Employee stated that the GT568 4 Wheeler jumped into high tram causing employee to strike head on the steering wheel. This caused a laceration to the forehead that required sutures. Employee did not miss any scheduled work days due to this incident.

While operating a Shuttle Car, EE's shirt sleeve caught between the coal rib and the SC. frame pulling EE's left arm into contact with the rib resulting in an evulsion to the Left forearm. EE received 16 Sutures.
Employee stated that ee had pulled onto the feeder to dump the shuttle car and stepped out of the buggy to turn seat around without setting the park brake on the center drive shuttle car. The car rolled back and caught lower right leg (calf) between the shuttle car and the mine rib.
Employee stated that ee was attempting to hook a chain to the scoop bucket when the operator of the scoop inadvertently lowered the scoop bucket onto left leg just above knee.
Employee stated they had stopped the Mac-12 Mantrip near a scoop in the #4 entry to attempt to pull a cable using the mantrip when the mantrip rolled into the scoop pinching their left leg. This injury resulted in a fractured left femur.
Employee stated that as employee was tramming back toward the section feeder the shuttle car slid toward the rib where there was a crib block. The crib block slid into the shuttle car and struck the employee on employee's left lower abdomen.
MANTRIP(RAILRIDE) GOING THROUGH DOOR, RAILRIDE CAUGHT DOOR KNOCKING SUPPORTS LOOSE. CINDER BLOCKS OVER DOOR HEAD FELL STRIKING INJURED ON HEAD.
Employee was backing up on rubber tired ride when he ran over rock / debris which caused him to hit ledge in roof and drag him off of ride.
INJURED WAS GOING THROUGH A SET OF AIR LOCK DOORS ON A BATTERY RIDE. HE CLOSED THE FIRST SET OF DOORS, OPENED THE SECOND SET. GOT ON HIS RIDE TO GO THROUGH THE SECOND SET. WHEN HE PUSHED THE TRAM PEDAL DOWN, THE PEDAL STUCK, HE HAD IMPACT WITH THE TOP OF DOOR FRAME. HE CUT HIS NECK, BROKE INDEX FINGER ON LEFT HAND, AND HAD A FRACTURE TO HIS UPPER JAW.
INJURED WAS TRAMMING A SCOOP, HIT A HOLE, AND GRABBED TO CATCH HIMSELF FROM HITTING THE BACK OF THE SEAT AFTER THE BOUNCE. HIS LEFT HAND WENT BETWEEN THE DECK AND REAR SECTION OF THE SCOOP WHEN IT WAS CLOSING TOGETHER. THE TIPS OF HIS RING FINGER AND MIDDLE FINGER ON HIS LEFT HAND GOT CAUGHT BETWEEN THE TWO SECTIONS. STITCHES WERE USED TO CLOSE THE ENDS OF HIS FINGER TIPS.
INJURED WAS TRAMMING A BATTERY POWERED SCOOP HAULING SUPPLIES. AROUND BREAK 94 HE HIT A POTHOLE THAT JARRED HIM AND SLAMMED HIM INTO THE DECK OF THE SCOOP. INJURY WAS TO THE BACK AND NECK.
EE was running a shuttle car. When the s/c was loaded he was going to switch seats, and as he was doing so, he twisted his shoulder.
EE was standing in a crosscut. EE stepped out of crosscut onto track. EE was struck by moving man-trip.
