Employee was guiding a forklift moving a heavy motor. the motor became lodged on the floor of a connex, and in attempting to dislodge the motor, it rolled on the top of their right foot.
While dumping dirt from a 775 haul truck, the bed lowered rapidly and jostled the operator in the cab, causing what was diagnosed as cervicalgia. However, they did not report the incident until 28 July 2025.
Conducting pressure checks on a brake valve, EE went between the right front tire and the frame to attach the pressure gauge. The other mechanic in the cab turned the wheel to the left, thinking they would check the valves in the same order as the previous truck, and this trapped employee between the tire and the frame. EE was diagnosed with four broken ribs and a punctured lung.
While driving loader 600-181 down to the city base pit, the bucket slowly dropped until it dug into the road, bringing the loader to an abrupt stop. EE reported the incident to EE's supervisor after it happened but did not need or want medical attention at that point. On 7 May EE stated that EE's right shoulder was very sore and requested to have it examined.
The miner was cleaning the catwalk along the truss conveyor near the chute transferring material from the overland, when EE was struck in the right eye by a small piece of fly rock.
Employee was shoveling next to tunnel feed conveyor belt when a 2 inch rock bounced off the belt and struck him on the left side of the neck in the soft tissue between the jaw and collar bone. Employee was taken to the hospital to be checked by a physician as a precautionary measure.

INJURED WAS OPERATING A 769. HE ENTERED INTO AN INCLINE IN TOO HIGH A GEAR. THE 769 BEGAN TO ROLL BACKWARD, SIDESWIPED A TREE AND LAYED OVER ON THE PASSENGER SIDE. THE ACCIDENT RESULTED IN MIN OR DAMAGE TO THE 769 AND A CRACKED RIB ON THE EMPLOYEE.
INJURED WAS WITHDRAWING HIS LEFT HAND AFTER TRAINING A TAIL PULLEY ON A CONVEYOR BELT TO INJURED INDEX AND MIDDLE FINGERS WERE PULLED BETWEEN A ROLLER AND CONVEYOR BELT HE SUFFERED CONTUSION O F THE LEFT HAND AND VARIOUS FRACTURES OF HIS LEFT INDEX FINGER
INJURED WAS DRIVING A WATER TRUCK WHEN HE WAS HIT BY A SCRAPER AT SLOW SPEED. INJURED RECEIVED MINOR BRUISES TO THE LOWER PART OF HIS LEFT LEG.
THE 980 CAT LOA8ER OPERATOR WAS MOVING MATERIAL UNDER A 40' QUARRY FACE. HE WAS PARALLEL TO FACE AND 50' AWAY. THE FACE HAD LARGE CRACK BACKS ON TOP NO ONE WAS AWARE OF. IT WAS DANGEROUSLY UNS TABLE THE FACE 7AVED IN BURYING THE LOADER FROM THE RIGHT SIDE AS HIGH AS THE C AB. THE CAB WAS CRUSHED CAUSING BRUISES AND CUTS TO THE OPERATOR. THE LOADER IS TOTALED
