While backing up haul truck to shovel, employee had neck pain when the truck struck the shovel dipper which caused the contents to drop in truck bed.

While traveling to the primary crusher, Haul Truck 17 veered into the berm and came to rest on its non-cab side.

While attempting to make a left turn in the company LV335 flatbed truck, it was hit by a personal vehicle on the driver side door. The impact caused the employee to hit their head on the driver side window.

while attempting to make a U-turn on the UTV, the UTV turned onto its side. This resulted in injury to lower back, right flank and right hip.

The employee was leaving Shovel 4 and 6 pit in 6th gear when EE hit a pothole in the road. The employee stated the truck bed raised up a little and came down, causing the seat to bottom out. The employee was seen by Occ Med on 9/23/25 and was released without restrictions. On 9/26/25, the employee was seen by Occ Med and was returned to work with restrictions.

Rubber tire driver hit a few potholes and felt a slight pain on EE's chest and low back. When the driver stopped to check equipment, EE felt dizzy and could not stand.

Haul Truck operator hit an unknown bump, a few minutes after began to feel pain in EE's back.

The employee was in the cab of the radial stacker when the 2450-002 luffing system base failed at end of shift causing the stacker cab to drop to the right side. The operator in the cab, experienced some discomfort to the left leg after the luffing system failed.

While disconnecting the rear hose attachment of the CCV003 (Vac Truck) to clear a plugged hose, the top part of the hose whipped back and hit the employee's front tooth, cracking the bottom part of the right front incisor (central) tooth.

While reversing EE's Haul Truck to park in a chock ditch, EE's rear tires dropped into the ditch causing EE's seat back to fail and when EE's body fell back with the seat, EE's feet slipped off the foot pedals and EE's right foot got caught between the brake and accelerator pedal causing pain.

Haul truck 66 was traveling behind haul truck 25 when the right-side rock guard of haul truck 66 made contact with the dovetail of haul truck 25 causing haul truck 66 to fishtail in a circle, jarring the operator from side to side inside the cab, causing injury.

The fork slid off and pinched the top of the employee's fingers between the shaft weldment lock plate lock and the fork. The employee suffered an injury to the tip of #4 digit of the left hand.

Employee was a passenger in a light vehicle on July 13,2022 when a man van failed to stop in time approaching a stop sign and rear ended the light vehicle causing lower back soreness. Restricted Duty implemented on December 22, 2022 due to increased/unresolved pain and MRI results.

Employee rounded a 90 degree corner on personal motorcycle and lost control, skidding along pavement on 5/26/22. Employee was evaluated at the ER 5/26 and Occ Med 5/27. Upon 4th follow-up appointment with Occ Med 6/6/22, employee work status changed to restricted.

Employee started experiencing back pain after the man van hit a pothole on the haul road coming in at shift change the previous day.

While attempting to turn at the Crusher pocket, the employee noticed the steering was tight on Haul Truck 27 and continued making the turn. This resulted in the employee feeling pain in their left shoulder.

The operator of Haul Truck 38 was traveling empty off of the 4675 waste dump when EE hit a pothole, jarring EE causing pain to EE's lower back.

Driver of Haul Truck 43 was backing into the shovel 5 pit to be loaded, the driver continued backing into the shovel dipper as the load was dropped into the truck bed, jarring the driver. An hour after the load, the driver noticed a headache and some neck stiffness.

The driver of Haul Truck 28 was being loaded at Shovel 2 leaning fully on EE's right arm on center console, feet not touching the floor boards. The shovel's second dipper loaded caused the operator to feel pain in the right shoulder due to the improper positioning for the task.

While employee was sitting in the passenger seat of a 994 loader, EE heard a pop in the neck while the loader was reversing.

The 1st tooth on the loader bucket made contact with the rear side of the canopy of the haul truck. The truck operator felt a jolt, and experienced lower back pain. The employee received treatment and was cleared to return to work by occupational medicine. On the employee's first day back at work, employee experienced additional pain and was given restrictions.

Employee complained of sore back after lowering the bed of the haul truck with material in it.

Employee pre-spotted Haul Truck #29 at Shovel 3. When the operator of Shovel 3 went to clean up material in the Shovel pit, the dipper caught the dove tail, raising the rear of the truck.

Employee claims back pain after the first shift riding along in a haul truck. Employee stated it may be due to a prior personal injury. No report of this incident had been made to supervision prior to receipt of workers compensation report from hospital 8/28/18.

Employee was shaken in haul truck and felt pain in the back.

While moving pipe and transformers in the warehouse storage yard with a forklift. The employee alleges to have hurt back in the process of driving the forklift. The employee alleges that after and during the task noticed ee back was starting to ache, and while walking into the warehouse felt sharp pain that ran down lower thighs to the back of knees.

Employee approached the extended boom forklift from the blind side to discuss job task with operator. After discussion employee turned to walk away at which time the employee's left leg was caught under right rear tire, pulling him to the ground partially running his leg over.

Employee was in a haul truck and was being loaded by a shovel. A boulder in the dipper of the shovel shifted after it had been dumped into the haul truck, striking the front of the dump bed causing the truck to be shaken, injuring the haul truck driver.

Employee claims to have strained his neck while operating a 994 loader. This was reported on 1/22/14 but the employee can not recall the exact date of injury.

Employee claims that his knee injury is a result of continuous operation of a 994 loader. Employee received work restrictions during a follow up appointment on 08/23/2013.
