A motor starter box, strapped and carried in loader bucket, tipped and fell on an employee. He suffered an ankle fracture requiring surgical repair with screws and plates and 4 months recovery. Contributing factors: 1) failing to ensure employee was clear from potential tipping of the box 2) non-routine nature of this task 3) inadequate securing of the load against tipping.

EMPLOYEE WAS DRIVING A PIT TRUCK AROUND CORNER AND COLLIDED WITH ANOTHER PIT TRAVEL TRAVELING IN OPPOSITE DIRECTION. EMPLOYEE SUSTAINED SLIGHT WHIPLASH TYPE INJURY TO NECK AREA.
EE WAS BACKING STOCKPILE TRUCK UP TO DUMP-BECAME HIGH-CENTERED ON SMALL PILE. LEFT VEHICLE TO GET TOW CHANIN-RETURNED, HOOKED ON TO END LOADER WITH CHAIN AND WAS PULLED OFF PILE AND UP INCLINE BY ENDLOADER. CHAIN BECAME SLACKENED AND SNAPPED ALLOWING VEHICLE TO ROLL BACK DOWN INCLINE-DRIVER APPLIED BRAKE & CLUTCH-RAN BACK ONTO PILE & OVERTURNED. BRUISES TO ABDOMEN, PELVIS & FLANK.
