Water Truck right tire pressed against sweeper causing it to deflate, and shifting the weight and making machine slide onto outer wall of the MCC room, causing metal box on interior wall to strike EE on right shoulder. Injury was considered first aid only until 3/9/22 when specialist prescribed physical therapy.

Employee was driving loader too fast on bumpy road and the jolting motion caused a strain in EE's lower back

EE was driving to the pit to load when EE saw another truck coming out of the pit. EE attempted to slow down and give the other truck the right of way, but in the process, jack knifed the truck. The steering wheel bruised EE's left leg.

Employee was operating the scraper. While discharging material from scraper, a large rock caused the machine to come to a complete stop violently shaking the cab and the operator.

EMPLOYEE WAS OBSERVING OPERATION OF EUCLID END-DUMP FROM PASSENGER SEAT. THE END-DUMP OPERATOR BACKED INTO RETAINING WALL OF LOAD OUT BIN CAUSING EMPLOYEE TO STRAIN HIS NECK.

INFECTION IN LEFT EYE DUE TO FINE GRAVEL STUCK OTO BELT COMMING LOOSE DURING START UP AND FALLING INTO EYE.

EMPLOYEE ATTEMTPED TO CLEAN CONVEYOR TENDION IDLER WITH PLANT RUNNING. HE WAS USING A SHOVEL TO SCRAPE MUD OFF ROLLER. SHOVEL WAS PULLED INTO PINCH POINT CAUSING SHOVEL HANDLE TO BE THROWING U P INTO FRAME OF CONVEYOR MASHING EMPLOYEES HAND. HE STUCK.

MECHANICAL BREAKAGE OF SEAT BRACKET ON EUCLID B-30, RESULTING IN CONTUSION OF LEFT TESTICLE, SCROTUM AND GROIN.

HIT POT HOLE WITH STOCKPILE END DUMP BOUNCING AGAINST BACK OF CAB CAUSING NECK STRAIN.

PIT TRUCK DRIVER PULLER OVER TO THE SIDE OF ROAD TO GIVE MORE CLEARENCE TO OPPOSING TRAFFIC AND GOT IN THE SEAT CAUSING MUSCLE SORENESS IN THE RIGHT LOWER BACK.

DRIVING TRUCK I3 RAIN TO FAST TRUCK SKIDDED AND JACK KNIFED EMPLOYEE NOT WEARING SEAT BELT FELL OUT OF SEAT INTO FLOOR. ACCIDENT HAPPENED ON SOUTH LOOP 289 IN LUBBOCK TEXAS.
