Traveling between stockpiles to load customer trucks, employee traveled over a pile of stone placed in the yard to prevent customer trucks from traveling over a soft area and becoming stuck. The employee was jostled around and struck the right side of EE's head against the cab window breaking out the window, cutting EE's head and contracting an abrasion to the right cornea.

Ee was examined by Fit for Work therapist was treated with ice, bandage wrap, exercises. Checked several days over week. Ee stated still had some pain. Sent Ee to clinic on 5/3. Ee returned on 5/4, restricted no manual transmission. Moved Ee to operate plant, hand/wrist not used.
Employee operating a man-lift across stone yard when ran over an 8" to 10" rock that catapulted employee in the man-lift. Employee had a retractable harness on keeping employee contained in the basket but not preventing all injuries. Employee received 4 stitches to lip along with a few scrapes and bruises.
Employee was performing a repair to a front end loader. He was removing a cylinder. He had a lifting strap secured to the cylinder he had the other end secured to his truck crane. He was raising the cylinder when it swung he reached for it with his left hand the cylinder pushed his left hand against the frame of the machine resulting in a laceration to his thumb.
