68935 MOVING BOLOGNA CABLE EMPLOYEE PLACED HIS LEFT HAND ON BACK OF TRACTOR FORK FRAME WHEN TRACTOR OPERATOR ROLLED THE TRACTOR FORKS BACK THE EMPLOYEES FINGER WAS CAUGHT BETWEEN THE FRAME & A 08849AULIC RAM RESULTING IN A LACERATION NEEDINGSTITCHES
THE EMPLOYEE DECIDED TO CLEAN OFF A BELT SCRAPER AT THE WASH PLANT. HE REMOVED THE GUARD, DID NOT DE-ENERGIZE THE BELT AND REACHED IN TO REMOVE THE DEBRIS. THE BELT CAUGHT HIS COVERALL SLEEVE AND PULLED HIS ARM BETWEEN THE BELT AND PULLEY. THE ACCIDENT RESULTED IN ABRASIONS AND CONTUSIONS TO RIGHT ARM AND SHOULDER.

THIS CAUSED THE TRUCK TO BE PUSHED A SHORT DISTANCE AND A DERAILMENT OF SWITCH ENGINES. TRAIN CREW PULLED EMPLOYEE FROM WREACKAGE.

EMPLOYEE WS INJURED WHEN THE CUSHMAN HE WAS OPERATING STRUCK AN I-BEAM COLUMN. THE CUSHMAN STRUCK THE COLUMN AS HE REACHED DOWN TO PICK UP HIS JACKET FROM THE FLOOR OF THE CUSHMAN. THE CUSHMAN STRUCK THE COLUMN AT APPROXIMATELY 5 MPH AND THE STEERING WHEEL STRUCK I.NAME IN THE STOMACH CAUSING INTERNAL INJURIES.

EMPLOYEE WAS PAINTING AN OVERHEAD PIPE. THE EMPLOYEE STEPPED FROM A STEEL BEAM INTO THE BUCKET OF THE FRONT END LOADE TO BE LOWERED TO THE GROUND.L WHEN THE OPERATOR SHIFTED THE LOADER INTO RE VERSE AND ATTEMPTED TO MOVE THE LOADER ROLLED FORWARD AND PINNED THE EMPLOYEE BETWEEN THE TOP EDGE OF THE BUCKET AND THE FLANGES OF THE BEAM.
HE WAS OPERATING CRUSHER FEEDER CAR A ROCK APPROX. 80 LBS. GOT CAUGHT BETWEEN TH E ROOF AND THE CONVEYOR AND SHOT OUT ABOUT 10 FEET HITTING THE HANDLES OF 2 SHOVELS GLANCING OFF AND BOUNCING ACROSS THE DECK OF THE FEEDER CAR THEN STRIKING HIM ON THE BACK
ENTERING DOOR INTO UNIT4 ON FORK LIFT. REACHED OUT FROM FORK LIFT TO OPEN DOOR WHILE DOING SO HIS FOOT SLIPPED OFF CLUTCH PETAL & JAMMED HIS HAND INTO WALL. EMP RECEIVED BRUISED KNUCKLES & S MALL ABRASION TO PALM OF HAND.
SEE ATTACHED REPORT-THE MAGNITUDE OF THIS EMPLOYEE'S SAFETY VIOLATIONS INFLUENCED EMPLOYEE TO EXAGGERATE THE INJURY IN HOPE OF AVOIDING TERMINATION.IN LIGHT OF THIS,WE ARE ASKING MSHA TO RULE ON THIS WITH RESPECT TO CLASSIFICATION.
