Employee was using a pick to cut a coal sample from the rib. While swinging the pick into the rib, a small piece of coal flew back and hit EE in the mouth, causing a small cut to EE's upper and lower lip. Employee received one stitch in EE's lip. Returned to work EE's next shift.

Employee was walking along the 7 North Track and the top of EE's hard hat struck the head of a roof bolt in the mine roof. Has pain in EE's neck. No rules or regulations were violated. Employee has training and experience.

Employee was installing roof bolts on the right side of the continuous miner. EE had completed drilling a hole and was pushing a roof bolt up in the hole when EE felt a "pop" in EE's right shoulder. No rules or regulations were violated and EE was wearing required PPE. Body failure performing normal job duty.

Employee assisting laying track rails on the 7 North Haulage. EE was helping co-workers roll rail over using a long pry bar. As they were rolling a rail, EE stopped but employees on the other end continued to roll their end, causing the bar EE was using to swing back and struck EE on the right side of EE's neck and jaw. Failed to maintain control of the hand tool EE was using.

Employee was installing center bolts using the Fletcher Roof Bolter. Had drilled EE's starter hole and just changed drill steels and when EE started drilling again, a piece of rock fell from the top and bounced off the drill head and struck EE's left forearm. Suffered a contusion with swelling. Missed 3 days of work. Has training and experience and was wearing required PPE.

Injured employee was carrying a belt roller with both hands along the 7 North #1 Belt between 24-25 blk. when they tripped on a piece of loose coal and lost balance. They fell against the side of a roller cradle, smashing right hand between the cradle and roller they were carrying. Suffered a contusion & swelling. Was wearing required PPE including metacarpal gloves.

Injured employee was cleaning belt water sprays in a spray bar. They brushed against a fire tap at the location they were working, between the top & bottom belt. When they backed out of the belt and stood up, the fire tap valve suddenly blew off the 8" waterline coupler and struck their left ankle, breaking it. Accident caused by a failure at the coupler tee due to corrosion.

Injured employee hooked a scoop winch hook to a shield on a 30-Ton car to center up the shield on the car. When the scoop operator tightened up the winch rope, the hook slipped off the shield and struck employee's right leg above the knee. They suffered a contusion/bruising, pain & swelling. Employee failed to position themselves in a safe location.

Injured employee was helping hang the continuous miner cable. Lifted it to EE's shoulder and then up into a hanger. Felt pain in EE's shoulder and later after work, EE noticed a small knot and swelling in EE's shoulder. Reported the incident on 3-9-26 and sought treatment that day and began missing work. No rules or regulations were violated and EE Was wearing required PPE.

Employee was operating a roof bolter installing bolts on the 8W CM Section. EE stuck EE's left hand down on the bolter head to check suction and at that time, a drill steel fell out of the hole EE was drilling and smashed EE's left little finger against the drill head. Employee suffered 2 small fractures to the end of EE's little finger. Was wearing metacarpal gloves.

Injured Employee was walking beside a parked Shuttle Car and slipped on loose coal (sloughage) and fell against the rib. They stuck right forearm out against the rib to catch themselves. Forearm hit a spad that was in the rib causing a small puncture and a cut requiring 3 stitches to close. Employee has training and experience. Returned to work next scheduled shift.

Employee was using a grinder to cut a bolt when the grinder disk came apart and struck EE in the left hand resulting in a laceration. The injury resulted in 5 sutures.

Injured employee was walking and tripped on a crib block causing EE to stumble and roll EE's right ankle. Has been diagnosed with a torn ligament. No rules or regulations were violated and employee has training and experience.

Employee was operating a diesel locomotive on the 8W CM Section track. They parked the motor and stepped out of the deck onto a steel tunnel liner for hoses to pass under the track rails. They slipped on loose gravel that was on the steel cover, slipped and fell down, landing on their right upper leg. No rules or regulations were violated & employee has training & experience.

Employee was standing beside the continuous miner waiting to connect and hang the next ventilation tube. While they were waiting the top fell out between the roof straps and struck them knocking off their hard hat. A small piece of coal/rock followed and struck the top of their head causing a small cut. They received one staple to close the cut. Returned to work their next shift.

Injured was assisting pulling longwall hoses at the Longwall belt drive using a sheave wheel connected to a roof hanger. A nylon rope was run through the sheave and connected to the hoses. As the rope was being pulled, the hanger broke and the sheave struck employee's right knee. Employee was behind guarding but moved away from it during the process.

Employee was assisting installing J-screws into rib. When the bit on the drill became lodged and stuck, employee attempted to free it by pulling on the drill. Employee's hand slipped onto the bit, which spun and injured the employee's hand. Injury was not the result of a training or compliance issue.

Injured employee was dragging a ventilation tube to the right side of the CM for installation. As EE approached the coal pile behind the CM EE set the tube down and stepped across the pile. EE stepped on a loose piece of coal and rolled EE's left ankle. Diagnosed with a ankle sprain.

Injured employee was lifting a scoop battery lid to prop it up for venting while charging. As they were lifting the lid with left hand, it slipped and fell smashing the tip of right thumb. Employee suffered a fracture to the tip of the thumb. No rules or regulations were violated. Employee has training and experience.

Injured employee was using a rail hook to lift a track rail and felt a "pop" in their back. They then stood up and stumbled and fell on right hand smashing their index finger. Employee sought treatment the following day for back pain. No rules or regulations were violated.

Injured employee and a co-worker were loading 10-foot joints of 6-inch pipe (waterline) onto the 7W Longwall Belt. One person on each end of the pipe. As they were tossing a joint onto the belt, the injured lost grip and left arm became smashed between the pipe and the belt structure. They suffered a broken left ulna bone as a result. No regulations were violated.

Injured employee was roof bolting on the left side of the continuous miner. Employee was in the process of installing a rib bolt when a piece of coal/rock fell from the brow behind them and struck them on the left shoulder. Was diagnosed with a deep tissue contusion and given time off. No rules or regulations were violated.

Employee was stepping out of the diesel locomotive they had parked. They stepped off the locomotive as they reached for a handle for support but the handle wasn't on this locomotive. Lost balance and fell landing on right shoulder. Has muscle damage in the shoulder. Employee has training and experience. Was wearing required PPE.

Injured employee was lowering a scoop battery lid and as they removed the lid support arm with right hand, the lid slipped out of left hand and pinched the tip of right thumb against the frame. Diagnosed with a hematoma under the thumbnail. Missed one day of work due to this injury. Was wearing PPE (Metacarpal Gloves) at time.

Injured employee was hanging a check curtain on the 7 North Mains CM Section. EE used an 18" high work platform so EE could reach the mine roof. While standing on the platform, EE pulled up on the curtain, lost EE's balance and fell off it, landing on EE's right shoulder. Result was a dislocated shoulder. Employee has training and experience. No rules or regulations were violated.

Employee was installing a ram pin at 148 shield on the 7W Longwall face. EE was using a hammer to drive the pin in. EE switched hands to hit the pin squarely. As EE was striking the pin with the hammer EE missed and hit EE's right little finger, causing a laceration requiring 7 stitches to close. Employee has training & experience and was wearing required PPE.

Injured employee was assisting a co-worker hang a ventilation tube from the mine roof at the face beside the continuous miner. As they were hanging the tube in place, a rock fell from above the tube and struck employee on EE's right shoulder and upper leg / thigh area. Has swelling and bruising to EE's leg. No rules/regulations were violated and employee has training & experience.

Two employees were building a dry-stack block wall. As the wall was near completion and they were attempting to wedge it tight, it began to move toward them. As they attempted to get away from it, it fell striking injured on EE's right side and below EE's waist, knocking EE to the mine floor. Suffered contusions to EE's lower left leg & fracture to EE's left foot.

Employee was engaged in the mucking phase of shaft construction. A bucket of muck was sent out of the shaft when a rock fell striking them in the hardhat. This resulted in the employee having a small laceration which needed 2 staples and a mild concussion.

Injured employee was driving a scoop to the section charging station when EE ran over a crib block causing EE to bounce up off of the seat. When employee landed back in the seat EE felt pain in EE's back and tail bone. Employee has training and experience.

Injured employee picked up a shuttle car cable to move it out of the roadway. Felt a burning ache in EE's upper forearm. Employee worked EE's next 2 shifts but later sought medical treatment. Began missing work on 9-16-25 due to this injury. Employee has training and experience. No rules or regulations were violated.

Injured employee was operating a shuttle car when a small piece of coal fell from the rib and into the operator's cab and struck their left little finger. Employee suffered a small abrasion/contusion to the finger. Employee worked their next shift but sought treatment the following shift. Employee began missing work on 9-12-25 and missed 2 shifts of work.

Injured employee was walking past the Longwall Stageloader toward the face when a piece of coal fell from the roof and hit EE on the back causing EE to fall against the side of the Stageloader near the #1 shield. Received a cut above EE's left eyebrow requiring 5 sutures to close. Returned to work EE's next scheduled shift. No rules or regulations were violated.

Injured employee was walking along the shuttle car haul road on the 8W CM Section. EE heard a shuttle car approaching and hurried to get in a cross-cut. Stepped on a lump of coal and rolled EE's left ankle. Diagnosed with an ankle sprain. Did not violate any rules or regulations.

Employee was placing scoop batteries on charge. After plugging in the batteries, EE raised the lids to vent and was overcome by an unknown gas. Employee suffered watery eyes, runny nose, headache, and labored respirations. Employee was transported, given breathing treatments, examined & released. No regulations were violated and EE has training and experience.

Injured employee was operating longwall shields when EE stumbled on a relay bar. EE put out EE's right hand to catch self but EE's hand landed on a bolt plate (Jennmar Mini Monster Mat), causing a laceration. Employee sought medical attention and the laceration was glued closed and bandaged. No rules or regulations were violated and employee has training and experience.

Injured employee was roof bolting from the continuous miner at the face on 8W CM Section. After installing a bolt, employee walked back to the bolt tray to get another bolt and a piece of coal fell from the roof and struck EE on the right shoulder. Employee stated roof conditions were good and coal fell without warning. Has pain & soreness and possible Labrum damage.

Employee was operating a hammer drill and struck themselves in the mouth causing them to chip their right front tooth.

Employee was vulcanizing belt when EE attempted to pick up a winch and strained EE's lower back. The employee began missing work on 7/7/2025.

Injured employee was removing a flight from the longwall stage loader chain. As EE was removing the top section from the bottom bar, it moved unexpectedly and fell on EE's left index finger. Injured received 7 sutures and a fractured fingertip as a result. No rules or regulations were violated. Employee has training and experience. Returned to work EE's next scheduled shift.

Employee hung a ventilation skin on the back of the section fan, and as they turned to walk away, a 10'L x 4'H piece of the inby coal rib fell striking them on right side knocking them to the mine floor. They sustained a cut to the forehead, and contusions and abrasions to the right side of torso and lower body.

Employee was mixing B-Bond cement by hand to seal a block stopping. EE was wearing rubber gloves and using hands to mix the material but got material on their shirt and over the gloves. Material caused burns to right arm where it contacted their skin. Employee was wearing required PPE but failed to keep the B-Bond from contacting skin. Missed 2 days of work.

Injured employee was removing "K-Wall" panels and as EE was pushing forward on a panel, EE felt a "pop" in EE's right knee. Finished EE's shift but sought treatment the following day and began missing work. Initial diagnosis is torn ligament around kneecap.

Injured employee was pulling on a cable during a Longwall Power Car move and as EE pulled on the cable, EE hyperextended EE's thumb causing a fracture at the base of the thumb. Employee returned to work EE's next scheduled shift.

Injured employee was walking through the #5-6 cross-cut at 116 block to get a ventilation tube. Stated EE stepped over a trailing cable and lost EE's balance and fell. Put EE's right hand out to catch EE's fall and hit the coal rib. Has pain and swelling in EE's wrist. Stated EE did not trip or step on cable or obstruction.

Injured employee was using a small "hook chisel" to remove a staple from a hose fitting at #74 Shield on the 7W Longwall face. As EE was prying the staple out, it broke free and the chisel came up and struck EE's face causing a chipped tooth. Employee did not lose any workdays due to this accident but had the tooth repaired on 4-23-25. Has training and experience.

Injured employee and co-worker were using a small crane mounted on a mantrip to lift and load a shuttle car reel on the end of the mantrip. As the reel was suspended it became stuck on the edge of the mantrip platform. The employees were attempting to free it up when the reel slipped off the hoist hook and fell on injured employee's right foot. Suffered a contusion and bone bruise.

Injured employee was dismounting the continuous miner after climbing up onto it to examine the conveyor chain for a possible rock jam. A EE was getting down, EE fell and struck EEs leg against the edge of the machine's cable handler. This caused a deep laceration to EEs left lower leg. Received stitches and given time off work for wound to heal. Has training and 2 years experience.

Injured employee was riding on the outby end of a 7-man diesel mantrip. As the mantrip approached a track switch to get in, the mantrip operator failed to pay attention to the mantrip in front of them also entering the switch, and struck the end of that mantrip. The collision jarred the injured employee who complained of back pain which became worse during the shift.

Employee was operating a diesel locomotive on the outby end of a trip when they ran over a d-rail device causing them to d-rail and collide into the coal rib. Their right hand was caught between the locomotive body and rib. The result of the injury is amputation of the middle and ring fingers to the middle phalanges and amputation of the little finger to the distal phalange.
