Laborer kneeling & stemming blastholes set hand shovel down behind self, truck moved forward with spotter direction, approx 5mph. Passenger front tire caught shovel-end; shovel handle kicked up and knocked laborer to the ground, legs extended in tire path. Tire ran over legs. No fractures or lacerations were sustained. Employee was released from the hospital same day.

Was snowing #6 head roll spinning out. Climbed onto unprotected elevated spot to heat head roll using a weed burner. The tip of the weed burner came into contact with head roll pulling employee into head roll catching sleeve between head roll and conveyor belt causing right shoulder to come in contact with receiving box shroud.
Haul truck operator backed of off stockpile approx. 25ft. landed upside down. operator was not wearing seatbelt. Berm was not in place where accident occurred. Miner has head and chest trauma.
Operating heavy equipment, very fast, busy day. Ride control was not working, repetitive movement caused pain in lower back.
Loader operator did not see the pedestrian in his path. Pedestrian on phone was not aware that loader was approaching from behind. Loader ran over pedestrian.
EE was cleaning around the jaw 5th wheel hitch, based on piles at location, when for whatever reason went to the grizzley conveyor where he became wedged between a return roller & bottom side of belt. He has several broken ribs, broken collar bones, & intestinal, kidney, spleen & pancreas damage. As of this report he is in critical but stable condition.
WORKER WAS PUTTING THE LEFT FRONT FENDER ON A 980-G CAT LOADER. HE PROCEEDED TO CLIMB DOWN FORM THE FENDER ON THE INSIDE OF THE TIRE AND CAUGHT HIS RIGHT FOOT BETWEEN THE TIRE & THE LOADER FRAME. HE LOST HIS FOOTING AND TURNED SIDEWAYS KNOCKING HIS HARD HAT OFF AND FELL HEAD FIRST INTO THE CORNER OF THE BACK OF THE LOADER BUCKET PUTTING A CUT ON TOP OF HIS HEAD.
EE WAS INSTALLING FAN VENT CAP FOR A/C IN CAB. HE DID NOT TURN THE FAN OFF AND PUT HIS FINGER UP IN THE FAN AND HAD A LACERATION TO HIS FINGERS.
EE WAS FEEDING CRUSHER WITH 988F AND WAS MOVING TO THE QUARRY FACE AND RAN THE BUCKET INTO THE GROUND STOPPING THE LOADER ABRUPTLY, CAUSING EE TO FALL OUT THE FRONT WINDOW AND LANDING ON THE B UCKET.
DRIVING ON PUBLIC ROADWAY THRU CONSTRUCTION WITHFLAGMEN WAS REAR ENDED TWICE BY SAME VEHICLE JAMMED THUMB INTO STEERING WHEEL
