On 1/26/2016 employee was operating a forklift and strained neck. Employee was seen by site medic & ER, full release was issued no days lost. On 3/7/16 employee was seen by doctor, who then issued restrictions, 3/11/16 is the 1st shift missed.
Employee states he felt lower back pain/discomfort while tramming a 4 yard when he ran over a piece of oversize in road. Went to medic.(this was on his last shift in rotation). On days off, he called & stated his back was still sore-went to doctor on 5/1/2015 & was taken off work.
Employee states he was driving water truck down ramp, when he started feeling dizzy - then his head contacted inside of cab, resulting in laceration above right eyebrow. 3 sutures required. No Days Lost. Full Release issued.
Operator was travelling backwards in haul truck down the ramp, when the haul truck's rear tire contacted the bank causing it to tip on its side. Employee was wearing seat belt and all proper PPE.
Employee stated that he was traveling in haul truck when the truck contacted the rib causing him to make contact w/door frame, resulting in laceration to lower lip & 2 chipped teeth
Employee was mucking out muckbay with skid steer when he hit some hard bottom with bucket causing him to be jerked forward, straining left shoulder. Doctor issued restrictions.
While operating a walk-behind skid steer, employee was walking backwards when his left arm contacted a rear support beam on TBM, causing a contusion, x-rays positive for fracture to left wrist.
Employee stated that while operating a haul truck, he ran over a rock & it bounced him in the seat. Later in shift, he stated he felt discomfort in his neck. We took him to ER-Full Release was issued. On 6/26/2014, he went to another doctor, who issued restrictions. 1st shift missed was 6/27/2014.
Employee was involved in train derailment & received laceration to chin, which required sutures.
The employee reported he was traveling towards the face and his LHD contacted the rib. The employee received a laceration on his eye brow from contacting the LHD frame and a second laceration on the left side of his head from contact with the hanging wall.
While driving underground, Employee stated he received a foreign body that got behind his safety glasses & into his R. eye. It was embedded & had to be removed. Full Release issued. No Days Lost. (This happened on his last shift before going on days off)
Employee was mucking on an incline when he backed down the ramp & the front tire on mucker rolled over rock and employee jarred his back. Doctor issued restrictions. X-rays were negative.
Employee was driving a kubota down the 72 722 ramp, when the kubota tipped on its side. This resulted in a fracture to employees left foot.
Employee was operating a UG Haul Truck on FWL, when he drove through a hole that had some water in it and hit a rock, causing his body to shift upward, and he struck his head on the inside of the cab on the haul truck. He reported neck pain. We took him to medic on site & down to ER. Doctor issued restrictions.
Rail materials slid forward on "Flat" that was being pushed by Loci & EE got his leg pinned. Went to medic on site. Off to ER, sutures were required. Full release issued. No days missed.
Employee was a passenger in a Kubota - while traveling down a ramp, the driver lost control of the Kubota, striking the left rib, and the Kubota turned over on its side. Employee sustained a right wrist fracture & bruised right inner thigh.

Employee was driving a haul truck underground, and got a laceration on his chin from hitting the inside of cab, which required 2 sutures. Road conditions and brakes were an issue. Employee did this on his last shift of his rotation, and returned to work on his next scheduled shift.
Employee was operating loaded muck railcar, when he got some dust into his left eye. He went to medic on site. He went to doctor after work, and RX was given to employee.
Employee was driving a skid steer.He looked behind him & drove over a rock which jarred his neck. He didn't report this. On 3/4/10, he came to work reporting discomfort in his neck & shoulder. He was sent to the medic on site. He worked his shift & then went on days off. He reported to work his next shift w/restrictions from doc.
Employee was driving down the ramp, when he reached out his window to turn on the block light, he did not come to a complete stop & continued to roll when the cord wrapped around his wrist & caused his shoulder to "pop". He went to medic on site, was sent to ER Room for x-rays, which were negative. He was given a release with restrictions.
While employee was operating a mucker, he came in contact with the rib, causing his head to hit the cab. He received a laceration to his head & lip, which required sutures. He was released to full duty. This happed on his Friday shift, then went on his 4 days off. Came back to work on his scheduled day. He missed no days.
Employee ran over a pot hole while driving a Getman, causing lower back pain. This was on 9/11/09. Did not report it until 9/12/09 which was his last day of rotation.He was off for 4 days, returned to work on his next shift. 9/17/09. while at work on 9/17/09, he complained of lower back pain,went to our medic on site & then sent down for evaluation by a doctor.
Employee was driving a normet truck, when twice during his shift he hit a hole filled w/water in the road causing him to bounce up & hit the roof of the cab with his head while wearing his hard hat, causing neck pain. He went to medic on site, and went back to work. He went to doctor on his days off, and on 8/21/09 he was given work restrictions & is currently working light duty.
At the start of shift, while riding on the mantrip rail into the portal, the front mantrip derailed. Employee stated that he was ok, but a little sore in the lower back area. He worked his scheduled shift on 6/14 & 6/15. On 6/16/09 he sought medical treatment from a chiroprator who took him off work.
While employee was tramming in a 4 yard mucker, he went around the corner, and hit a parked vehicle , causing his hard hat to come off and substain a laceration above his right eyebrow, requiring sutures.
Employee was mucking a slusher stope borehole when he was struck in the forehead by a rebar bolt. To the best of our knowledge, the LHD tire kicked the bolt up off the ground.
While riding in a Normat Truck a small rock came up hitting the employee in the chin causing a laceration that was sutured.

After positioning and stopping the Marcotte Utility Vehicle to allow another vehicle to pass, an employee placed her right foot outside of the operator compartment. Her foot was struck by the passing vehicle resulting in a fracture and dislocation of the lower leg.
Employee was operating a utility vehicle while wearing fall restraint. One end of the restraint came loose and fell outside of the operator compartment. The employee was pulled out of the vehicle dislocating his shoulder.
Employee was pushing a hose back into place as the cab of a skid steer was being lowered by other employees. The 2nd and 3rd digits on his left hand were caught as the cab closed, resulting in lacerations that were sutured.
Employee was struck by a LHD as it passed by him in a 12.5' wide drift while going around a curve. The contact broke a bone on the outside of the left hand, a bone at the tip of the little finger on the left hand and cracked two bones on the outside of the lower spine.
Driving skidsteer safety glasses fell off, bent to get them, jacket caught lever & loader pinched fingers.
Employee was operating an LHD when a tire went into a hole. He experienced back pain when he was jarred. On 4/9/07 the employee informed the company he had sought chiropractic adjustment for back pain.
Employee was exiting cab of mucker when the machine articulated. He was pinned in the door and received a pelvic fracture. MSHA was contacted immediately pending additional information about the extent of the injury.
Employee was positioning 6" drisco pipe with loader bucket. Pipe rolled off back of bucket and struck employee on side of head. Employee received concussion and was not released to work.

Employees were traveling to the change house via the surface access road in a Toyota mantrip vehicle. Upon reaching the lower-most intersection, the vehicle skidded through the intersection and struck a concrete barrier. Investigation revealed the braking systems were operable.

Employee struck jaw against mantrip during train derailment on surface, 11/5/06. Employee worked until Dr took him off work on 11/12/2006.
Employee was riding on a locomotive on 35 west that derailed. She experienced back pain from being jarred. On 10/5/06 the employee received chiropractic treatment, which makes this a reportable injury.
Returning to base on 4-wheeler, hit rock and was thrown from vehicle.
Employee stopped haul truck to activate a pull switch. With the door open and his right arm extended, he repositioned the haul truck to reach the switch. The door pinched his forearm against the cab when he stopped the truck. He received a hairline fracture to the ulna.
Employee was a passenger on a tractor that was pushed by a flow of water and sand. The employee struck against the underground rib. He worked the next shift, but experienced continued discomfort. He was examined again on 5/30/06 and an x-ray revealed three fractured ribs. The employee did not return to the remaining shifts in the rotation.
Employee was standing at the back of a utility vehicle when a skid steer slowly backed up and pinched him between the two vehicles. The employee's pelvis was bruised as a result and he was taken off work by the physician.
Employee was riding in tractor when drill rod hit the heel of his boot causing his foot to move forward and fracturing tip of right great toe
While employee was operating LHD remotely, the exhaust tube came off turbo. Employee attempted to remove LHD from workplace by manually operating it. Subsequently inhaled LHD exhaust. This is reportable for oxygen and breathing treatment. Awaiting pulmonology exam.
