Injured employee was driving a scoop to the section charging station when EE ran over a crib block causing EE to bounce up off of the seat. When employee landed back in the seat EE felt pain in EE's back and tail bone. Employee has training and experience.

Injured employee was riding on the outby end of a 7-man diesel mantrip. As the mantrip approached a track switch to get in, the mantrip operator failed to pay attention to the mantrip in front of them also entering the switch, and struck the end of that mantrip. The collision jarred the injured employee who complained of back pain which became worse during the shift.

Employee was operating a diesel locomotive on the outby end of a trip when they ran over a d-rail device causing them to d-rail and collide into the coal rib. Their right hand was caught between the locomotive body and rib. The result of the injury is amputation of the middle and ring fingers to the middle phalanges and amputation of the little finger to the distal phalange.

Employee stepped under a loaded muck bucket to clean the bottom when it dropped suddenly striking employee in the top of the hard hat.

Injured employee was traveling on a mantrip with a co-worker to go make a belt splice. Injured employee dismounted the mantrip at a track switch for the mantrip to enter and as the mantrip passed by employee, a splice board was sticking out and struck upper left leg. Employee has pain and a contusion. The employees failed to follow Company safety policy, rules & regulations.

Employee was a passenger on a mantrip that was struck by another mantrip as it entered the 7 North Main Line Haulage from 4W section haulage at the 4W section switch. When the collision occurred, the employee's head struck the window frame of the mantrip causing a laceration. EE received 5 staples to close the cut. Injured employee didn't violate any rules or regulations.

Employee was parking EE's mantrip on the 7 North Portal Bottom, 11 blk. spur. As EE entered the spur track, EE pulled on EE's brake lever and locked up EE's brakes and slid into a parked mantrip. Employee fell from EE's seat and landed on EE's back inside the mantrip.

Injured employee was a passenger on a diesel mantrip seated on the enclosed inby end. The mantrip operator failed to slow down for a kink in the rail at 83 1/2 blk. on the 3W section track haulage causing the mantrip to de-rail. Injured employee fell sideways striking left shoulder against the side of the mantrip.

Injured employee was directing a scoop operator into position to unload supplies. Injured was standing behind a Longwall shield that had been prepared for transportation off the 2W Longwall tear-down face. As the scoop operator was driving past the shield to unloading point, the scoop slightly contacted the shield causing it to pivot onto the injured's right foot.
