIS FORLIFT STALLED IN A PILE OF SALT, HE GOT OFF LIFT TO CLEAR SALT AWAY FROM HIS WHEELS WHEN A FRONT END LOADER HIT THE FORKLIFT WITH ITS TIR E CAUSING THE FORLIFT TO ROLL OVER THE INJURED\S FOOT.
EMPLOYEE WENT UP A STATIONARY BELT CONVEYOR TO GREASE THE HEAD PULLEY. THE BELT STARTED AND THREW HIM OFF OVER THE CHUTE AND ONTO A SPIRAL SAOLT CLASSIFIER, THAT WAS RUNNING SUFFERED A SEVEREL Y LACERATED DELTOID MUSCLE OF HIS RIGHT ARM, BRUISES AND ABRASIONS ON HIS CHEST AND ARMS AND LEGS.
