Two employees setting up conveyor belt. They turned it on to check alignment. There was material still in the plant. A large rock on conveyor belt began to roll back towards employee, then made contact, causing abrasions & bruising to EE's neck & upper back. Employee was taken to local emergency room & released. Follow up with local clinic on Monday & placed on restricted duty.

Mostrando los 5Operator dismounted loader, lost footing, fell from last step. Landed on ground, ended up in front of pickup. Driver didn't see employee. Driver thought operator walked around loader, put pickup in drive, realized pickup wouldn't move. Driver reversed & saw operator on ground in front of pickup. Operator had abrasions on right arm & bruising on chest & ribs. Operator taken to ER.
An employee was attempting to feed the hopper with a loader and the bucket was low to the ground. When EE turned EE's bucket the top side of the bucket scraped the ground which caused the loader to come to an abrupt stop. (The employee admitted to not wearing seat belt during time of incident.) The impact caused the employee to strike the windshield with EE's head and shoulder.
Employee was driving a mechanic truck and drove through a rough area in the mine road causing employee's back to be strained.
On 6/29/2017 employee was operating a loader and strained back when the machine drove through a rut. Injury did not result in reportable restrictions until 7/10/2017 due to employee on vacation for the holiday. Spoke to the MSHA office of Injury Illness and employment.
