Mechanic sustained injury to fingers while up righting CAT #4306 electric pallet lift when hand contacted a pinch point. Employee was evaluated by site responder and nurse. Employee was brought to healthcare facility for further evaluation and treated for finger tip fracture. Safety department initiated investigation into the incident with support from the department.

Scraper Operator was operating scraper and didn't report an exhaust leak. Exhaust leak was fixed as soon as mechanic was aware of the problem and the machine was back in service before the end of the day. Employee went to the hospital at the end of shift.

Employee operating #4267 forklift on Tilden 2 fan floor level with tines lowered when left tine caught the top of a floor launder channel causing the forklift to stop suddenly. Employee was seen by on-site nurse practitioner. Employee returned to work. EE developed neck pain after the incident and was eventually prescribed PT. EE complete 2nd session of PT on 05-04-2017.

Employee was operating a loader when operator stated the loader bucket hit something hard, jarring the operator. Employee was seen by on-site nurse and told nurse that the pain ee feels is related to older back problems from 2009 and 2011. Taken off work for 3 days then returned with restrictions.

The operator was being loaded when a chunk came of the bucket striking the fan tail. The operator allegedly was jostled in the cab. The operator reported it to dispatch and stated being fine. Operator reported soreness at end of shift and was seen by site responder. Instructed to follow up with nursing staff. Employee sought own medical care and was taken off work.

Operator struck chunk in roadway of Tower Hill Ramp with 1085 van during shift change. UT51 was called for an EMT to respond and an ambulance for driver. Driver was taken to Marquette General for further treatment and evaluation. EE was admitted to the hospital with a back injury.

Employee stated that he was jarred while sitting in the haul truck seat with seatbelt on after the tail end of truck bed was contacted by shovel bucket. Employee has reported multiple similar events that may have contributed to the current medical diagnosis of sciatic pain.

EE was using a vacuum truck to clean out the bentonite pit area. He felt an irritation to his right leg which was believed to be caustic residue. He was evaluated by a Physician during his recovery from the chemical burn. A week after the incident he was prescribed antibiotics.

Employee alleges right shoulder pain after turning a production truck steering wheel. He was evaluated by a physician on 5/19/13 and prescribed a sling and work restrictions. Employee is unable to work at this time due to restrictions.

Employee alleges neck pain after being jostled in his loader from hitting a pot hole. He was evaluated by a physician on 4/21 and given prescriptions for a neck collar, pain meds, and released back to work with restrictions. Employee missed his 4/22 shift due to taking the prescription pain meds. He was scheduled off on 4/23 & 4/24 and returned with restrictions on 4/25.

Employee reported neck pain after hitting pot holes in a production truck. He was evaluated by a physician on 4/19 and released back to work with restrictions. He was also given a prescription for physical therapy.

DRIVER WAS HOOKING PUP TRAILER WHEN LOADER BACKED INTO EQUIPMENT CAUSING EQUIPMENT TO STRIKE DRIVER THROWING DRIVER APPROX. 20 FEET. DRIVER SUFFERED BRUISING AND SLIGHT CONCUSSION. PUP TRAILER SUSTAINED HEAVY DAMAGE TO FRAME.

Employee was operating a loader and stated that he was jostled while mining the bank. He stated he was experiencing back and neck pain. He was evaluated by a Physician and taken off of work. He was released back to work on 9-22-2011.

The operator was operating his loader in a confined area-the Tilden pit. He then alleged that his back started to feel sore. He was evaluated on March 3rd-continued to operated equip. On 3-29-11 he was re-evaluated with complaints of hip discomfort and given restrictions not allowing him to operate the loader.At this time it is unknown in complaints are related to 3-3-11 event.

Loader operator jarred neck while attempting to break frozen concentrate. She was evaluated by a Physician and given restrictions on 2-28-11.

The employee was operating a loader moving chunks for the chunk breaker operator. As the employee was backing up, debris flew through the open window of the loader striking the employee in the left eye. He was evaluated, surgery was performed and taken off of work on 9-29-10.

An employee was jostled when his loader contacted a toe at the shovel site. He was evaluated by a Physician & taken off from work. It was made known on December 18th that it was a lost time injury.

Employee climbed onto a stopped conveyor belt without it being locked out. The belt started and he was caught by a material hopper, resulting in mulitiple injuries.

Employee was a passenger in a golf cart that rolled over. The cart landed on top on his legs. He was evaluated by a physician and was taken off of work on 8/21/08.

While driving T16 truck (240 ton) the operator drove on to the left hand berm causing the truck to tip onto it's right hand side. Employee was taken off of work per physician.

Employee was cleaning his safety glasses and adjusting his heater and fan controls on the loader that he was operating. Employee complained of dust going in his eye when the fan started to blow. He was evaluated by a physician and eye medication was prescribed.

Employee was operating SL35 (loader) and hit a toe in the driveway. When she struck the toe, she was jostled and complained of upper back pain. She was evalutated by a physician and physical therapy was started on May 23, 2006.

Employee was acting as the conductor of a rail crew assigned to drop off and spot 120 empty rail cars at the Tilden Mine Facility. During the 50 car move, the employee was fatally injured when he apparently fell from the lead car and was run over. He became caught between 2 wheels and dragged approximately 560 ft. The employee had on appropriate PPE for the job.

EE WAS OPERATING A LOADER - HE HIT SOMETHING SOLID IN THE TUNNEL AND THE LOADER STOPPED. HIS HAT FLEW OFF AND HIT THE WINDSHIELD BREAKING THE GLASS. HIS HEAD IMPACTED THE CAB - A LACERATION RESULTED REQUIRING 4 SUTURES.

EE WAS OPERATING 1553 PRODUCTION TRUCK. HE HIT A ROUGH AREA OF ROAD AND BECAME JOSTLED AROUND CAUSING DISCOMFORT TO HIS NECK. HE WAS REFERRED TO A CHIROPRACTOR ON 3/15/02.

EE REPORTED BEING JOSTLED AROUND IN THE SEAT/CAB OF A PRODUCTION TRUCK WHILE BEING LOADED BY A LOADER. HE COMPLAINED OF LOW BACK PAIN AND WAS EVALUATED BY A PHYSICIAN ON 1-21-01 AND TAKEN OFF OF WORK.

EE WAS RIDING DOWN THE MAIN AISLE IN A FORKLIFT AND FELT IRRITATION IN HIS EYE. HE HAD HIS EYE FLUSHED BUT STILL FELT LIKE THERE WAS IRRITATION IN IT. WAS EVALUATED BY A PHYSICIAN ON 12-5-01 A ND MEDICATION FOR HIS EYE WAS PRESCRIBED AND USED.

EMPLOYEE WAS OPERATING T37 HOT CHANGE VEHICLE AT S29. THE SHOVEL OPERATOR BLEW THE HORN CAUSING EE TO SPIKE THE BRAKES. WHEN SHE DID THIS, SHE HIT HER ELBOW ON THE BOTTOM OF THE STEERING WHEEL . SHE WAS EVALUATED BY A PHYSICIAN AND PHYSICAL THERAPY STARTED ON 10-16-01.

EE REPORTED A SORE BACK RELATED TO NOT BEING ABLE TO ADJUST THE TILT ON THE SEAT OF THE PRODUCTION TRUCK THAT SHE WAS ASSIGNED TO. SHE WAS EVALUATED BY A PHYSICIAN AND REFERRED TO A CHIROPRACT OR ON 7-27-01. IT WAS DETERMINED TO BE OCCUPATIONALLY RELATED ON 8-22-01.

EE REPORTED SORENESS IN HIS UPPER BACK/ LOWER NECK AREA RELATED TO THE ROUGHNESS OF THE HAUL ROADS IN THE AREA OF 1430 SHOVEL ON 1575 ELEVATION.HE WAS EVALUATED BY A PHYSICIAN AND PHYSICAL THE RAPY WAS STARTED ON 8-8-01.

EE WAS DRIVING 1552 TRUCK AND AT THE END OF HIS SHIFT HE COMPLAINED OF A SORE BACK FROM THE TRUCK SEAT. HE WAS EVALUATED BY A PHYSICIAN AND PHYSICAL THERAPY WAS STARTED 12/28/00.

EE WAS OPERATING 1547 PRODUCTION TRUCK. ON THE SECOND PASS OF THE LOADING CYCLE THE MATERIAL JOSELED EE AROUND AND HE FELT PAIN IN HIS LOWER BACK. HE WAS EVALUATED BY A DR. IN THE ER ON 7-30-0 0. IT WAS MADE KNOWN ON 9-28-00 EE HAD PHYSICAL THERAPY ORDERED ON 8-10-00.

EMPLOYEE WAS DRIVING THE HOT CHANGE VEHICLE TO SHOVEL 27 WHEN HE HIT A BUMP IN THE ROAD AND HIT HIS ELBOW ON THE ARM REST. HE WAS EVALUATED BY A PHYSICIAN ON 8-10-00 AND FLUID WAS ASPIRATED FROM THE ELBOW.

EMPLOYEE WAS DRIVING A HOT SHIFT CHANGE VEHICLE AND HIT A BEAM, CAUSING HIM TO IMPACT THE STEERING WHEEL. HE WAS EVALUATED BY A PHYSICIAN AND TAKEN OFF WORK.

EE WAS OPERATING LOADER IN ESCAPE TUNNEL IN VICINITY OF LINE 6 OR LINE 7. HE WAS BACKING OUT OF THE TUNNEL AND THE LOADER CANOPY CONTACTED A SHEET METAL DUCT HANGING FROM THE R5OOF OF THE TUNN EL. THIS CAUSED THE LOADER TO SUDDENLY SLOW DOWN & STOP AND IN THE PROCESS, EE'S LOWER BACK WAS JOLTED. ELECTED NOT TO SEEK MED. TX UNTIL 1-25-99. DX.LS STRAIN.

STRIPPER HOPPER WAS PLUGGED. THE OVERFLOW PIPE TO FAN FLOOR WAS STARTING TO BACK UP INTO PIPE. EE GOT ONTO MUSTANG LOADER ON FAN FLOOR & BEGAN TO REMOVE THE HOT MATERIAL FROM THE OUTLET OF THE OVERFLOW PIPE. HE TRAMMED 6 BUCKETS OF HOT MATEERIAL TO BASE OF KILN PIER #2. WHEN HE DUMPED 7TH LOAD ON TOP OF 5'HIGH PILE, HOT DUST & POSSIBLY STEAM BLEW INTO CAB OF LOADER. LOSS TIME 1/4.

EE HAS A SORE SHOULDER. ATTRIBUTES IT TO OPERATING A LOADER. DETERMINED ON 1/27/99 TO BE WORK RELATED. PT WAS STARTED ON 12/28/98. DX= SHOULDER STRAIN. (PHYSICAL THERAPY)

EE WAS LEAVING 1431 SHOVEL WITH A LOAD. HE TURNED HIS HEAD WAY OVER TO THE LEFT TO LOOK FOR ONCOMING TRAFFIC. DURING THE MOTION OF TURNING TO THE LEFT AND THEN BACK FORWARD HE FELT A MILD PAIN IN HIS NECK AND SHOULDER. IT WAS DETERMINED ON 12-15-98 TO BE WORK RELATED.

EE WAS DRIVING 4163 PU. AN EQUIPMENT SALVAGE CREW HAD BEEN DISMANTELING A 85 TON TEREX TRUCK. THESE PARTS INCLUDING SECTIONS OF THE DIFFERENTIALWERE POSITIONED ALONG THE OUTSIDE ROAD EDGE. AS EE TRAVELED TO INSPECT THE MPHM5 STOCKPILE, HE STRUCK A SECT OF THE DIFFERENTIAL. THIS CAUSED THE PU TO STOP IMMEDIATELY AS THE PART WEIGHS 2000# HE WAS TRAVELING AT THE PIT SPEED LIMIT OF

WHILE OPERATING A FORKLIFT IN THE BAGGING AREA, AS HE BEGAN TO BACK UP, HE TURNED HIS HEAD TO LOOK TO THE REAR. AT THAT TIME HE FELT A SHARP PAIN IN HIS NECK. SAW A DR ON 7-15-98. DIAGNOSED: N ECK STRAIN.

EE WAS OPERATING YALE FORKLIFT WHILE UNLOADING 1500 LB. MOTOR FROM FLATBED TRUCK. HE SUSPENDED THE MOTOR ON A SLING ATTACHED TO THE RIGHT FORK. AS HE BACKED AWAY FROM THE TRUCK THE RIGHT FRONT WHEEL OF THE FORKLIFT DROPPED INTO A SHALLOW DEPRESSION ON THE ROADBED CAUSING THE MOTOR TO SWING BEYOND THE FORKLIFT'S CENTER OF GRAVITY, RESULTING IN FORKLIFT TIPPING AND COMING TO REST ON

EE & PARTNER WERE ATTEMPTING TO CONNECT 2ND HALF [FEMALE END] OF FEED CHAIN TO 1ST HALF [MALE END] WHICH WAS ALREADY INSTALLED. FEMALE END IS "Y" SHAPED & ON INSIDE OF BOTH LEGS THERES IS A CO UNTERBORE FOR A SERIES OF WASHERS. EE SET THE FEMALE END ON TOP OF THE MALE END BY OFF SETTING THEM, REMVED HIS GLOVE & ATTEMPTED TO CLEAN THE COUNTERBORE WITH HIS FINGER ... AMPUTATION OF FI

EE WAS IN ROUTE TO SOUTH 3 DUMP NEAR LINDBERG'S CRUSHER WHEN HE HIT A POTHOLE IN THE ROADWAY. HE FELT PAIN IN HIS LOWER BACK. PHYSICAL THERAPY WAS STARTED ON 5-12-98. (HE WAS SEEN BY A DR. ON 5-4-98, DIAGNOSIS: ACUTE LUMBAR STRAIN.

EMPLOYEE WAS OPERATING 1545 PRODUCTION TRUCK. HE SAID HIS SHOULDER WAS SORE BECAUSE OF THE DUMP LEVER BEING HARD TO PULL. HE SAID THE DUMP LEVER HAD TO BE PULLED PAST NORMAL DUMPING POINT. SHO ULDER STRAIN. HE WAS PLACED ON RESTRICTED DUTY.

EMPLOYEE WAS ON THE WEST SIDE OF 15 CONVEYOR ON THE SOUTH BEND PULLEY. HE WAS ATTEMPTING TO HAND A PIECE OF CHANNEL IRON UNDER THE CONVEYOR BELT TO TWO OTHER EMPLOYEES. THE 2 EMPLOYEES HAD A HOLD OF THE CHANNEL IRON BUT COULD NOT PULL IT THROUGH. IT WAS HUNG UP BETWEEN THE PULLEY AND SUPPORT BEAM. WHEN TRYING TO PUSH CHANNEL IRON BACK TO EMPLOYEE THE END FLIPPED UP PINCHING THUMB.

EE HIT A POT HOLE ON BOTTOM OF LONG RAMP WHILE OPERATING 1541 HAUL PAC AND FELT A JAR IN HIS UPPER BACK. HE WAS SEEN BY A PHYSICIAN ON 11-20-97, DIAGNOSIS THORACIC BACK STRAIN. HE WAS PLACED O N RESTRICTED DUTY.

EMPLOYEE WAS CLEANING ON THE WASHDOWN FLOOR, OPERATING A SMALL LOADER. HE ROAD OVER SOME HARD MATERIAL WHICH WAS ON THE FLOOR CAUSING THE LOADER TO BOUNCE AND IN DOING SO HE FELT SOME PAIN IN HIS LOWER BACK. HE WAS SEEN BY A PHYSICIAN ON 9-11-97 DIAGNOSIS LOW BACK STRAIN. PHYSICAL THERAPY WAS STARTED.

WHILE OPERATING LOADER, LOADING TRUCKS IN PIT, EMPLOYEE FELT STRAIN AND A PULLING SENSATION FROM ONE OF THE TENDONS IN HIS RIGHT WRIST. EMPLOYEE SAID IT IS FROM THE REPETITIVE ACTIONS USING T H EHOIST AND CURT CONTROL LEVERS. HE WAS SEEN BH A DR, PHYSICAL THERAPY WAS STARTED ON 7-16-97.

WHILE WALKING INTO THE PELLET PLANT T-2 ELEVATOR, EE WAS STRUCK BY THE DOOR AS IT CLOSE. DIAGNOSIS: CONTUSION LEFT SHOULDER. PHYSCAL THERAPY WAS STARTED (5-1-97).

EMPLOYEE WAS HAULING FROM 27 SHOVEL TO WEST 2 DUMP AND STRUCK A DIP WHERE DUMP HAD SETTLED ACROSS THE ROADWAY. HE FELT A PAIN IN HIS NECK AFTER STRIKING THE DEPRESSION. DIAGNOSIS: CERVICAL STR AIN. HE WAS PLACED ON RESTRICTED DUTY.
