Employee was operating end loader on quarry pit, hydraulic hose ruptured and bucket dropped to pit floor. Operator states was "jarred'. Operator requested hospital check up and went to Bluefield Regional Medical Center. On workers compensation at this time.

Employee was operating a CAT 236B skid steer loader on irregular surface area, the machine went forward and back shifting his neck in same direction of machine. This happened at 1:00PM employee requested to go to doctor at 2:10PM.

EE parked 769-C truck at fuel pump at end of shift. When preparing to exit cab, he pulled something in lower stomach area. After getting home and taking a shower he called supervisor to inform about going to ER. After leaving ER, he called supervisor to inform he had a hernia and an appointment with Dr. on 1/26/07.
CAUGHT LEFT HAND 4TH FINGER BETWEEN LOADER DOOR AND DOOR JAM.
EMPLOYEE WAS USING A HAMMER TO BEAT MUD OFF ROLLERS. THE HAMMER BECAME LODGED BETWEEN THE MOVING BELT AND ROLLER. BEFORE OUR EMPLOYEE COULD LET GO OF THE HAMMER HIS RIGHT THUMB AND INDEX FINGE R BECAME CAUGHT BY THE BELT.
