Employee was operating an open cab haul truck and while turning a strand of wire from the wire mesh ground support on the rib caused a puncture wound to the back of the employee's left shoulder requiring two sutures.

Employee was pulling into a cutout to get it out of the roadway and as they were pulling in they hit a bump and reached their arm out toward the rib and strained left shoulder. Employee ended up having to get surgery for a rotator cuff injury.
Employee was finishing mucking scraping the face when something struck the employee in the right side of the forehead.
EE was operating a haul truck on the mainline when they felt something go into their eye. EE was taken to the eye clinic and checked out. EE was given a full release at that time. On 11/21/2022 they contacted the eye clinic with discomfort in the injured eye. EE was given an antibiotic to treat the eye making it a reportable injury.
EE was standing on the 5900 station awaiting the conveyance for a ride out of the mine at the end of shift. The conveyance went by the station and blew some dust into the EE's eyes. EE was wearing safety glasses but somehow some debris got into EE's left eye and had to be removed by a doctor.
Employee Strained back while riding passenger in the lube truck. The driver hit a bump and the employee came off the seat and then landed back down on the seat. The employee was wearing a seatbelt. The employee did not seek medical care until 12/28/2020 when employee was place on restricted duty.
The injured employee hit forehead and nose on the radio mount of the Kubota when the vehicle slid into the rib.
EE was leaning over a rail by the 5900 ore pocket and was using a long hook to pull rock bolts off grizzly. While attempting to pull on a rock bolt the hook and bolt hung up causing EE to feel pain in his groin area. Later while operating a haul truck EE turned to see behind the truck and he felt pain in his back. EE was taken to a medical provider and was put on restricted duty.
EE was operating a haul truck when the e brake set, causing the EE to be jarred in the operators compartment.
On 07/07/2014 E was operating a 1 yd loader to stack muck in a muck bay. While the bucket was elevated at the muck pile a rock rolled behind the front wheel opposite the operator. When E began backing up with the bucket elevated the wheel rolled up on the rock causing the loader to tip over on its side.
EE was mucking with a 2 yard LHD along the left rib and felt a pain in his lower back. EE stated he was jarred in the operators compartment and that's when he felt the pain.
Employee cut his hand on the rib while driving a tractor, he came too close to the rib and cut his palm.
While driving a tractor down the road, he ran over a rock in the road and jarred his back.
Employee was scraping the rib near the face with his loader bucket. A piece of rock was loosened and fell against the bucket. When the rock struck the loader bucket it fragmented and piece of it was swept into his eye from the ventilation stream. The employee was taken to an optometrist who removed the particle. An anti-biotic was prescribed to avoid infection.
LHD rolled over injuring employee.
EE said rock got jammed on 10 belt; he reach for it and somehow his left arm got caught in a roller.
EE was operating a mine tender. A cushion slipped off the passenger seat & he leaned over to catch it. He took his eyes off the road & hit a rock. This caused his neck to whiplash.
Driving a tractor and hit a bump causing him to hit his head on a pipe and jam his neck.
Employee was driving an empty haul truck & hit a bump in the road, which bounced his hard hat off. He reached for the hard hat & his arm got pinched between the truck & the rib wall. The injuries to his arm were a severe avulsion of the forearm & fracture above the wrist. Seven days after the incident his hand had to be amputated above the wrist.
EMPLOYEE WAS HAULING A 12' JUMBO STEEL ON A LHD WHEN HE HIT A BUMP IN THE ROAD. THE STEEL BOUNCED UP WHEN THE LHD ARTICULATED AND ONE END WEDGED AGAINST THE FRONT OF THE LHD AND THE OTHER END PINCHED HIS FINGERS.
WHILE MOVING AN ACO PUMP IN THE BACK OF A TRACTOR BASKET, HIS PARDNER DROPPED THE BASKET ON HIS FOOT.

WHILE DRIVING A LHD IN THE MAIN RAMP, DUST BLEW INTO HIS EYES.
EMPLOYEE WAS TRAVELING ACROSS A SNOW FIELD AND HIT A TWO FOOT BOULDER BURIED IN THE SNOW. EMPLOYEE WAS OPERATING A SNOW MACHINE AT THE TIRE. THE IMPACT CAUSED THE EMPLOYEE TO FALL FORWARD INT O THE HANDLE BAR HIS RIGHT HIP WAS FRACTURED.
EE WAS MOVING EQUIPMENT FROM WORK AREA TO SHAFT STATION TO BE SENT TO THE SURFACE. WHILE ROUNDING A CORNER THE FLAT CAR JUMPED THE TRACK RUNNING OVER EE'S RIGHT CALVE. FLAT CAR WAS BEING PULLE D BY ELECTRIC LOCOMOTIVE.
EE'S LEFT EYE WAS IRRITATED BY DUST WHILE OEPRATING A 16 TON WAGNER TRUCK FROM THE 5700 LEVEL TO 5750 LEVEL. HE HAD A SMALL SCRATCH ON HIS EYE EMPLOYEE ASKED FOR A DAY ON SURFACE FOR 1 DAY. DR. WROTE A ONE DAY RESTRICTED.
HE WAS RIDING IN A MAN CARRIER ON THE BACK OF THE LANDINE TRACTOR THE TILT CYLINDER BRACKET BROKE THE MAST FELL AGAINST WHICH STRAINED HIS RIGHT ELBOW. MORE DR S CAR AND THERAPY MADE THIS MSHA REPORTABLE.
EE WAS RIDING IN A MAN CARRIER ON THE BACK OF A LANDINI TRACTOR. THE TILT CYLINDER BRACKET BROKE. THE MAST FELL AGAINST THE MAN CARRIER. IT PINCHED HIS FINGER (RIGHT INDEX). FRACTURE OF FINGER . ALSO, A LACERATION.
1ST SCRAPER LEFT UPPER CUT. 2ND SCRAPER WAS MAKING TURN TO HEAD INTO UPPER CUT, WHEN THE TWO SCRAPPERS CABS GLANCED OFF ONE ANOTHER. THE INITIAL CONTACT CAUSED THE 2ND SCRAPER TO JACK KNIFE IN TO BOWL OF 1ST SCRAPER. THIS CAUSED EXTENSIVE DAMAGE TO CAB OF 2ND SCRAPER, MINIMAL DAMAGE TO CAB OF 1ST SCRAPER. THE INITIAL CONTACT OCCURRED ON THE UPPER LEFT CAB CORNER OF SCRAPER.
EE WAS GRIZZLYING MUCK THROUGH A GRIZZLY WHEN A ROCK 18INX18IN ROLLED DOWN THE MUCK PILE & STRUCK HIM ON THE BIG TOE OF HIS LEFT FOOT THE BONE IN HIS BIG TOE WAS CRACKED
EMPLOYEE WAS DRIVING A 16 TON TRUCK WHEN A ROCK DUST OCCURED. THE TRUCK STRUCK THE DRIFT WALL JARRING HTEMEPLOYEE CAUSING INJURY TOT HE OPERATOR.
BACKED A SERVICE VEHICLE UP AND OVER A LARGE ROCK JARRING HIS BACK.
AT 8:45 AM EE WAS ASSISTING THE CONCRETE CREW SHOVELING CONCRETE OUT OF THE CHUTE OF HTE CEMENT TRUCK. EE MOTIONED FOR THE DRIVER TO MOVE AHEAD, DRIVER KNEW HE AHD TO LFIT THE CHUTE UP IN OR DER TO CLEAR THE REBAR BUT PUSHED THE HYDRAULIC CONTROLLED TOGGLE SWITCH THE WRING WAY FOR AN INSTANT, DROPPING THE CHUTE ONTO EE KNEE.
WHILE OPERATING AN UNDERGORUND TRAMMER, IT JUMPED TRACK ADN HE STRUCK HIS CHIN ON LIGHT BRACKET DUE TO THE SUDDEN STOP.
THE EMPLOYEE HIT A ROCK WITH HIS SCRAPER AND HE WAS JARRED IN THE CAB AT THE TIME OF THE ACCIDENT THE EMPLOYEE WAS TURNED LOOKING BACK. THE ACCIDENT WASN'T REPORTED UNTIL 10/14/93. THE EMPLOYE E WAS TAKEN OFF WORK BY HIS DOCTOR.
EMPLOYEE WAS DRIVING TO LUNCH AREA WHEN DUST PARTICLES BLEW OFF LHD HITTING HIM IN EYE.

EMPLOYEE AND PARTNER WERE MOVING DRILL AND EQUIPMENT TO NEW DRILL STATON. DRILL WAS ON MINE CAR WHEN IT DERAILED FROM TRACK AND CAUSED THE DRILL TO ROLL OFF THE CAR. IT PINNED EMPLOYEE BETWEEN DRILL & RIB.

HE HIT A ROCK WHILE HE WAS OPERATING A SUPPLY TRACTOR & JARRED HIS BACK.
HE WAS DRIVING HAUL TRUCK DOWN MAIN HAULAGE WHEN TRUCK HIT RIGHT RIB GLANCING INTO BACK FILL. THE TRUCK HOOKED ON THE PIPE STOPPING HIM AND THROWING HIM FOREWARD INTO THE FRONT OF THE TRUCK B RUISING HIS RIGHT SIDE AND SHOULDER.
OPERATOR BACKED INTO A HUNG STEEL WHILE OPERATING A L.H.D. 1 YARD LOADER FRACTURING 6 RIBS.
RECEIVED MUSCLE STRAIN IN BACK WHILE MUCKING WITH LHD.
MUCKING ROUND WITH A LHD WHEN HE STARTING HAVING BREATHING PROBLEM FROM POWDER GAS IN THE MUCK PILE.
MUCKING ROUND WITH LHD STARTED HAVING BREATHING PROBLEMS BY POWDER GAS IN MUCK PILE.
HE WAS DEICEING A ROLLER ON BELT WITH A TORCH THAT WAS APPROX 4 INCHES LONG. HE TURNED TO LOOK DOWN THE BELT AND AT THE TIME THE BELT GRABBED THE TORCH TAKING IT OUT OF HIS HAND THE VALVE PUL LED HIS GLOVE OFF AND CUT HIS FINGER.
HE FELL OFF THE PERSONNEL CARRIER OF A KUBOTA TRACTOR AND HIT HIS HEAD ON THE ROAD.
