Employee stated that they and a co-worker were removing supplies from a rail mounted flat car and had removed one pallet from the flat car with a chain. When the employee tried to step back on to the flat car they slipped and their leg was now on the ground between the scoop bucket and side of the flat car. at this time the scoop rolled forward pinching their left calf.

Employee was attempting to place a cable reel onto a pallet. Chain was placed around cable reel. Forklift operator was setting reel down onto pallet when employee decided to step closer and remove chain from fork of forklift. At that moment, the reel rolled over onto the side of the employee's left foot near the ankle.

The contractor stated was coming down haul road at the #3-2-1 markers to the Harris Flat in a 10-wheeler. EE shifted down into 2nd gear, but it popped back out, and EE couldn't get it back into gear. EE then put it into the ditch on the offside of the road and it was slowing down but ran up on a rock and the truck turned over onto the offside.

EE stated that EE was operating an end drive shuttle car when EE drove through a hole in the mine floor. This caused EE's head to be jarred into the canopy which caused pain in EE's neck. Employee continued working but had to have surgery on 8/29/2024 (this is also the first day of work EE has missed).

Employee drove down from the loader pit to the dump area in a loaded rock truck. As the Employee turned left toward the dump, EE did not observe another rock truck ahead that was stopped waiting to dump. As a result, Employee's rock truck cab contacted the back of the tail chute of the rock truck ahead. Due to the impact, the Employee sustained injuries to the left leg.

IW was operating a rock truck. IW was struck by another rock truck operator. The collision impact resulted in injury to left shoulder.

Employee was bent down cutting a piece of belt in a cross cut behind a diesel mantrip. A diesel scoop turned into the crosscut striking the employee on left torso. Employee sought medical treatment on 09/23 and was released to come back to work. Employee went to doctor again and was placed off until 10/4 where results show contusions to rib area.

Employee stated was operating an 830E size rock truck and dumping some material. Employee stated while lowering the bed of the truck it jarred the cab. Employee felt discomfort in neck and left arm.

Employee stated: was trying to remove a stuck cutter motor from the #255 miner and had a chain hooked to the motor from the service scoop. They put pressure on the chain and was hitting the top of the miner with a sledgehammer. The chain broke and the boom of the service scoop hit employee on the left middle finger.

Employee was loading spoil material and came in contact with a large size rock. Employee began to lift the rock with the loader bucket when the rock tilted over the edge of the bucket causing the weight of the rock to push the bucket into the ground. Resulting in jarring the cab and employee feeling discomfort in the lower back area.

Employee states that while riding outside on the #4 mantrip that the mantrip spread the track at bk7 on 9c. When the mantrip did this, the employee stated that EE's back hit the metal seat causing pain in the lower back and rib cage.

Employee was operating a 980H front loader. Employee was handling coal from the SHM-75 stacker and transporting over to a stockpile area. Employee hit a hole located in the pit area and jarred the neck area.

The move crew loaded belt crossover on mantrip canopy. Employee sat on mantrip batteries while someone else trammed mantrip. Crossover got caught on lifeline and pinned employee's head against canopy resulting in laceration to head

Employee Stated as the owl crew were departing the motor barn on #1 rail runner mantrip. The mantrip struck the derail and wrecked. The derail was located one foot outside of the motor barn. Injured had a hand positioned at the top of the batteries. When the mantrip wrecked the batteries slid forward catching right pinky finger between the batteries and frame

Employee stated was riding the Diesel mantrip to the section when the ride hit a bump causing employee to jar back. Employee was taken off work under doctors care due to back strain.

Employee stated was attempting to pick up a roll of belt with the loader when the belt slipped off the forks. When the belt slipped off this caused the loader to teeter throwing EE into the canopy injuring neck. Employee sought medical treatment on 7/10/2019 and was placed off work under doctors care.

Employee was operating the articulating service truck on a slight grade when it started sliding, the employee tapped the breaks and the back end of the vehicle fell on its side, leaving the operators cab right side up. Employee jarred left shoulder against cab during the turbulence. Employee was taken off work under doctors care.

Employee stated was riding in a mantrip when the mantrip hit a kink in the rail then wrecked causing the injured employee to hit head on the seat. Employee complained of head and neck pain. Employee was taken to the hospital and taken off work under doctors care.

Employee stated EE was operating a rubber tire diesel mantrip when EE hit a bump in the road causing EE to strain EE's back.

Employee was operating a 777 cat rock truck and getting loaded by a 992k cat loader. When the loader man dropped the first bucket into the bed of the truck, a big rock hit the truck and jarred the truck driver, causing EE to hurt EE's back.

Employee was driving to work in EE's personal pickup truck on the haul road, when EE lost control of EE's truck, causing EE to ride up on the berm and roll EE's truck one time.

Employee stated they had loaded roof straps on top of the scoop (battery end). Employee stated they backed up battery end of scoop and hit the rib and pushed the straps into the operators compartment striking their left hand causing a laceration. Employee was placed off work under doctors care.

Employee stated was operating a shuttle car when ran over a rock causing the employee to be jarred into the canopy. Employee complained of back and neck pain. Employee was taken off work on 6/5/2018.

Employee was operating a shuttle car when the shuttle car struck a rut causing back pain. Employee was initially given a full release but on 4/30/2018 ee returned to doctor and was taken off work at which time the incident became reportable.

Employee stated EE was operating a shuttle car when the shuttle car struck a small rut causing the employee to jar EE's back resulting in a sprain/strain injury and lost work days.

Employee stated that the man-trip hit a hole and jarred their back. Pain started going down the right leg immediately. This resulted in sprain/strain and lost work days.

Employee stated EE was operating a shuttle car on the #2 section when EE hit an irregularity in the mine bottom, which caused EE to be jarred in the operators compartment. This resulted in back/neck strains and lost work days.

Employee was operating a 785C Rock Truck along the haulage road to the valley fill area and lost control of the truck and went through the berm. The truck overturned and came to a rest at the bottom of the valley fill road. This resulted in fractures to the lumbar vertebrae and lost work days.

Employee stated was operating the scoop on the #2 Section in the #3 Entry when the scoop went over an irregularity in the bottom, causing EE to be jarred in the operator's compartment of the scoop. This resulted in a left shoulder and neck strain and lost work days.

Employee stated ee was operating a shuttle car in the #8 entry and the shuttle car got close to the line curtain. In order to prevent tearing the curtain down the employee jerked leg over to hit the brake. When ee did this ee felt pain in groin area. This resulted in a strain and lost work days.

Employee was operating a shuttle car on the #2 section when the car bottomed out and became hung in the intersection. When the shuttle car was freed from the hole in the mine bottom, the employee was jarred inside the operators compartment. This resulted in a neck/back strain and lost work days.

Employee stated was operating the shuttle car on the #1 Section when the shuttle car went over an irregularity in the roadway, jarring employee. This resulted in a back strain and lost work days.

Employee stated EE was operating the shuttle car on the #1 Section when the shuttle car went over an irregularity in the roadway, jarring EE. This resulted in a back strain and lost work days.

Employee stated EE was operating the shuttle car on the #1 Section when the shuttle car went over an irregularity in the roadway, jarring EE. This resulted in a neck strain and restricted work days.

Employee stated employee was riding the #865 mantrip to the section with the rest of the crew when the mantrip hit a hole in the roadway jarring employee's back. This resulted in a strain and lost work days.

Employee stated that ee was operating the #15 mantrip at 94XC when the seat came loose and fell off of the ride. EE struck left hand and left knee, resulting in strains and lost work days.

Employee stated was operating a scoop and encountered a communication and junction box hanging from the mine roof. Employee used left hand to hold the cable away from the canopy while operating the scoop. Employee's L index, middle, and ring fingers were caught between the canopy of the scoop and the rib, resulting in an amputation and lost work days.

Employee stated ee was riding in the passenger side of a 14 man diesel mantrip. The mantrip was backing into a break to turn around when the right back corner caught the rib jarring the employee and straining lower back.

Employee stated employee was riding in the back of a diesel mantrip when the mantrip hit an irregularity in the mine road causing employee to strain back. The result of the strain was lost work days.

Employee driving down the hill and wrecked vehicle, went over the hill. EE told the supervisor that steering wheel locked up and gas petal stuck. EE told supervisor ee had cut knuckle and a concussion and could return to work on 2/16/17

Employee stated was operating a scoop along the #2 Roadway and hit a hole, jarring employee inside the operators compartment. This resulted in a back strain and lost work days.

Employee stated ee was operating the shuttle car on the No. 2 Section. EE states ee hit a hole in the roadway, which caused ee to bounce in the operators compartment. This resulted in a strain and lost work days.

Employee stated EE and another employee were loading a spool of cable into the bed of a truck. The spool was lifted with a forklift, and EE guided the spool into the truck. EE was holding the end of the cable with left hand. When the forks lowered, EE's left arm was pinched between the forks and truck bed. This resulted in a contusion and restricted work days.

Employee stated that they were operating the scoop along the roadway when the scoop hit a hole. This caused the employee to be jarred in the operator's compartment. The incident resulted in a back strain and lost work days.
