Zemer maintenance crew was performing a lift of a roll of belt. During the lift miner's hand became entrapped between the lift chain and the roll of belt resulting in an injury to his right thumb.

EE claims he opened port downstream of gate on airslide to verify gate was closed. When EE opened port on airslide, material immediately began flushing from silo. EE inhaled bypass dust during incident.
Injured employee & co-worker used strap attached to forklift to lower Pfeiffer grinding segments into open box. While lowering segment, strap stuck on front of one fork. Injured employee claims that when he used his right hand to pull strap off fork, segment weight shifted in box applying a load on strap causing strap to pinch employee's finger between strap & fork.
CLEANING RETURN ROLLER ON CONVEYOR BELT, HE GOT DUST IN EYE.
EMPLOYEE DISREGARDED DIRECTIONS AND REACHED INTO OPERATING CONVEYOR TO CLEAR MATERIALS. HIS HAND CAUGHT BETWEEN BELT AND ROLLER.
WHILE MOVING RAILCARS, RAN INTO A RAILCAR THAT EE WAS ON. INJURED HAD JUMPED OUT OF THE WAY OF THE ONCOMING RAILCARS AND WAS HOLDING ONTO THE LADDER WHEN THE CARS COLLIDED. THE COLLISION CAUSED INJURY TO HIS SHOULDER, NECK AND BACK.
EE WAS SITTING IN TRACKMOBILE, ATTACHED TO A STRING OF 8 CARS. HE WAS MOVING THE CARS SOUTH WARD ON TRACK 3 AS THEY WERE FILLED. WHILE HE WAS WAITING TO MOVE THE CARS, THE LINE OF CARS WAS STRUCK BY CARS BEING DELIVERED.
EE WAS WALKING ALONG ROAD IN PROCESS AREA & WAS STRUCK BY A GOLF CART COMING AROUND A CORNER.
EE WAS A PASSENGER IN THE #4100 TRACKMOBILE THAT WAS BEING DRIVEN BY ANOTHER EE WHO WAS DRIVING IN REVERSE AND DID NOT SEE #4000 TRACKMOBILE THAT WAS STOPPED ON ADJACENT TRACK NEAR SWITCH. DRI VER TRIED TO STOP BUT COLLIDED CAUSING PASSENGER EE TO INJURE HIS CHIN.

EE FELL OUT OF GOLF CART THAT WAS BEING DRIVEN BY ANOTHER EE CAUSING LACERATION TO LEFT ELBOW CONTUSION TO RIGHT HIP AND LOWER LEFT SHIN.CONTRIBUTING CONDITIONS WERE UNEVEN ROAD SURFACE, LOOSE GRAVEL, SHARP TURN AND TRAVELING TOO FAST. MODIFIED DUTY.
EE WAS DRIVING A GOLF CART AND PULLED OUT FROM BEHIND CEMENT TRUCK TO TURN LEFT IN FRONT OF SHIPPING OFFICE WHEN METROPLEX ELEVATOR TRUCK HIT FRONT LEFT TIRE OF GOLF CART KNOCKING EE OUT OF CA RT AND ONTO PAVEMENT.
EMPLOYEE WAS RIDING IN BACK OF CHEVY TRUCK; AS TRUCK WAS BEING PUT INTO GEAR, TRUCK JERKED AND EMPLOYEE LOST BALANCE AND FELL TO GROUND, WHICH RESULTED IN SPRAINED WRIST.
EE RECEIVED SECOND AND THIRD DEGREE BURNS WHEN USING BOBCAT TO REMOVE MATERIAL FROM PYROCLONE DUMP AND THE HOT MATERIAL PUFFED UP LANDING ON EE'S LAP AND RIGHT ARM. HIGH WINDS BLOWING OUT OF S OUTH AT APPROX. 35 MPH.
EMPLOYEE WAS CLEANING UP AREA ON EAST SIDE OF RAW MILL WHEN ANOTHER EMPLOYEE OPERATING A BOBCAT BACKED INTO HIM CAUSING INJURY TO LEFT KNEE. THE INJURED EMPLOYEE WAS STANDING IN THE BOBCAT OPE RATOR'S BLIND SPOT AND WAS AWARE OF THE BACK-UP ALARM, BUT DID NOT BECOME AWARE OF THE DISTANCE BETWEEN HIMSELF AND THE BOBCAT UNTIL IT HIT HIM.
