Employee was lifting part with overhead crane in shop when part swung striking left leg

Employee was operating forklift when the forks contacted the ground causing the machine to jar causing employee to hit head against side bar of machine.

Employee caught fingers on belt drive while inspecting belt clips, causing fracture to left ring finger and laceration to left middle finger.

Employee struck his head on side window of haul truck he was operating when it came in contact with a front-end loader causing a neck sprain and forehead contusion.

Employee was walking in back of a pickup truck that had just started to go in reverse when EE was struck in right shoulder/chest area, resulting in a chest contusion.

Employee stated that when he pressed on the foot brake on his 988F loader, he felt pain in his left ankle. This was not reported to Management until 8/6/13. Diagnosis: ankle sprain.
