Tension on V Belts was being adjusted when employee slipped, to prevent falling he grabbed V Belts. The V Belts rolled backwards pinching his finger between the sheave and V Belts.
Driving water wagon uphill on ramp from stockpile area to truck scales. Machine lost power, started rolling backwards. Brakes failed, rolled approx 100', jacknifed on level ground. The EE unbuckled his seat belt & jumped or was thrown from cab.
Loose material sloughed causing rear tires of trailer to drop off stockpile. Trailer turned over on its side causing the road tractor to turn over on its side. The air filter, ferring, mirror, hood were damaged on the road tractor.
Employee was driving a Peterbilt Semi Tractor with a belly dump trailer. Was stockpiling material. EE drove truck through app 6" of water and struck an object which caused the truck to jump, jarring the ee and injuring his back.
