Employee was driving a CAT 775 rigid frame truck from stock pile. While being loaded the truck was hit with the bucket of 988 CAT loader. Employee said they felt something back pop. There was no immediate pain but over a short period of time they felt pain in their back.

While backing up the truck to dump, employee stated that the truck downshifted, thus hurting employee's back. Employee was diagnosed with a pulled muscle and missed two days of work.
As Contractor backed his tool truck, he struck the bucket of the Excavator. Injured EE was located next to the bucket and was pinched between the service truck and the 345B bucket. EE was transported by EMS to the Hospital. The injury resulted in a completely serparated Pelvis.
MECHANIC WAS ASSISTING THE BOBCAT OPERATOR IN REMOVING THE BUCKET. AFTER THE PINS WERE RELEASED AND THE OPERATOR STARTED TO BACK AWAY, HE ACCIDENTLY MOVED THE CONTROL THE WRONG WAY CAUSING THE EDGE OF THE BUCKET TO LIFT. THE MECHANIC WASSTILL TOO CLOSE AND THE BUCKET EDGE STRUCK HIS LEFT LEG.
EE WAS CUTTING SECTIONS OF CONVEYOR BELT W/UTILITY KNIFE. HE HAD TO BEND OVER A LOT AND PHYSICALLY HANDLE THE SECTIONS. THE SECTIONS WERE 10' LONG AND 16" WIDE. THE REPETITION OF THIS MAY HAVE CAUSED THE STRAIN, ALTHOUGH EE IS NOT CERTAIN OF IT.
