He climbed up on the front of a shovel track to get into the cab and hit his head on a mirror bracket. He did not use the step mounted on the machine and came up under the bracket from the wrong direction. The ER doctor gave him 5 sutures and said he suffered a concussion.He was seen again by the doctor on 3/9/2010 and cleared to return to work on the 10th.

Victim reported that smoke from diesel fuel filled his cab when a fuel leak occurred. He went to the emergency room on 7/29/06. From 7/30/06 until 8/4/06, he did not report for work. On 8/6/06 the events were reported to me. I investigated and reported to the insurance company. They notified me 9/21/06 that they were paying the claim.

THE DRIVER DECIDED TO DUMP ANYWAY, HE BACKE D UP BUT GOT TOO CLOSE TO THE EDGE. THE TRUCK RAN OFF ON THE RIGHT SIDE & FLIPPED OVER LANDING 147' DOWN THE ROAD BANK IN THE STATE HWY. DRIVER WAS WEARING SEAT BELT.
