EMPLOYEE WAS OPERATING A UNIHAULER WHEN HE STRUCK THE RIB, UNDERCUTTING IT. THE RIB ROLLED INTO THE DECK, STRIKING HIM IN THE BACK.

Employee was coming around the canopy (making a turn to the right) when his finger tips were caught in the swivel point. This resulted in two finger tips being pinched & crushed.
Employee alleges that he injured his back while operating a shuttle car over what he claims to have been a rough roadway. He did not report this accident until 1/29/2005. Mine mangement feels that this accident should have been reported before the employee left the mine- if that is where the injury occurred.
Employee was injured when the shuttle car that he was operating in #2 Entry (near spad # 10686) hit a hole in the bottom & jammed his head into the canopy. Management became aware of this accident on 1/17/05 & he started missing work on 1/20/05.
EMPLOYEE WAS TRAMMING SHUTTLE CAR TOWARDS THE INNER WHEN HE STRUCK A TIMBER CAUSING THE TIMBER TO ENTER THE OPERATOR'S DECK AND STRIKE THE EMPLOYEE.
Employee claims to have injured his back when he ran over a rock in the roadway while operating a scoop. He did not start missing work until 10/21/04 & did not report accident to mine management until 10/22/04.
