The truck operator was driving from the material extraction point to the crushing plant when apparently lost the vehicle control and impacted the berm and as consequence overturned.
The miner did not notice that a pick-up truck was driving in reverse and was hit by the side of the truck. The driver of the truck stopped immediately once he notice the miner was in the ground. The truck was subject to inspection by MSHA personnel September 4,2014 and there was no deficiencies.
The employee was operating bobcat which was unloading the bucket and the machine overturned.
Driving the truck and the seat system failed causing injury to the employees back.
THE LEFT REAR TIRE WENT FLAT. IT WAS REPAIRED AND INSTALLED. AFTERWARD THIS TIRE NUTS BECAME LOOSE AND THE TRACTOR LOST THE WHEEL MAKING THE TRACTOR TO FALL.
WHILE EE WAS WORKING ON A CONCRETE FOUNDATION BENEATH THE SIDE OF A CONVEYOR, THERE WAS A POWER FAILURE. WHEN THE POWER CAME BACK, THE CONVEYOR STARTED TO OPERATE, MAKING SOME ROCKS FALL DOWN, HITTING EE ON HIS BACK.
WHILE THE MINER WAS DRIVING A CAT 769C (DUMP TRUCK) LOADED WITH STONES UP A HILL THE TRUCK STARTED TO LOSE POWER AND HE STOPPED, THEN THE THE DRIVER TRIED AGAIN TO CONTINUE, BUT THE ENGINE OF THE TRUCK STALLED. HE TRIED TO BACK IT UP AGAINST THE EMBANKMENT AND WHILE DOING SO, HE TURNED OVER TO THE SIDE.
EE WAS INSTALLING A LOUDSPEAKER TO HIS VEHICLE WHEN HE WAS DISMOUNTING FROM THE CAR, HE WAS HIT BY ANOTHER CAR BY HIS CO-WORKER. EE FELL TO THE GROUND ROLLED FURTHER DOWN THE ROAD AND THE CARS REAR TIRE RAN OVER HIS RIGHT LEG.
THE EE SHUTDOWN A CONVEYOR BELT AND DIRECTED OTHE EE TO START THE CONVEYOR AS SOON AS HE FINISHED TO CLEAR THE MAGNETO. HE WAS STILL ON THE CONVEYOR WHEN THE OTHER EE TURN ON THE CONVEYOR WITH OUT SIGNAL. THE EE IN CONCERN TO GET TRAP ON THECONVEYOR HITTING THE MAGNET WITH HIS BACK.
EE WAS REMOVING A STONE FROM THE FEEDER WHEN THE DRIVER RAISE THE DUMP WITHOUT NOTICING THE EE. THE EE JUMP AND HURT HIS BACK.
EE WAS OPERATING A LOADER WHEN HE FELT A PAIN ON HIS WAIST DUE TO A MUSCULAR SPASM.
EMPLOYEE WAS GOING TO REMOVE A TREE BRANCH FROM A STOCK PILE, WHEN HE ACCIDENTALLY PUT HIS HAND ON THE CONVEYOR DRIVE MOTOR GRASPING HIS GLOVE AND PUNCTURE HIS RIGHT FINGERNAIL.
OUTSIDE TRUCKER LOADER "POLIVILLO" ONLY FROM THE BIN, BECAUSE THE CRUSHED ROCK'S BIN IS TO DIFFICULT. BEING LUNCH TIME, HE HAD A TICKET ISSUED FOR WAITING A LOADERHE WENT INTO THE BIN, BECAU SE OF THE HEIGHT OF THE TRUCK HE RUBBED AGAINST THE CRUSHED STONE.
EMPLOYEE WAS ADJUSTING THE SYMONS CRUSHER BY PULLING IT WITH A CHAIN. THE CRUSHER BOWL GOT JAMMED AND THEY PULLED IT WITH A LARGER LOADER BREAKING THE CHAIN. CONSEQUENTLY, THE CHAIN FLEW AND H IT HIS RIGHT LEG.
THE TRUCK DRIVER WAS FULLY FAMILIAR WITH THE UNIT IN SUCH A WAY THAT HE WAS OVER CONFIDENT & SHIFTED THE TRUCK UPHILL CAUSING IT TO MISS THE GEAR & SPEED DOWN THE HILL
WHILE DRIVING A DUMP TRUCK, USING THE SEATBELT, THE ROAD WAS WET THE TRUCK SLIP AND FLIP OVER. THE DRIVER WAS USING THE SEATBELT BUT HE GOT HIT ON THE RIGHT LEG. THE DRIVER DIDN'T HAVE ANY FRA CTURE BUT HE WAS REPORT TO THE FS.D.E.
INJURED WORKER WENT TO GET ASLACH AHMMER TO BREAK A ROCK IN THE FEEDER, WHILE THE FEEDER WAS RUNNING. THE FLOOR WAS WET, BECAUSE IT WAS RAINING, AND HE SLIPPED AND FELL IN THE FEEDER AND THE ROCK TO BE REMOVED FELL OVER HIM. THE FEEDER DRAGE HIM AND HE GOTS HIS RIGHT ARM AND SHOULDER BROKEN.
