HIS TRUCK SPEED OUT 180 AND HIS RIGHT SIDE TIPES HIT SOFT TAILS CAUSING THE TRUCK TO DUMP SLOWLY ON ITS R IGHT SIDE. HE STRUCK HIS HEAD ON THE PASSENGER WINDOW CAUSING A LACERATION REQUIRING SEVERAL STITCHES. HE CLAIMED HE HAD HIS SEAT BELT ON.

ING WAS OPENING DOOR TO STEAM ROOM TO ALLOW TRUCK TO BE DRIVEN IN GRATES HAD BEEN REMOVED FORM FLOOR SUMP TO ALLOW SUMP TO BE HOSED OUT AND WERE STACKED NEAR INJ TRCK WINDOWS FOGGED FROM STEAM AND DRIVER DROVE OVER LOWER GRATE CASUING TIP GRATE TO CATAPULT ONTO INJ FOOT FRACT 3 MEATARSALS.
INJ REMOVING FROZEN CHUMB FROM FINE ORE FEEDER WITH BELT RUNNING AND HAND WAS PINCHED AGAINST PAD EDGE WELDED TO FRAME.

GROUND WATER LEAKING INTO THE TUNNEL TURN ED TO ICE & CO5ERED THE WALKWAY. INJURED SLIPPED ON THE ICE HIS LEGS WENT FROM UNDER HIM & HIS RIGHT LEG GOT PULLED INTO THE RETURN STAND BETWEEN IDLER & BELT. HE PULLED TRIP & STOPPED BELT
SLIPPERY ROADS.
