Employee was using Lull forklift to cover bag load from rain damage. Boom should not have been used to perform task. Boom was all the way out while traveling, forklift started leaning. Trying to balance the forklift, lever got stuck & tipped on side. Employee will be retrained on using the proper equipment to perform the task.

Operator moving 2 pallets of material stacked on top of each other, the material weight exceeded the forklifts moving capability. Operator raised the load too high from the ground and when the forklift was moved forward the forks tilted forward lifting the forklift and once the pallets fell off the fork the lift landed back on the ground.
Operator upset equipment while dumping load. Operator failed to ensure truck was level before attempting to dump. Truck upset on passenger side. Operator was utilizing their seat belt at the time of the incident. Resulting injury were lumbar, lumbar radius strain. Miner refused medical treatment.
Operator stated that while operating loader loading outside contract trucks EE pulled or strained back.
Employee was operating forklift. Back started hurting (twitching and minor spasms).
A mechanic was standing behind his pickup truck outside the office when another employee pulled up behind him in another vehicle. When the other employee shifted his vehicle into park, the vehicle rolled forward pinning the mechanic's leg between the two bumpers.
EE WAS ON LIFT CAGE OF LULL FORKLIFT, REMOVING TRIM FROM BLDG. DAMAGED BY HURRICANES. FORKLIFT BACKED UP TO LOWER LIFT CAGE & WENT FROM CONCRETE TO SAND. WHEN THIS HAPPENED, THE LIFT CAGE SHIFTED & EE HAD HIS HAND AT THE BACK OF THE CAGE, OVER THE HANDRAIL. WHEN LIFT CAGE SHIFTED IT PINCHED THE END OF EE'S FINGER BETWEEN CAGE HANDRAIL & FORKLIFT PALLET GUARD.
EMPLOYEE WAS STACKING BAGS AND CO-WORKER WAS MOVING PALLETS ON FORKILFT AND EMPLOYEE WAS PINNED BETWEEN STACKING TABLE AND FORKLIFT.
