Employee was operating end loader on quarry pit, hydraulic hose ruptured and bucket dropped to pit floor. Operator states was "jarred'. Operator requested hospital check up and went to Bluefield Regional Medical Center. On workers compensation at this time.

Using a Walk-Behind-Skid Steer loader to clean spillage from surge tunnel, unit slid sideways, causing hand and elbow to strike the side of the tunnel structure.

Employee was operating a CAT 236B skid steer loader on irregular surface area, the machine went forward and back shifting his neck in same direction of machine. This happened at 1:00PM employee requested to go to doctor at 2:10PM.

Employee stated he hurt his back while operating a CAT 226 during clean up of plant area. An inspection on this CAT 225 the following day by MSHA inspector found nothing relative to create a back injury. Employee must now be off work through the end of Sept is reason for filing injury report!

Pre shift inspection of highway delivery truck. Driver was raising the tuck hood and the hood hinge broke on one side causing the driver to support the extra weight of the hood. Right shoulder area strained.

OPRATING 980 CAT END LOADER IN QUARRY PIT-LARGE BOULDER SLID DOWN MUCK PILE AND HIT SUSPENDED LOADER BUCKET RAISING REAR OF LOADER OFF GROUND AND SLAMMIGN VIOLENTLY TO GROUND JARRING OEPRATOR. OEPRATOR EXPEERIENCING STIFFNESS AND SORENESS, ESPECIALLY IN RIGHT SIDE AND LOWER BACK.

"SOFTBALL SIZE" ROCK FELL FROM OVERHEAD OFF NO. 2 SCREENING UNIT AT CRUSHING PLANT AND STRUCK SUBJECT ON LOWER BACK.

THE INJURED WAS ASSISTING ANOTHER EMPLOYEE IN ERECTING STRUCTURE FOR STURTEVANT SAND SEPARATOR.THE OTHER EMPLOYEE,OPERATING BANTAM TRUCK CRANE HAD SUSPENDED ON EPIECE OF 8"X21"X28 FT H-BEAM AN D THE EINJURED WAS HELPING PLACE H-BEAM.THE OTHER EMPLOYEE RECEIVED WRONG SIGNAL FROM GROUND AND LOWERED BEAM TRAPPING INJUREDS LEFT AHND BETWEEN TWO H-BEAMS.

WAS DROPPING RAILROAD CARS WHEN CAR STOPPED SUDDENLY HE STRUCK HIS RIBS AGAINST BRAKE WHEEL

EMPLOYEE WAS OPERATING EUCLID HAUL UNIT WHEN IT SLID OVER EMBANKMENT CONDITIONS SLICK ROAD AND SHOULDER OF ROADWAY GAVE WAY NO DAMAGE TO THE HAUL UNIT
