EMPLOYEE WAS OPERATING A 25 TON UNDERGROUND HAULAGE TRUCK AFTER MAKING SEVERAL RUNS TO SURFACE HEAD FROM ENGINE RADIATED INTO OPERATORS COMPARTMENT CAUSING 1ST & 2ND DEGREE BURNS TO HIS RT.FOO T BURNS LATER BECAME INFECTED & HE WILL BE OFF WORK FOR UP TO THREE WEEKS
THE EMPLOYEE WAS OPERATING A 25 TON UNDERGROUND HAULAGE TRUCK. HE MADE 6 OR 7 RUNS TO SURFACE. THE HEAT FROM THE ENGINE RADIATED INTO THE OPERATORS COMPARTMENT CAUSING 1ST AND 2ND DEGREE BURN S TO HIS RIGHT FOOT.
EMPL WAS ON SURACE OPERATING 560 HOUGH FRONT END LOADER MOVING STOCKPILE NEAR HOIST. AS HE WAS INSIDE CAB OPERATING LOADER END OF LARGE HYDRAULIC HOSE BLEW OFF SPRAYING HIM WITH HOT HYD. FLUI D. WINDHSIELD HAD RECENTLY BEEN DAMAGED & REMOVED. HYD. FLUID CAUSED BURNS TO HIS LEFT SIDE & ARM. HE SUSTAINED 2ND DEGREE BURNS TO HIS ABDOMEN ON HIS LEFT SIDE.
EMPLOYEE WAS BACKING INTO AN UNDERGROUND WAREHOUSE AREA MUCKBAY BETWEEN THE 1152L AND THE 1284 LEVEL AND WHEN HE WAS PULLING OUT HIS HAULAGE TRUCK HIT THE OPPOSITE RIB AND SEVERAL SMALL ROCKS FELL INTO THE CAB. HE SUSTAINED A 11/2 INCH LACERATION TO HIS FOREHEAD AND ALSO A SMALL LACERATION TO IS LEFT HAND WHICH REQUIRED SUSTURES NO LOST TIME.
EMPLOYEE WAS BACKING UP A 25 TON TRUCK ON THE 880 LEVEL TO BE LOADED WITH BOULDERS. AS HE WAS BACKING UP, THE LEFT REAR TIRE RODE UP ON A SMALL BOULDER AND THEN SLIPPED OFF CAUSING THE TRUCK TO BOUNCE AND INJURE HIS BACK. THE ROCK HAD BEEN SPILLED ONTO THE ROAD WHEN OTHER TRUCKS WERE BEING LOADED WITH OVERSIZE BOULDERS.
THE INJURED WAS SHOVELING NEAR THE APRON FEEDER. A SMALL CLUMP OF DIRT FELL OFF OF THE MACHINE AND HIT NEAR HIM CAUSING SOME DIRT TO GET BEHIND HIS SAFETY GLASSES AND INTO HIS EYE.
EMPLOYEE WAS CO4ING FROM THE 1020L TO THE 1284 LEVEL IN HIS EMPTY TRUCK. EMPLOYEE STATES THAT HIS TRUCK JUMPED OUT OF GEAR. THE EMPLOYEE DID NOT HAVE THE TRUCK IN FULL CONTROL AND THE TRUCK SM ASHED INTO THE 9IB AT THE 1152 MUCKBAY. THE DRIVER WAS PINED INSIDE OF THE TRUCK AND HAD TO BE CUT OUT. HE SUSTAINED MULTIPLE BROKEN BONES AND CONTUSTIONS.
WORKMAN WAS RID3NG ON MAN CARRIER WHEN IT BROKE DOWM.HE TRIED TO CATCH A RIDE ON A TRUCK THAT WAS NEARBY.HE DISCOVERED THE TRUCK WAS PARTIALLY LOADED SO HE STOOD AGAINST THE EAST RIB OF THE D RAWPOINT PLACIN1 HIMSELF BETWEEN THE RIB AND THE TRUCK.THE TRUCK DRIVER STARTED TO MOVE NOT REALIZING THAT THE MAN WAS THERE.THE WORKER SLIPPED AND FOOT GOT RUN OVER BY TIRE OF TRUCK.
INJ BACKED HIS 5OADER INTO THE GRIZZLY BECAUSE OF STEERING PROBLEM. HE LIFTED THE COMPARTMENT TO CHECK STEERING CYLINDERS, MOVED THE JOY STICK, LOADER TURNED HARD LEFT AND LID FOR THE COMPARTM ENT CAME DOWN O3 HIS FINGER
THE INJURED EMP5OYEE WAS DRIVING A HAULAGE TRUCK IN THE DECLINE.THE TRUCK HIT A FEW BUMPS IN THE ROAD AND CAUSED INJURY TO HIS BACK.THE EMPLOYEE CONTINUED TO WORK UNTIL 5-3-87 WHEN HIS BACK PA IN WORSENED AND3WAS KEPT OFF WORK.
INJ EMP WAS HAN1ING FANLINE IN THE DECLINE. HE WAS RIDING ON THE MANTRIP WORK DECK WHEN THE MANTRIP TIRE HIT A BOULDER AND CAUSED HIM TO FALL DOWN ON THE WORK DECK STRIKING HIS SHOULDER
THE INJURED WAS3DRIVING A TRUCK DOWN THE DECLINE WHEN THE STEERING WENT OUT AND THE TRUCK TURNED INTO THE RIB. IT BOUNCED OFF THE LEFT RIB AND HIT THE RIGHT RIB AND STOPPED. THE DASH BENT IN T OWARDS THE SEAT3AND THE STEERING COLUMN HIT HIS KNEE AND LEG.
EMP WAS IN A LO5DER BUCKET SCALING LOOSE ROCK FROM THE BACK AND RIBS OF THE 128'4 ACCESS. HE HAPPENED TO LOWER THE SCALING BAR OVER HIS FOOT TO HIT A LOOSE ROCK, AT THE SAME MOMENT THE LOADER OPERATOR ROLLED3THE BUCKET BACK. WHILE THE SCALING BAR WAS STILL OVER HIS FOOT, IT CAME IN CONTACT WITH THE BACK. HIS LEFT FOOT WAS PINCHED BETWEEN THE SCALING BAR AND THE LOACER BUCKET
WAS DRIVING A H5ULAGE TRUCK AND THE BOUNCING OF THE TRUCK OVER THE ROAD CAUSED MUSCLE DAMAGE IN HIS LOWER BACK.
