EE ran into a JLG man lift. EE was wearing a seatbelt.

EE states they have back pain from driving a haul truck. EE was placed off work due to not being able to drive to and from work.

Years of driving a haul truck is taking a toll on EE's lower back.

EE was operating the loader and hit a pothole causing back to hurt. EE declined medical attention at first but asked to go to the doctor on 9/19/17.

Employee crashed vehicle on ice after shift on way home.

Over the last 4 years ee noticed back was hurting when operating mobile equipment. Over time the pain became unmanageable and the employee was placed on light duty.

EE was struck by a fan while trouble shooting the starting system of the skytrac forklift.

Employee was vactoring out a plugged chute in the clinker shed and the hose came out of the clay and attached to the right knee. The employee moved away from the hose and fell striking the knee on conduit lines.

Employee was tracking and training a conveyor belt by tapping the return roller hangers with a hammer. The hammer head was grabbed by the roller and pulled the employee's hand to the roller. The right thumb was pinched between the roller nad the belt. The right thumb was lacerated requiring suture repair.

EE was backing water truck & misdirected the truck into a berm at a sharp angle. Truck was full of water & with R-rear wheels up on berm rolled over to left & settled upside down on access road a few feet below. EE was using seatbelt. EE was evaluated by ER on 5/1 & by Occ Med MD on 5/2 and given full release; On 5/12, eval by Occ Med MD resulted in TTD.

V EXPRESS TRK DRIVER WITH FULL LOAD OF BULK CEMENT RAN OFF THE PAVED ROADWAY BEFORE REACHING THE ENTRANCE GATE. TRUCK TIPPED OVER ON L SIDE. DRIVER WAS EXAMINED & TREATED FOR HEAD LACERATION. DRIVER WAS DISCHARGED FROM EMPLOYMENT FOR DRIVING INFRACTION.

Employee attempted to retrieve a large rock from the end of a 6-inch vacuum hose and caught his left arm in the hose. The power was shut off within 30-seconds by a helper nearby. Injury was limited to upper arm soft tissue injury/hematoma.

Employee was operating a loader with a quick-connect bucket attachment. Unknown to the operator, the locking pins for the bucket were released. The employee was dumping material while in forward motion when the bucket dropped off and the right front wheel rode up on the bucket and dropped off. Employee reports using seatbelt, but received multiple injuries.

CARTAGE VEHICLE HAD LEFT PLANT AND ROLLED OVER ONTO ITS LEFT SIDE JUST BEYOND THE FIRST SHALLOW CURVE IN THE ACCESS ROAD ABOUT 0.7 MILE OUTSIDE THE PLANT ENTRANCE GATE. AT 3:00 AM IT WAS DARK BUT CLEAR AND DRY. NO KNOWN OBSTACLES WERE PRESENT ON THE ROADWAY. THE DRIVER'S SPEED IS UNKNOWN.

PRIMARY ROCK STACKER FELL FROM CRUSHER OPERATOR WAS MOVING FROM #1 FEEDER TO #2 FEEDER, FALLING ON 8YD, HITTING RAMS JUST BEHIND THE BUCKET SLAMMING THE 8YD OPERATOR BACK & FORTH IN CAB.

EE WAS DRIVING AND HAULING THE 75 TON HAUL TRUCK. HE NEEDED TO USE THE ESCAPE RAMP REASON UNKNOWN AT THIS TIME. TRUCK WENT HALF WAY UP THE RAM AND BEGAN TO ROLL BACK. TRUCK WENT UP AND SIDEWAY S AGAINST A BERM CAUSING THE TRUCK TO ROLL OVER ON IT SIDE.

INJURED WAS ASKED EARLY IN THE AM TO PICK UP HIS COKE SAMPLES.HE PICKED UP THE ONES ON THE COKE WEIGH BELTS #1 & #2 AND THE ONE FROM THE DELIVERY TRUCK,BUT FORGOT TO GET THE COKE FINES SAMPLE. WHEN THE LAB SUPERVISOR ASKED ABOUT THE SAMPLES,I TRIED TO GET IN TOUCH WITH INJURED,I COULD NOT GET HIM AT THIS TIME BY RADIO,SO I CAUGHT THE OTHER SHIFT HELPER IN THE LUNCH TRAILER AND TOLD

STEPPING OFF LOADING PLATFORM IT BEGAN TO RAISE HITTING L ELBOW

WAS WIRING ON A MOTOR MAINTENANCE PEOPLE WORKING IN SOME AREA USING A FLEX LIFT AND A FREE SWINGING HOOK-THE PERSON GIVING DIRECTIONS TO THE FLEX LIFT OPERATOR COULD NOT SEE I,NAME, AND BELIVE D IT WAS SAFE TO GO IN AND HOOK UP TO THE EQUIPMENT.
