Mostrando los 4EE was asked to stockpile rock at stockpile area. He was backing up across pad and struck an F-250 in front left corner. The F-250 was parked with nobody in it at time of accident. At this time I was notified and went to the scene of the accident. EE was sent for a drug screen at 10am, the very next morning (no personal injuries occurred from this accident)
While operating a front end loader, loading trucks, employee turned his head to look behind him and strained back side of right shoulder and caused numbness to his ring and pinky finger on his right hand.
Technician operating Lull forklift (which he is trained on)he backed up & hit a berm in the sand. The lift tipped over sideways, he was wearing all PPG & rode the lift down. He hit his head & bruised his arm but no further injuries & no lost time. Minor accident no chance of death.
