Hit pothole in truck.

Employee was operating a water truck on Grapevine North Contour. Employee was going up an incline to the pit. The truck engine died and began to roll backward. Employee said it rolled about 75 ft. and they steered the truck toward the highwall. The rear tire rolled up on a berm and it flipped on its side.

EE was using a 15 ton motor to move a VFD box. The motor derailed and as ee was exiting the motor it slid approximately 12 inches and dropped on ee foot. EE sustained 3 fractures to right foot.

Employee was riding down #3 belt on a 4 wheeler. The ride dropped off the tracks and hit a hole causing lower back pain.

While travelling through the curtain, the scoop and shuttle car collided.

EE was attempting to release the emergency park brake and exited the vehicle to try to release the brake. The vehicle had been left in gear and when the brake released it knocked the EE down and drug EE approximately 10 to 15 feet. The EE managed to get back in the vehicle and bring it to a stop. EE sustained a dislocation to the right ring finger.

Employee was operating a 980 G end loader and backed over a rock. The loader slammed down and the seat suspension bottomed out twice, jolting the employee and straining employee's neck, shoulder, and lower back. Employee continued to work until 5/9/2016 at which time the doctor took the employee off work.

Employee was moving cable trees so Highwall miner could advance to hole #36. While he was moving the cable trees, one tree tipped over and struck the 980H Loader on the off side mirror. The mirror hit the side windshield glass, busting it, and causing the glass to hit the employee. He had cuts on the right forearm and glass in eyes. RTW the next scheduled work day 3/16/15.

Employee was operating a scoop, cleaning the #9 heading near the face. Employee reached out of the operator's compartment to move the ventilation curtain and his left hand was caught between the canopy on the scoop and the rib.

Employee was loading shot material into rock trucks. He stated he was prying on a rock and the teeth on the loader bucket slipped off the rock causing the endloader to shake him. During the shift he began to experience pain in his right hip. His first day off work was 4/28/14.

PUT TRUCK IN DITCH. FELL UPON EXITING VEHICLE.AMBULANCE CALLED. WENT TO PIKEVILLE MEDICAL CENTER AND RELEASED. EMPLOYEE CALLED AFTER HOSPITAL VISIT STATED THEY COULDN'T FIND ANYTHING WRONG

DRIVING, SAW A DOG TRIED TO DODGE IT, BUT WENT IN DITCH AND EMPLOYEE COMPLAINED OF SHOULDER PAIN

Employee raised his head while riding in mantrip, his head stuck the roof pulling his hard hat off causing his head to hit the metal bracket on the sander box requiring staples.

Employee was operating truck when he hit some potholes and began complaining with his back and numbness in his legs and feet.

Employee was in mantrip that was stuck by another mantrip causing severe jarring of this occupant resulting in back strain.

EE was operating a 830 Rock Truck up a steep incline, the truck went into neutral and started to roll backwards. The truck struck a blade on the dozer that was following behind for safety precautions. An eyewitness to the accident stated that the truck rolled backwards a maximum of 15' before striking the dozer blade. EE taken to Medical Center because of neck pain from seat belt.

Parked on a hill on a mantrip. Before he exited the mantrip, another mantrip hit him.

Employee was operating rock truck when he leaned over the arm rest of seat to pick up a pencil and hurt his ribs. Taken off work effective 1/18/11.

EE was operating loader when it slipped off a large rock, causing his right shoulder to strike the dash and windshield. He was taken off work effective 10/28/10.

EE was a passenger on a ride that collided with a mantrip on the tracks. EE sought medical attention at Pikeville Medical Center. He was given crutches and returned to work the following day on restricted duty.

Man was on the way underground riding on the man trip and they were hauling shovels. The shovels hit the top causing shovel to strike the ee in the shoulder.

While going down a hill employee lost control of truck causing him to collide with a mountain side. The reason for the loss of control of his vehicle is being investigated.

Employee was operating a shuttle car when he hit a hole resulting in a bruised hip.

Employee backed his loader into a rock truck who was positioned to be loaded resulting in a strain to his back.

Employee was positioning his rock truck to be loaded when the loader backed into the side of the truck causing him to hit his head on the side door.

Hanging curtain when a shuttle car was moved pulling the cable down on his face and causing him to fall on rock.

Employee was operating a loader when he struck a rock in the shot causinng his left knee to strike the safety guard of the steering control. Employee was not taken off work until 1/30/09 when he had surgery.

EE hit left shoulder on roofbolt, resulting in a laceration, required stitches.

He was operating personal carrier and hit rock, jarring steering wheel out of hand hitting his finger.

EE had completed a pre-shift of the belts & was pulling into the belt entry on a 3-wheeler to mark the date board. he hit a hump in the entry and was bounced into the mine roof. He was taken outside by the beltman and transported by ambulance to Medical Center. CT scans and x-rays were negative for fractures or deform. hospital

Operating a three wheeler, the steering wheel spun around hitting the top of his hand fracturing two fingers.

Truck was coming down bank and driver could not get it back into gear, so he jumped out of truck, causing multiple contusions and abrasions.

While operating a shuttle car EE's head came in contact with the mine roof.

Operating shuttle car when he drove into a hole causing his head to hit canopy injuring his head and neck.

Employee lost control of the rock truck he was operating and went off the road, ran over a berm where the truck dropped 2' to 3' down into pit area.

Operating a utility truck when another vehicle rear-ended his vehicle. Miner was taken off work effective 6/29/07.

Operating scoop, hit head on top then hit right elbow on scoop also injuring shoulder. Employee started missing work due to this injury on 7/16/07.

Miner was walking to silo feeder to clean up spillage, tripped and got arm caught in top roller. It pulled his arm in and slammed his chest into an angle brace injuring his left arm, left elbow, left chest and left thumb. Employee was not taken off work by his physician until 12/12/06.

Miner was operating coal loader when the bucket struck a rock, jolting him. Operator stated that he felt something in his stomach "pop".

Employee was guiding fan shroud that was being lifted with a boom truck when lifting eye broke allowing fan to strike employee cutting his left forearm.

Employee was driving personal vehicle and leaving job site when he stated that he misjudged the roadway and went into a ditch on left side of road.

Inserted hand into heater box inside of cab; fan motor was running and lacerated his finger.

Operating a front end loader carrying a large rock in the bucket. This caused the loader to bounce allowing the operator to leave his seat and be bounced around in the cab of the loader.

He was operating a haul truck, when he ran up on the berm and turned the truck over on its side.

Employee was driving a shuttle car. After dumping his load of coal and on his way back to the face the shuttle car hit a bump or hole in the mine floor. The jolt caused the employee to hit his head on the canopy of the shuttle car.

OPERATING 980 FRONT END LOADER, WHILE TAKING A BUCKET OF GRAVEL DOWN A HILL, EE LOST CONTROL OF THE END LOADER. EE JUMPED FROM THE LOADER BEFORE THE FRONT END LOADER WENT INTO A DITCH.

EMPLOYEE WAS OPERATING A ROCK TRUCK WHEN A ROCK ROLLED OUT OF THE ENDLOADER BUCKET AND STRUCK THE SIDE OF THE ROCK TRUCK BED. THIS BOUNCED HIM AROUND IN THE CAB OF THE ROCK TRUCK.

THE DIESEL TRK THE INJ EE WAS DRIVING DIED. THE EE SET THE EMERGENCY BRAKE & GOT OUT TO LOOK UNDER THE TRUCK. WHILE LOOKING UNDER THE TRUCK FROM THE SIDE, THE EMERGENCY BRAKE FAILED AND THE DOOR CAUGHT HIM AND DRAGGED HIM FOR APPROX 4 FT.

While operating loader, EE apparently jarred himself, causing pain to his lower back. He worked 6.5 hours the next day and went to Williamson A.R.H. to have check-up.
