While trying to retrieve the loading spoon from inside the boom assembly on GME 02 emulsion truck, his partner who was in the basket at the time accidently touched one of the control levers causing the boom to retract breaking employee's left arm. At the time the engine on the unit was running.
EE was operating a 740 Cat Haul Truck on frozen surface of tailings disposal site. He hit a bump and his seat bottomed out jaring his spine. He felt pain in his back and was taked to the Dr in Fairbanks. The Dr feels that this is a strain of the back associated with possible bulged disk. The Dr has prescribed two weeks of Physical Therapy.
Employee was holding a metal concrete hose from a pump truck. The hose surged and struck the employee on the left shoulder.
Employee was working in the forklift manbasket and when the operator lifted the basket the employee's left ring finger was pinched between the basket and the forklift frame. Cut tip of his finger off, requiring three sutures.
The vehicle was moving 10-15 mph over a frozen road, when the vibration caused an outrigger pad to fall between the cab and the moving track, which resulted in the operator's cab being jarred. Operator's head received impact, no bump on head, but cut which required stitches; approximately $1000-1400 damage to rig.
