EE was in the manlift helping to repair a guard. The manlift basket got caught on a piece of metal and then jolted loose. When it came loose, the EE fell against the handrail, hit lower back and pinched EE's sciatic nerve. The doctor placed EE on light duty.

Mostrando los 22EE was assisting forklift to turnover cheekplate. The forklift operator got the ckeekplate picked up on one side to about 85 degrees. The EE began to push on the cheekplate. The forks lost tension under the plate, popped up, knocked the EE's hand loose from the plate, and the plate fell back on the EE's left, lower leg causing a fracture of the two bones.
Employee was positioning HDPE pipe in fusion machine. 300' length of 18" pipe was suspended from a trackhoe by a chain & being aligned by a rough terrain forklift. Employee attempted to make a final adjustment in the pipe alignment & placed hand between the pipes. At that time the pipe shifted in the chain & entrapped the employee's fingers. Gloves were being worn.
The EE was moving sand with the 988 loader when the joystick locked into the autoraise position with a full bucket of sand. Due to the incline of the pile, this raised the counter weight and rear wheels. The operator kicked the loader out and this slammed the bucket to the ground. This threw the operator into the ROPS and caused a 3" gash in head and a concussion.
The EE is the Pit Loader operator. EE was digging in a difficult shot and hit EE's right elbow several times throughout the day. EE went to the doctor on 2/11/16. Doctor wrote EE out-of-work until 2/18/16. On 2/16/16, EE went to a specialist, and they said it was a deep bruise. Doctor gave EE medicine, and the swelling and pain went away. EE returned on 2/18/16.
EE was cutting down a tree. They were using forklift to guide the fall of the tree. When they were getting the forklift in position, the forks hit a dead limb, a 1" x 6" piece of the limb fell and struck the ee on the ground in the left eye. Had to have surgery on eye to repair. EE was wearing safety glasses.
EE WAS HAULING DIRT TO THE DUMP AREA WHEN A PIN BROKE IN THE STEERING UNIT CAUSING LOSS OF CONTROL. THE TRUCK WENT OVER THE BERM & ROLLED 1 & 1/2 TIMES LANDING ON ITS SIDE. EE SUFFERED MINOR B RUISES & A CUT INDEX FINGER REQUIRING 4 STITCHES. HE ALSO SUFFERED A STIFF NECK SEVERAL DAYS LATER.
I WAS REPAIRING SHOP LIGHTS. I WAS ON A MANLIFT ABOUT 13'-15'HIGH. I NEEDED TO RAISE THE LIFT ABOUT A FOOT TO GET TO A JUNCTION BOX. WHEN I STARTED TO RAISE THE LIFT IF SUDDENLY TILTED ON ONE END AND I FELL TO THE FLOOR WITH THE LIFT. I RECEIVED FACIAL CUTS & BRUISES, 2 BROKEN RIBS, A BRUISED LIVER, A BRUISED HIP, A BRUISED RT. CALF MUSCLE, AND MY LT. HAND WAS BRUISED.
EE WAS TAKING A LOAD OF ABC TO THE THE PIT FLOORWITH A CAT 980C LOADER WHEN THE BRAKES FAILED. EE JUMPED OUT AND LANDED ON HIS LEFT LEG BREAKING THE BONE(FEMUR). THE LOADER ROLLED ABOUT 40 F EET AND STOPPED AT A BERM. THE BUCKET WAS NOT LOWERED AND THE PARK/EMERGENCY BRAKE WAS NOT ACTIVATED.
THE EE JAMMED HIS BACK WHILE OPERATING A 980C CAT LOADER. NOTE' THE EE TOLD HIS SUPERVISOR HIS BACK HURT AND WENT TO HIS DR THE NEXT DAY. IT WAS NOT UNTIL 8-10-98 THE EE GAVE NOTICE OF AN INJU RY ON THE JOB.
EMPLOYEE WAS OPERATING CAT 988 LOADER WHEN HE STUCK HIS HAND INTO THE FAN VENT IN THE ROOF OF THE CAB TO DETERMINE IF THE FAN WAS WORKING.
FORKS ON FORK LIFT HUNG WHILE LOWERING. EE WAS EXAMINING PROBLEM WHEN THE FORKS DROPPED AND INJURED HIS FINGER. HE HAD PLACED HIS HAND IN AN AREA PLATES CAME TOGETHER.
EE WAS OPERATING A FRONT END LOADER, PLACING STEMMING AT A 5 HOT SITE. EE PLACED HIS HAND ON THE OUTSIDE OF CAB TO LEAN OUT TO INSPECT TERRAIN. AS THE LOADER MAILED INTO UNEVEN GROUND THE DOO R SLAMMED SHUT ON EE'S LEFT THUMB.
AS EE WAS MOVING A RAILCAR UNDER A CONEYOR BELT HE HIT HIS HEAD ON THE CONVEYOR FRAME.
HAUL TRUCK DRIVER-FOREIGN BODY IN EYE DRIVER DOES NOT RECALL HOW OR WHEN OBJECT GOT INTO EYE.
EMPLOYEE WAS DRIVING A HAUL TRUCK WHEN THE RIGHT FRONT "A" FRAME BROKE, THROWING HIM AGAINST THE STEERING WHEEL.
PUTTING ELECTRI7 MOTOR IN STORAGE IN SUPPLY RM WITH BOBCOAT LOADER. MOTION WAS PLACED ON FLOOR AND PUSHED IN PLACE BY HOLDING A BOARD NEXT TO LOADER BUCKET I NAME REPOSITIONING THE BOARD WHEN LOADER LUNGED 0ORWARD AND MASHED HIS FINGERS BETWEEN LOADER BUCKET AND BOARD.
RAILROAD RAIL W5S SUSPENDED BY CHAIN FROM TEETH ON 980 CAT LOADER BUCKET.THE HOOK ON THE CHAIN CAME LOOSE AND ALLOWED THE SLACK CHAIN TO FEED OUT OVER THE BUCKET TEETH CAUSING THE RAIL TO DROP , HITTING THE M5N IN THE LEGS.
EMP WAS RELEASI3G BRAKE ON RAIL CAR. THERE AS TOO MUCH SLACK IN CHAIN AND LEVER ON BRAKE HOUSING WAS IN WRONG POSITION TO PICK UP SLACK IN CHAIR IN TIME TO STOP THE CAR FROM HITTING THE CA IN FRONT. EMP J6MPED MOVING CAR AND SPRAINED HIS LEFT ANKLE
R/R CAR ROLLED OVER EE FOOT ON TRACKS AT SCALE HOUSE WHERE LOADED CARS ARE WEIGHED PRIOR TO SHIPMENT. FACTS NOT ACCURATE AT PRESENT BECAUSE NO WITNESS. EE WAS APPARENTLY ATTEMPTING TO RELEASE COUPLING WHEN AMPUTATION OCCURRED.
AN INSECT FLEW INTO WINDOW OF TRUCK, STRIKING EE IN THE EYE.
EE WAS BRINGING DOWN RAIL CAR TO BE LOADED AND STRAINED A GROIN MUSCLE WHILE PULLING ON THE CAR BRAKE.
