Employee placed their hand on a stationary safety ladder positioned on the forks of a forklift. Forklift operator was instructed to extend the boom. Ladder struck a piece of steel, ladder bounced and struck EE on the left side of head, resulting in lacerations to left cheek and behind ear. EE was transported for treatment.

Operator not paying attention to surroundings.

While working, employee noticed eye was irritated. After further medical evaluation, employee received prescription eye drops.
