The employee was parked in a haul truck waiting to dump his load at the crusher. One employee pulled off in his truck and the side of his truck hit the left side mirror and bracket of the other truck causing it strike the side glass and windshield. When the glass broke it landed in the other employees eye.

Mostrando los 8EE was climbing down ladder on a Euclid R-50 size Haul truck. When got to the next to last step he stated that he thought that he was on the last step and stepped to the ground which was further than he thought causing him to lose his balance and fall. He fell 3' landing on his left side. X-rays revealed no injury, but family Dr. put ee off until 2/8/10.
Using skid steer to move a conveyor belt, the operator had the bucket over the belt moving it. The employee was trying to help move the belt. Operator raised the bucket and the belt started to roll. He sat the bucket back on the belt. Operator couldn't see the employee's hand on the belt trying to stop it from rolling.
Operating load in the pit. A rock rolled out at shot pile, hit loader bucket, made hand slip and jammed thumb which broke a bone above the thumb.
OPERATOR WAS DRIVING DOWN THE HILL LOADED. HE SAID THE TRUCK STARTED GOING FAST AND HE DROVE IT INTO THE DITCH TO STOP.
SPLICED CONV.BELT, AFTER COMPLETING SPLICE EE STARTED BELT & THOUGHT IT WAS RUBBING SOMEWHERE SO HE WAS LOOKING TO SEE WHERE WHEN A ROCK FELL OFF & LANDED ON HIS FOOT. EE BROKE A BONE IN HIS FOOT, EE HAD STEEL TOES ON.
OPERATOR TRAVELED DOWN HILL TO TURN AROUND SITE PUTTING MACHINE IN REVERSE AN BEGAN TO BACK UP FELT A JERK AND CAB OF TRUCK TURNED OVER AND CAME TO REST ON ITS RIGHT SIDE.
EE WAS OPERATING A CLEAN UP TRACTOR IN THE SECONDARY PLANT. HE WAS CLEANING FROM UNDER THE CONVEYORS WHEN A 2" TO 2 1/2" ROCK FELL AND HIT HIM ON THE LEFT KNEE.
